Request included non-destructive results of concrete testing, multiple inspection reports, and asphalt core sample depth reports for Watervale Drive, and Oxbow Drive. Please note: Shimp Engineering Chesterfield Landing — As - built Road Plan comment response letter d. 12/13/18 and As -built drawing (rev. 12/13/18) indicate VDOT CD -1 cross -drains (shown on profiles: two (2) on Watervale; three (3) on Oxbow], underdrains beneath CG -6 (gutters/both roads), and a 15" DIA STM pipe (C5A - C513), were not installed. Albemarle County expectation is cross -drains and STM pipe, C5A - C5B, are to be installed prior to acceptance of Watervale or Oxbow Drive into the state secondary system of highways. VDOT may stipulate additional requirements relating to these or other items as condition of roadway acceptance. Please advise Albemarle and VDOT concerning timeline/s for remedy of cross -drains, and STM pipe. This bond is currently valued at $455,080. Based on inspections (1/16/19) and review of submitted documents, this bond can be reduced to be $152,400.(Note. 20% of original value of bond =$152,316) Please see attached spreadsheet which lists bonded items, and % completion. VDOT acceptance is required for bond release. Albemarle does not reduce bonds to less than 20% of original value of bond, prior to road acceptance. Do not show cul-de-sac (S. end Oxbow); sidewalk (multiple locations); CG12 ramps (3 locations); 15" HDPE storm pipe (Lot 8 /6" DIA installed); cross drain (under streets); or landscape trees where none, other, or fewer exist. As -built drawings correspond nearly exactly with constructed conditions, by definition, with misalignments both minor and rare. In this instance, there are major discrepancies. 2. Provide as -built and design stone depth and pavement depths (sub -base, stone, base /surface mix asphalt) on the As -built drawings. 3. Revise C5A-0513 profile (sheet C6) to show as -built diameter, length, and slope. Revise C4 (plan view) to show correct location of 6" storm line between C5A-05B. (Note: 6" STM line is unacceptable.) 4. Revise STM Al -A4 profile (sheet C6) to show as -built diameter —re£ video image, below: 15" DIA. Also, pipe video /other (below). 5. Show the DB & PG for all easements, and ROW on As -built drawings. Related /BOND REDUCTION ACTION ITEMS: 6. C6: Storm Sewer profile STM C5-0513 lists 15" HDPE pipe. A 6" line was installed. Remove 6" line. Install 15" STM pipe per approved Road Plan. 7. Install CG12 ramp south of driveway, Lot 12. Home site builder appears to have shifted driveway location. Accept VDOT guidance. Relocate landscape tree prior to constructing ramp. See photo. 8. Revise CG12 ramp at Crozet Ave. - Watervale Drive entrance. Remove concrete and Ex. truncated domes, as needed, to reconstruct per design /VDOT standards. Reorient truncated domes to align perpendicular with (future) receiving ramp to be constructed on south side of Watervale Drive. The ramp and domes are oriented on an angle, which may misdirect pedestrians into Crozet Ave. 9. Construct receiving ramp on south side of Watervale Drive at Crozet Ave. 10. Construct two (2) CG -12 ramps (shown on As -built, yet unbuilt) on Oxbow Drive. 11. Provide stop bar at stop sign approaching SR 240. Provide any other required pavement markings. 12. It appears the Cross Drains indicated in the plans/asbuilts are not outletted to drainage structures or EW 12 per VDOT design standard. If they were not installed, the asbuilt should indicate "Not Installed" (f this isn't a huge concern per VDOT comment). VDOT will be monitoring the pavement in these areas during the two-year warranty period. Field Visits (8/28; 9/20/2018) /BOND REDUCTION ACTION ITEMS: 1. Replace damaged concrete marked with field paint. Replace intersecting cracks in front of Watervale Drive DI (may be unmarked). Replace concrete damaged by future construction, for final acceptance. Note VDOT 45 -day punchlist relative to concrete repair. Ref. photos provided with checklist. 