County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any
VSMP permit by issuing a project approval or denial. This project is denied for single reason listed, below
(LOD to match). The application may be resubmitted for approval if all comment items are satisfactorily
addressed. Engineering welcomes .PDF preview.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain
(1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. Link to County template:

a. Revise cover of SWPPP to reference WP0201800093. (Rev. 1) Addressed.
b. Sec. 4/5: Include with final submittal. (Rev. 1) Not addressed. Since only slight revision required
please submit two (2) 11" x 17" print copies of ESC and SWM plan sheets to insert in SWPPP at
your convenience, once revised. Engineering is able to help with this (slip -sheet).
c. Sec. 6 /Exhibit: (Rev. 1) Addressed.
i. Provide /show a trapping measure for treatment of vehicle wash waters.
ii. Provide /show lined concrete washout adjacent to PCE.
d. Sec. 6E: List persons responsible for pollution prevention practices; name individual/s. (Rev. 1)
Addressed.
e. Sec. 8: Identify, by name, qualified personnel responsible for SWPPP inspections. (Rev. 1)

f. Sec. 12: Provide inspection logs. Feel free to contact Engineering for sample logs. (Rev. 1)

B. Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17-404.

C. Stormwater Management Plan (SWMP) (Rev. 1) Prior review comments addressed; plat review in process.
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is
approved.
1. C0.0: Revise plan title to include `VMSP/WPO# 201800093.' [Senior Sunrise Living, or Sunrise Senior
Living?]
2. C0.0: Revise index to include Landscape Plan. Include Landscape Plan listing bioretention basin plantings.
3. Provide SWM Facility and Access Easement. Provide Deed to accompany easement. Feel free to contact
plan reviewer to discuss. (Rev. 1) SLTB201900025 /Easement plat under review.
4. Provide Construction Record Drawings As -build for VSMP on the plans; i.e., list information required on
construction record drawings. Recommend transfer .PDF to plans; link to .PDF:

8. C3.0: See Step 1, Bioretention Installation, C5.2, and provide Note designed to limit construction traffic
and activity in area designated for bioretention basin. (Rev. 1) Addressed; Note 12, sheet C3.0: 'Utilize
protective construction matting over its (bioretention basin) footprint throughout construction.'
9. C3.0: Show protective barrier/s that prevent traffic in area designated for bioretention.
10. C4.0: Label bioretention approx. floor dimensions (L X W); design SA = 463ft'.
11. C4.1: Str. 4 top listed=571.00, while C5.2 lists top=571.35. Please reconcile.
12. C5.1: Recommend include MH step detail.
D. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan
is approved pending minor revision: SWPPP Registration Statement Area to be disturbed (0.87 Ac.) must
match plans (sheet C0.2, Part 1, Erosion and Sediment Control Narrative, 2"d paragraph — 0.82 Ac.).
Please revise for consistency.
1. C2.0: Show a paved construction entrance (PCE), distinct from CRS. (Rev. 1) Addressed.
The VSMP permit application and all plans may be resubmitted for approval when all comments have been
addressed. For re -submittal, please provide 2 copies of the permit package with a completed application form.
Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss
this review.
Process
After approval, VESCP plan will need to be bonded. The bonding process is begun by submitting a bond estimate
request form and fee to the Department of Community Development. One of the plan reviewers will prepare
estimates and check parcel and easement information based on the approved plans. The County's Management
Analyst will prepare bond agreement forms, which will need to be completed by the owner (or ACSA contractor,
if contractor will post ESC Plan bond) and submitted along with cash, certificates or sureties for the amounts
specified. The agreements will need to be approved and signed by the County Attorney and County Engineer.
This may take 2-4 weeks to obtain all the correct signatures and forms.

After bonding is complete, the County can hold a pre -construction conference. Applicant will need to complete
the request for apre-construction conference form, and pay the remainder of the application fee ($00). The form
identifies the contractor and responsible land disturbed, and the fee remaining to be paid. This will be checked by
county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At
the pre -construction conference, should everything proceed satisfactorily, a grading permit will be issued by the
County so that workmaybegin.
