[] Sweep the road of loose stones and debris prior to final inspection. ❑ All post larger than 4"x 4" shall be drilled with a hole configuration equivalent to VDOT standard 602.3 ❑ Mailboxes are to be 18"from the edge of pavement to the post ❑ Mow all areas within the right-of-way ❑ Grade all shoulders and ditch lines to the approved plan cross section. ❑ Inlet and outlets of all culverts are to be graded and cleared of debris to provide positive drainage. All bent or damaged culverts to be repaired or replaced ❑ Need Permits and Quit claims of any companies with facilities in the right of way Contact the utility companies who will complete and submit forms to VDOT. D Need Two normal size copies of the recorded subdivision plat for this section. ❑ Need Two copies of the recorded plat and as-Built plans 8.5 inches by 11 inches. D Need Two copies of As-Built Plan for this section. Note: Official Inspection is valid for 45 days. Street: Field Creek Lane AS-BUILTS and PLATS to be reviewed for easements, road centerline, grade to see if other problems are in need of adjustment. Will be reviewed and comments provided if there are adjustments to be made. These items will be updated on punch list and resent after review for correction if needed. AS-Bunts are Required to show existing grades, planned grades and finished_grades of roadway along with drainage items and easements. 1) ReconstructfRegrade the roadway per approved plans at entrance area station 10+12 to approximately station 13+50 and approximately station 22+50 and 26+50 and culdesac. Also regrade and repair shoulders and ditches to establish grass in washed areas. 2) Guardrail to be placed at end of culdesac. 3) Updated as-builts needed and plats. 4) Additional drainage easments may be needed based on updated as-builts. 5) Repave entire road with a 1" overlay of SM-9.5A. 6) Change stop sign to 30" x 30"with new reflective sheeting per MUTCDNDOT spec. 7) Have current test on road, but will need new test for reconstructed areas and new overlay for subgrade, stone and asphalt per VDOT spec. Street: AS-BUILTS and PLATS to be reviewed for easements, road centerline, grade to see if other problems are in need of adjustment. Will be reviewed and comments provided if there are adjustments to be made. These items will be updated on punch list and resent after review for correction if needed. Street: Misc Items ALL STREETS 1)All stop signs will need to be 30" with 16"above the top of stop sign for VDOT route markers to be placed and 7' from the bottom of stop sign to the ground for pedesrtian head clearence. 2)All speciality signs, plantings, irrigation equipment that is in the right of way will need to have a maintneance agreement with a permit to keep in place and will need to be completed after the road officially is in the state system. 3) Other problems that may exist or have occurred since thie review that is not on this 45 day punch list will be added. Metal Post for State Signs will have a 7 gauge base and 10 gauge post. Wood post 4x4 can be used as well. Street: Mandatory Testing Needed 1) All Subgrade Density Test and CBR Test NEE[ 2) All Roller Patterns and Density Test for#21-A or B Stone. NEED 3)All Roller Patterns and Density Test for Sm-9 5A. Need 4)All Density test for Fills. NEED 5)All Pipe Backfill test reports.NEED 6)All CD and UD types and sizes will be required prior to final overlay to have a video camera inspection to check for problem GENERAL NOTES: All asphalt repairs will be milled and paved at the existing grade of planned roadway design. Any se- ction not marked during this review does not relieve the developer of the responsibility of other places that have settled or intersections that have been overlayed above the designed grade of the remaining sections of roadway that have not been disturbed for open cuts and overlayed with out being milled and paved properly. Developer will review and call if futher review of a section or spot in the field is needed from VDOT to be marked between the final inspection. Notes: All test retorts for Subgrade Test, Asphalt and Stone density and roller patterns and structure backfill test, fill reports CHECK AND GRADE ALL AREAS AND MAKE SURE THEY WILL DRAIN PROPERLY. STABILIZATION MATTING OR RIP RAP SHALL BE USED TO STABILIZE WHEN NEEDED. ALL AREAS SHALL BE STABILIZED WITH GRASS GROWING BEFORE FINAL ACCEPTANCE. All CBR reports for all roads with a asphalt design attached will need to be sent in as well prior to paving. Trim grass around guardrail as well. 5) First pipe on left needs to be put in to VDOT PE-1 standard. 6) Clean around inlet and outlet ends of pipe and re-establish grass for stabilization or rip rap area. To Include Sycamore Creek Circle as well. Street: Sycamore Creek Circle AS-BUILTS and PLATS to be reviewed for easements, road centerline, grade to see if other problems are 4 in need of adjustment.
