Number or location or detail of: • CBR tests performed • Stone base inspection • Pavement inspection Engineering /Road Plan Bond Inspection Checklist Page 2 of 3 • Pavement roller pattern • Pavement material • Pavement depth • Pavement design (As -built) • Concrete tests • Concrete reports (date, time, temp, air, slump) • Pipe video are insufficient to reduce subdivision bond at this time. 2. While Oxbow Drive is a limited fill section roadway (Sta. 24+00 — 25+00), ASTM D-6938 test performed 3/5/18 showed insufficient compaction. While test repeated 3/6/18 showed compaction met 95% of Max. dry density std, there is no other description or information on number of lifts, equipment used, or remedial steps taken to address 3/5/18 failure. 3.There is a single ASTM-6938 test for 100-ft fill section. 4.There is a single reference to stone base depth. 5. There is no record of pavement pattern, equipment, date -time -temp placed, or number of days paving occurred. 6. There is no record of concrete placement (concrete mix: air, slump, temp), or of compressive strength. 7.There is no apparent visual record of asphalt or concrete placement. 8.No pipe inspection video was submitted with bond reduction request. 9.A PE -certification did not accompany subdivision bond inspection request. 10. Copies of recorded easements did not accompany bond inspection request. 11. An asphalt core sample report indicates less than design depth of base asphalt at two locations (ref. item 16., below) 12. Stone depth was not evaluated when base asphalt depth was cored. 13. What is apparent: a. There was slight observation, testing, reporting of roadway construction. b. Lots, sanitary, storm, and water utility line work were focus of detailed inspection reports. c. Weather was unfavorable for significant periods of time while roadway Oxbow Drive, Phase 2 was under construction. d. Two reports for Oxbow Dr., Sta. 1+22 — 20+00, Sta. 20+00 — 22+00 show `no pumping or rutting.' No proof -roll observations were submitted for Oxbow Dr., Sta. 22+00 — 26+50 (450'). County received: 14. Proof -roll reports for —580' LF of 900-ft. proposed public road (Sta. 17+22 — 23+00). 15. A single stone base placement report reads, for this portion of 3/29/18 report, in its entirety: `Six inch lift of 21A placed.' (Oxbow Drive, Sta. 20+00 — 23+00). 16. F&R Asphalt Coring (d. 12/12/18) performed at Oxbox Drive Sta. 75-ft. south of Int. with Watervale drive (Phase 1?), Sta. 19+50 (2.625") and 24+00 (2.75") are evidence design base mix asphalt depth of 3.0" does not exist at two locations on this project. Report does not substantiate project is built to plan. 17. F&R, while coring, did not estimate stone depth at this location. Albemarle received a single notation on base stone placement for Phase 11 (Ref. item 15., above). 18. There are no photos for any aspect of construction, other than photos accompanying pipe repair on Lots 11 and 14, which are Phase I lots. (These reports should be removed from Phase II Material Test Reports, d. 3/21/19). 19. Albemarle County requests: a. Road subgrade, base, and pavement construction photos, reports, observations, material or performance test reports that may not have been submitted (dates, weather, equipment used, etc.). b. Representative information for each required bond inspection checklist item, at reasonable station intervals, for entire Phase 11 length of Oxbow Drive, that shows construction meets design. Engineering /Road Plan Bond Inspection Checklist Page 3 of 3 c. CBRs based on testing every 300' of a linear roadway facility. 20. Albemarle requires: a. Detailed observations and descriptions to accompany PE -certification that improvements are built to plan. b. PE certification that improvements are built to plan. c. Pipe interior inspection reports and video. d. Concrete test reports using cylinders obtained during construction. e. Non-invasive concrete test values for representative sections of concrete curbing, aprons, etc., absent tests performed on samples obtained during construction. f A narrative letter that outlines how materials included with bond inspection request packet submittal meet county bond inspection request /bond reduction requirements. Narrative should outline deficiencies, propose remedy. Note: Albemarle will not accept items piecemeal; please submit required missing or insufficient checklist items, else propose remedy since construction is complete and contemporaneous testing, observation, etc. is not possible. 21. As -built drawing: a. Provide match -line references (include sheet numbers) b. Include As -built pavement design based on base mix asphalt depth < 3.0011, CBR value, 8_8. Re -calculate Dp. Report Dr and Dp on As -built. (Ref. VDOT Pavement Design Guide.) c. Revise Storm Drain As -built profile labels (slope, length, Inv. Elev.) so readable. Engineering is unable to evaluate As -built storm drain profile. d. Label phase 1 - phase 2 lines. e. Note reading: `Only base layer asphalt installed at time of survey. As -built road elevations as shown are based on a 0.074-ft. offset from surveyed top of curb elevation.' It is unclear why base asphalt elevation was not surveyed, directly. 0.074 ft. = 0.88 in. It is unclear why this value is meaningful in this context. Is base asphalt elevation 0.88" lower than top of curb? f. C7: Show full extent of STM Str. D4 - D1. Albemarle requests written response to this correspondence. Engineering will estimate maximum bond reduction in part based on responsiveness to request for a complete bond inspection request packet. We have been through this with Phase I: an irregular time-consuming process. We look forward to receiving information required to calculate bond reduction based on reports and tests available for work complete to date, or information obtained to fill gaps in construction or As -built information.
