County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any
VSMP permit by issuing a project approval or denial. This project is denied. The rationale is given in the
comments below. The application may be resubmitted for approval if all of the items below are satisfactorily
addressed. The VSMP application content requirements can be found in County Code section 17-401.
A. General
1. Show Amendment 2 in title next to WPO number.
(Rev.1) Addressed.
B. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must
contain (1) a PPP, (2) an ESOP, (3) a SWMP, and (4) any TMDL measures necessary.
1. Please submit an updated Registration Statement.
(Rev.1) Not addressed. Did not receive R.S.
2. (General) The SWPPP should be updated in the field with the latest approved amendment.
(Rev.1) Update sections with the approved amendment changes.
C. Pollution Prevention Plan (PPP)
The PPP content requirements can be found in County Code section 17-404.
1. Update the PPP to show any proposed landscaping operations, staging, stockpile, borrow,
concrete washout locations on the PPP. Also, show any locations where concentrated stormwater
discharizes to and from site.
(Rev.1) Partially addressed. I just ask the PPP be shown separate from the WPO as a
standalone sheet to be inserted in the SWPPP. Show underlined above.
D. Stormwater Management Plan (SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP.
This plan is disapproved, and the reasons are provided in the comments below. The stormwater
management plan content requirements can be found in County Code section 17-403.
1. Sheet C-600: Show propose Swale/ditch contour elevations. (Rev.1) Not addressed.
Show top of wall elevations. (Rev.1) Addressed
2. Sheet C-610:
a. Correct typo `Drainage Area STR-7A' should just be STR-7.
(Rev.1) Addressed.
b. STR-7 - The allowable spread in a sump condition shall be under 10'.
(Rev.1) Please leave in the grate inlets in design table LD-204.
c. You should design the storm structures to handle the expected volume, i.e. STR-5A
should be designed to handle phase 2. You're pipe calc. tables show 0.43ac for Str.
5A. Same for STR-8 & it's outfall.

d. Show HGL in the stormwater profiles.
(Rev.1) Addressed.
e. Show end sect;-ns (ES-1) or endwalls (EW-1) on all pipe outlets.
(Rev.1) Addressed.
f. The velocity out of STR-1 is 11.73 fps. Flow velocities in excess of 10 fps should be
avoided (abrasion). {VDOT)

3. Sheet C-611: The required storage volume according to the post-dev 1-yr hydrograph is 26,136cf.
It was 21,000cf on the approved plan. Provide supporting calcs/analysis of the post-dev. peak
discharge. Provide a non -erosive level spreader design. The design of the facility should meet the
10-yr design and outfall to natural channel. Need to verify that the 1% area is in the Moore's
creek watershed. The limits of analysis for channel protection may need to be analyzed, and if
energy balance not met must be determined if adequate. Let's meet to discuss.
(Rev.1) It appears that water quantity is met with Energy Balance; narrative updated. Volume
of the facility has been corrected now. Level spreader calculations have been added.
Callout/label level spreader location & include a detail.
4. Nutrient credit agreement and purchase has already been completed. State the amount
purchased (3.05 lb/yr) and the remaining available for the site.

5. Please provide VRRM excel file. What area are you accounting for as `D.A. A'?

6. Provide concrete inlet shaping (IS-1) specified on any stormwater structure with a 4' or greater
drop.
(Rev.1) Partially addressed. Add note about IS-1 on sheet C-610, and/or detail.
7. Standard SL-1 Safety Slabs shall be required as part of the drainage design for manholes, junction
boxes and drop inlets with heights greater than 12 feet. On structures whose vertical height is 12'
or greater and Safety Slabs are not specified, the use of bolt down or lock down covers or grates
should be considered. The drainage descriptions should specify how many safety slabs are needed
for each structure and the quantity should be noted in the remarks column on the Drainage
Summary. {Vdot Drainage Manual, 9.4.5.2.21
(Rev.1) Add a note to the drainage str. table (C-610) where SL-1 & IS-1 is required.
8. The following General Note should be included on the General Note Sheet (Vdot Drainage
Manual, 9.4.5.2.2):
D-18 St'd. SL-1 Safety Slab locations are based on the assumed use of precast
structures. If cast -in -place structures are utilized, and the interior chamber dimensions
(length and width, or diameter) are less than 4 feet, the safety slabs shall not be installed.
(Rev.1) Partially address. Please include the note above on sheet C-610, and/or detail
showing use of safety slabs in MH str.
9. Include this checklist on the plan set.

E. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP.
This plan is disapproved, and the reasons are provided in the comments below. The erosion control
plan content requirements can be found in County Code section 17-402.
1. Show existing conditions sheet. Refer to SW3 of the approved plan.
(Rev.1) Addressed; Easements shown on sheet C-6.12.
2. E&SC sheets: Show phase 1 limits of disturbance and area.
(Rev.1) Addressed. Disturbed area is 5.71 ac, no change.
3. Sheet C-501: Show OP at rnua vi u.v rrsion near building Tl 2 or extend to trap.

4. Sheet C-510:
a. The weir lengths should be 12'.

b. Provide check dam detail.
(Rev.1) Addressed.
5. Provide low maintenance (not grass) ground cover specified on the plans where
grades are steeper than 3:1; Show shaded/hatch area(s). {DSM, Sect 8A)

6. Sheet C-600: Show B/M symbols for the long slope — Treatment 2 (EC-3) for slopes

The VSMP permit application and all plans may be resubmitted for approval when all comments have
been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package
with a completed application form.
Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to
discuss this review.
Process:
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond
estimate request form and fee to the Department of Community Development. One of the plan reviewers
will prepare estimates and check parcel and easement information based on the approved plans. The
County's Management Analyst will prepare bond agreement forms, which will need to be completed by
the owner and submitted along with cash, certificates or sureties for the amounts specified. The
agreements will need to be approved and signed by the County Attorney and County Engineer. This may
take 2-4 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded.
The County's Management Analyst or other staff will prepare the forms and check for ownership and
signature information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ
database for state application processing. DEQ will review the application information based on local
VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid
directly to the state. For fastest processing, this is done electronically with the emails provided on the
application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the
application, they will issue a permit coverage letter. This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference.
Applicants will need to complete the request for a pre -construction conference form, and pay the
remainder of the application fee. The form identifies the contractor and responsible land disturber, and the
fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction
conference will be scheduled with the County inspector. At the pre -construction conference, should
everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that
work may begin.
