GENERAL NOTES
1, Polo Grounds Road Improvement Plans shall be submitted as a separate project.
2. A video camera inspection is required for all storm sewers and culverts that are
deemed inaccessible to VDOT inspections and are to be accepted into VD(OT's
maintenance program and ownership. The video Inspection shall be condulcted In
accordance with VDOT's video camera inspection procedure and with a VDIOT and/or
County inspector present. In addition, if high-density polyethylene (HDPE) pipe or pipe
system is installed, the contractor and a representative from the pipe malnufacturer
should hold a pre -construction meeting for installation training. The pipes manufacturer
should spend a minimum of 2 hours on the job site during the initial pipfe installation."
3. Energy Dissipating Outlet Protection Shall be installed at the end of nem stormwater
conveyance channels.
4. All utility and drainage easements shall be dedicated to public use.
5. All landscaping to be planted within Public Right Of Way must be approved by VDOT
prior to planting.