2. Mill /pave surface mix asphalt near SM Charlottesville model unit, Watervale Drive (half roadway width X 95'(f)). Replace asphalt damaged by future construction, or roll on -roll off dumpsters. Note VDOT 45 -day punchlist relative to required mill -pave section. Ref. photos sent with county checklist. 3. Signs a. Review height to bottom edge of signs (7', minimum). b. Replace stop signoast at Crozet Ave and Oxbow Drive, to obtain Min. ht. to bottom edge of sign, and to provide 16" length above stop sign, for VDOT to mount state route number blades. Note VDOT 45 -day punchlist. Ref. photos. Engineering /Road Plan Bond Inspection Checklist Page 3 of 6 c. Ensure all sign posts are vertical. d. Provide sign mounting bolts that pass through sign post; secure per VDOT spec. STP -1 (washer /nut). e. Provide sign post base that meets VDOT material and break -away requirements. f. Ensure nearest edge of all signs is at least 2' behind face of curb. 4. Remove 20 MPH speed limit signs, per VDOT request (45 -day punchlist). Install one (1) 25 MPH sign upon entering the development. 5. Complete landscape tree plantings. 6. Clear entrance storm pipe outfall ditch line north of Watervale Drive and Crozet Ave. intersection. Ditch line is overgrown and requires maintenance prior to acceptance. (Photo /punchlist) 7. Storm Drains (DIs), ref. photos /VDOT punchlist: a. Clear inlets throats /MH bottoms of debris. b. Remove inlet gutter buddy near SM Charlottesville model unit. c. With future development, ensure MH lids are at grade, per design, not elevated (re£ photo). d. Seal gap at DI throat, within DI. Gap does not match VDOT DI Std. section views. A tight, mortared, or caulked gap is acceptable. e. Seal /parge MH thru-wall penetrations (openings) if not connected to a pipe. Remove foam seals used in several DIs to seal openings; instead, use VDOT-approved grout, mortar, or approved method. £ Inspect all DIs /MHs and compare with plans, and VDOT specifications /standards. g. Provide brick /mortar around annual space (DI on Watervale). h. Provide crack control joints at inlet throats, per VDOT Std. i. Repair intersecting cracks (gutter pan at throat one DI). 8. Lower elevated water valve lid in Watervale Drive, so flush with pavement (VDOT punchlist /photo). Other /BOND REDUCTION ACTION ITEMS: 9. Two (2) CG12 ramps on Oxbow are not constructed. Construct all CG12 ramps for final acceptance. 10. Coordinate with VDOT on 45 -day punchlist (Watervale and Oxbow Drives). 11. Provide (copy of) Chesterfield Landing sign post agreement. 12. Provide (copy of) HOA landscape /sidewalk easement. Prior to VDOT acceptance, a VDOT landscape maintenance permit and the sidewalk agreement will need to be submitted. Coordinate with VDOT. 13. Provide copies of recorded plats. 14. Provide PE -seal letter that attests construction meets design; it may be premature to submit such a letter given range of checklist items, but PE -certification is required for bond reduction. SM Charlottesville letter, d. 8/9/18, states `we'll discuss this deliverable for a release,' misstates this requirement: a licensed engineer must certify improvements to date meet design (and standards), else Albemarle cannot reduce a subdivision bond. Albemarle does not typically inspect drainage or pavement construction items, which underscores the need for PE -certification (required for bond reduction, confirmed by County Engineer). Pipe video /BOND REDUCTION ACTION ITEMS: a. Ref. F&R email, 9/18/2018 3:30 PM (Emily Dean), re. Pipe Summary Report, Item #12: "We did not witness any repairs near/between Structures A2 or Al." Pipe Summary Report Sheet (table) identifies 1.25" gap. Repair this gap. Note any VDOT 45 -day punchlist item/s relative to summary sheet item #12. b. Provide internal pipe inspection video of repairs documented by F&R reports. c. Provide internal pipe inspection video of 1.25" joint separation repair (item #12). d. Discrepancy: A2 -A1 video label lists 15" diameter (which is design). As -built lists 18". e. 1.25" separation: Coordinate with Tri-State to