High St. Charlottesville, VA 22902 (j ames@shelterltd. com) Plan received date: 18 Sep 2018 Date of comments: 23 Oct 2018 Reviewers: John Anderson County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any VSMP permit by issuing a project approval or denial. This project is denied for reasons listed below. The VSMP application content requirements can be found in County Code section 17-401. Note: Engineering welcomes (.PDF) preview of rev. plans prior to print re -submittal for final approval. Please segregate /distribute information on current title sheet across several sheets (see SWMP comment 2.). Also, please revise plan title, and include additional general project information (please ref. sample). A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Anticipate addressed via SWPPP. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. Revise Title Sheet (Plan) title to: VSMP /WP0201800071, Stalfort Residence. 2. Title Sheet contains more information than should be presented on a Title Sheet; please distribute details across additional sheets. Recommend Title sheet with additional general information (see Attached recent plan title sheet for unrelated project); ESC Plan /pipes; Site Layout that includes drainage /culvert pipes. 3. Ref. Attached sample title sheet: provide additional, general parcel /zoning information. (Call if questions). Engineering Review Comments Page 2 of 3 4. SW-02: VaRRM .xls lists 16.38 Ac. in SWM Forest /Open Space. Attached sample .xls indicate less acreage in permanent easement as an alternative approach. If consider soil groups A + B, the range of acreage required =4.75 to 5.85 Ac., depending on whether Applicant has applied for a building permit, or not. If Applicant has applied for a single-family dwelling building permit, then drive length beyond existing drive from construction entrance to broad parking area may be excluded as impervious area. Engineering estimates area = 800'(f) X 12' (0.22 Ac.). Engineering has not examined VaRRM .xls post -development project land cover table values beyond this, but it may also be possible to remove portion of turf land cover if shoulder section of new drive is included in the current land cover table and i there is a building permit application on file with Albemarle. Applicant may place 16.38 Ac. in Forest /Open Space Easement; if so, proposed design nets a surplus (3.61 lb.) phosphorus credit, but use of credits may be limited by overall restrictions if 16.38 Ac. is in SWM Forest /Open Space easement. 5. Related to 4, above: SWM Forest /Open Space Easement plat with Deed of Dedication is required prior to VSMP Plan approval. Note: Less acreage in easement may ease somewhat the expense of site survey required to prepare easement plat to accompany a Deed of Dedication. Engineering will format a Deed of Dedication template for Applicant, once SWM Facility Maintenance Agreement is recorded (please see Process section, below). 6. Level spreaders (LS) used to trim velocity prior to discharge to SWM Forest /Open Space are an element of a stormwater best management practice (VA DEQ Stormwater Design Specification No. 2: Sheet Flow to a Vegetated Filter Strip or Conserved Open Space); for this reason: a. Include VSMH (2013) 9-C.3.0., Sheet Flow to Vegetated Filter Areas and Conserved Open Space: O&M Checklist (Attached). b. Include following Note on Plans: `Construction of the gravel diaphragm or engineered level spreader shall not commence until the contributing drainage area has been stabilized and perimeter E&S controls have been removed and cleaned out.' (BMP Clearinghouse VA DEQ Stormwater Design Specification No. 2; pg. 17). c. Provide (Typ.) LS detail: include Fig. 2.6 (Clearinghouse Spec. No. 2; pg_8). d. Provide LS dimensions (L X W X H) and material type: LS 1, 2, 3, 4, to aid construction and (future) compliance inspections. e. Include plan Note acknowledging SWM Facility As -built requirement/s, including Construction Record Drawings (each LS). f. SWM Quantity Control: provide energy balance equation/s for proposed development, each outfall point — please see 9VAC25-870-66.B.3. 8. SWM Facility Maintenance Agreement and a Deed of Dedication of SWM Forest /Open Space Easement (with Easement Plat) are required prior to VMSP Plan approval. 9. SWM Plan and ESC plan bonds are required prior to project receiving a grading permit. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved for reasons listed, below. The erosion control plan content requirements can be found in Engineering Review Comments Page 3 of 3 County Code section 17-402. 1. Label ditches upslope of Culverts 1, 2, 3, 4 as Ditches 1, 2, 3, 4, for ease of reference during construction inspection. 2. Recommend labels /leader lines (plan view) to clarify that Ditches 1, 2, 3, 4, are to receive American Excelsior Curlex 1 Matting. 3. Provide check dams as an ESC measure between culvert pipes; ref. VESCH, 3' Edition, 1992, for technical and design details (check dams, a temp ESC measure, will be removed once upslope areas are stabilized). 4. Revise Culvert 4 data on current title sheet (possible cut -paste error). 5. Additional comments possible once ESC Notes /design /measures shown on a separate plan sheet. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. (.PDF preview welcome). Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. Stormwater Management Facility, SWM Facility Access, Forest /Open -Space Easements must be recorded prior to final approval. This means a Grading Permit may be issued prior to recordation of VSMP-related easements. After bonding and agreements are complete, Albemarle County can hold a pre -construction conference. Applicant will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
