Present: Justin Shimp, Shimp Engineering; Joel Kauppila, VDOT.
The bullet number and issue correspond with the attached photo sheet and plan sheet sketch. Le. area
1 highlight several issues near the entrance. Issues from previous 45-day lists are also included.
1. Entrance area issues
a. Curb and gutter at structure was installed improperly. No dowels and no crack control
for 50'. Remove and replace with minimum 6' segments at cracks/failures.
b. Tripping hazard along sidewalk, fill in with topsoil/sod.
c. Sidewalk cracked in several areas as indicated on plansheet.
2. Several issues in future entrance area, just north of Trailside entrance. Per Justin Shimp, these
will be addressed with the future entrance permit.

b. Exposed fiber optic
c. Inlet with Erosion Control
d. Rutting
3. Sidewalk Joint at #535. Grind down per specifications or remove and replace ADA non-
compliant joint that is greater than %".
4. Construction Material and Activity along roadway. Drop off behind curb needs to be backfilled
and grassed w/sod or seed and mulch.
5. Sidewalk cracked at #543.
6. Asphalt Pavement cut out just west of Rte. 20 — restore pavement per VDOT special provision
trench restorations detail.
7. Grass not established / damaged including drop off behind curb.
8. Speed limit sign hidden by tree and dented. The speed limit sign will need to be relocated
AFTER the VDOT speed study is conducted after acceptance. The sign will most likely need to be
150' from Route 20, with an additional plaque indicating "UNLESS OTHERWISE NOTED".
a. Drop off behind curb, construction material within roadway prism.
9. Address tree blocking stop sign. The tree should be relocated or removed.
10. Gutter pans at CG 12's need to be 5% max per ADA and VDOT standards.
11. Repair any pavement damage due to track equipment and ongoing construction. Asphalt has
settled near valves which are not flush or slightly below the surface (think plow trucks).
12. Install one way sign panel on correct side of post. Please note — a right one way arrow will need
to be fabricated.
13. Cracked sidewalk outside of R/W but along the accessible route.
14. Remove non-standard items off of the R/W or apply for a perpetual permit.
15. Check all signs for proper fastening, shown street name sign is very loose.
16. General — remove all unnecessary erosion control, clean drainage and roadway, repair any
damage present.

17. Ongoing Construction — permits are required for any work utilizing the R/W construction after
acceptance. All R/W laws must be followed once road is taken into the VDOT system. Each
individual lot will need a permit with a $3000 surety.
18. Striping — striping is faded and does not meet retroreflectivity, install thermoplastic per VDOT
specifications.
19. Landscape Maintenance Agreement and Land Use Permit needs to be submitted.
20. At end of trailside, verify PVC pipe installed is not broken or compromised. This type of pipe is
not allowed with in VDOT R/W.
21. Verify Outlet protection missing on previous punchlist has been installed.
22. All roads need to be swept of silt, rocks & debris
