This plan is approved. 1. Please revise title to include VESCP /WPO1201800076. (Rev. 1) Addressed. 2. CO. 1, Topography Source Note: Please revise by appending `as required by Albemarle County.' (Rev. 1) Addressed. C0.1 3. Construction Sequencing Note 9: Revise to read, `Remove perimeter control measures, if and once approved by county inspector.' (Rev. 1) Addressed. 4. Construction entrance (CE) location conflicts with proposed 3 15" DIA RCP lines. CE location must work in phase I and Il. Relocate CE to avoid conflict. (Rev. 1) Addressed. 5. Approximate Ex. septic tanks are shown. Show approximate location of any Ex. septic lines leading to septic tanks, unless this is a RV septic discharge station; that is, a buried tank without lines. (Rev. 1) Addressed. 6. Label sediment trap floor dimensions, L X W. (Rev. 1) Addressed. 7. Provide `horseshoe' stone CIP (Plate 3.08-1, C1.3) at 3 — 15" DIA RCP, Phase 1. Note /label recognizes contractor will need to flush pipes clean. A stone /gravel filter CIP is required while ST -1 active, Phase 1. (Rev. 1) Addressed. Cl.l 8. Label contours that lie outside LOD. (Rev. 1) Addressed. 9. Add TS, MU symbols (temporary seed, straw mulch). (Rev. 1) Addressed. 10. Provide additional plan /profile CIP scale drawing detail, with dimensions. Plate 3.13-2 provides typical but not specific detail, while adjacent image on C1.3 fails to provide stone filter which is the only mechanism to limit direct discharge of sediment -laden storm runoff under proposed design (i.e.: there is no dry storage; it is all wet storage. When stored volume in sediment trap reaches pipe INV elev., 622.16', there is no sediment trapping feature. There is also potential, without dry storage, of subsequent runoff exiting immediately through the 3 — 15" DIA RCPs.) (Rev. 1) Addressed. 11. Recommend provide spot elevations at inlet 3- 15" DIA RCP (Rev. 1) Addressed. Engineering Review Comments Page 2 of 3 12. C1.3, Sediment trap details and calculations: (Rev. 1) Addressed. a. Label ST floor elevation. b. Label 3- 15" DIA storm line INV IN C. Show stone filter at inlet of 3 -15" DIA RCP pipes. (item #10, above) C2.0 13. Triple line of 15" DIA pipe discharges to Ex. 24" CMP at an angle. Provide poured concrete wall to ensure storm runoff makes this turn, or else align pipes. (Rev. 1) Addressed. Note: A concrete wall would impede inflow from ditches to the north and southwest; contractor will shape riprap bowl to direct storm runoff from triple line pipe outfall to twin line pipe inlet. 14. Pipe -carrying capacity of Ex. 24" CMP is less than combined carrying capacity of 3 -15" DIA RCPs. Provide additional 15" CMP, or replace Ex. 24" pipe with 30" pipe, or propose design that ensures triple line of 15" pipe does not discharge to pipe with less capacity. Project need is driven in part by saturated conditions /poor drainage, which may indicate that the Ex. 24" pipe is undersized; in any event, proposed triple line may not discharge to a single 24" line. (Rev. 1) Addressed. 15. If additional 15" line is added in parallel with existing 24" CMP, recommend locate north of 24" pipe such that majority of flow is first directed into the 24" line, with any excess flow passing to the 15" pipe. The 15" pipe may even have a slightly higher INV elev. (0.25-0.5'). (Rev. 1) Addressed. 16. Provide ES -1 at both inlet and outfall of Ex. 24" CMP to limit erosion of Misty Mountain Road embankment. (Rev. 1) Addressed. VDOT EW -6 provided. 17. C2.1: Provide scale profile for 3 - 15" DIA RCPs. (Rev. 1) Addressed. 18. C2.1, Site Section 3: Label slope at edge of regraded field (slope varies, but not at location of section detail). (Rev. 1) Addressed. 19. C2.2, outlet protection table: Revise apron width upstream to 13.25' since there is 1' separation between lines. Revise apron width downstream to 17.25'. (Rev. 1) Addressed. Process After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. After bonding is complete, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid ($300, in this instance; 2nd plan review). This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a grading permit will be issued by the County so that work may begin.