identify precise location, and repair gap. Reviewing video, gap is likely 3.6' or 23.5' from MH A2. If 23.5', it appears that no concrete demolition is required to access this 15" pipe located near the subdivision entrance at Crozet Avenue. Utilizing the nuclear test method to check the compaction of stone; test results indicated that the compacted material, at the areas and elevations tested DID NOT meet the project requirements of 95%v of the maximum dry density as obtained in our laboratory using the Standard Proctor Method (ASTM D-698). The stone observed on this date appeared to be placed in accordance with project drawings and specifications with regard to lift thickness and moisture content. 2. The undersigned also checked the stone depth at each test location and each location met the minimum requirements of 8 inches with the exception of one measurement of 7" at station 13+00. 16. As requested, the undersigned arrived on site at Chesterfield Landing in Crozet, Virginia, to observe: The placement and compaction of 3 inches of BM -25 asphalt for Roadway Entrance. Please see the attached sketch. Utilizing the Nuclear Method (ASTM D 2950) to check the compaction of the asphalt; test results indicated that the compacted material, at the areas and elevations tested did meet or exceed the project requirements of 98% to 102% of the maximum wet density as obtained by using the Control Strip Method. The asphalt observed on this date appeared to be placed in compliance with project specifications with regard to lift thickness, lay down temperature, and number of roller passes as determined by using the Roller Pattern Method, 19. Given ECS non-compliant stone compaction results for Watervale Drive between entrance to site at Crozet Ave. and Sta. 15+50 (f), 7/25 — 7/27/2016, with base asphalt (3" BM -25) placed on 7/28/2016 without report that non-compliant stone base was addressed, Engineering is unable to reduce bond for stone base or pavement items for Watervale Drive. Remedy appears necessary prior to bond reduction. Albemarle requests constructive (or test) proposal to ensure Watervale Drive stone base compaction meets design specification prior to bond reduction of stone or pavement items on Watervale Drive. Pavement design information provided to VDOT /County Engineering via email (9/24/2018 1:03 PM): Calculation using CBRT =3.6; CBRD =2.4 (ADT =580) confirms pavement depth design is adequate. Oxbow Drive: DP =13.725; DR =13.618. Albemarle requests inspection report/s to confirm that As -built intermediate (IM -19.0) and asphalt surface mix (SM -9.5A) depths meet design (3'; 1.5", respectively). If asphalt depth inspection reports overlooked, please contact reviewer. If reports are unavailable, arrange to core Oxbow Drive in several locations, and submit core results to county prior to bond reduction of asphalt pavement items. 21. Pavement design information provided to VDOT /County Engineering via email (9/24/2018 1:03 PM): Calculation using CBRT =45.7; CBRD =33.3 (ADT =750) confirms pavement depth design is adequate. Watervale Drive: DP =13.725; DR =10.62. Albemarle requests inspection report/s to confirm that As - built asphalt surface mix (SM -9.5A) depth meets design (1.5"). If asphalt depth inspection report/s overlooked, please contact us. If report/s unavailable, arrange to core Watervale Drive in several locations, and submit core results to county. 22. Albemarle requests stone depth report for Oxbow Drive prior to bond reduction of aggregate base (VDOT 21A stone). If observational report/s overlooked, contact Engineering. If report is unavailable, please arrange to core Oxbow Drive to depth sufficient to report stone depth, and submit report to county. 23. Provide concrete test reports for sidewalk, curb and gutter. If concrete field inspection reports, cylinders, or cylinder break reports are unavailable, Albemarle requests a non-destructive test proposal to provide results for comparison with concrete design standards, prior to bond reduction of concrete items.
