This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Suggest submitting the 2019 registration statement as well as the 2014 registration statement. The permit cycle ends July 1, 2019, therefore a new permit/registration statement must be submitted. 2. The total disturbed area 3.04 acres does not match what is described in the project narrative on sheet C 1.2. Please clarify. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Ensure the PPP includes a stockpile and staging/parking area. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. A SWM maintenance agreement will need to be executed before plan approval. Contact Ana Kilmer with our office to begin this process (akilmer(c�r�,albemarle.org or 434-296-5832). 2. The notes on the cover sheet reference Cascadia and another WPO plan number. Please remove or revise. 3. Please ensure the professional seal is signed and dated. 4. Please provide the date and source of the topographic information. 5. There are critical slopes on this parcel. Please show them on all applicable sheets on the plan. 6. Are the level spreaders intended for use as E&S only? Or as a permanent measure? The grass channels must be in a SWM facility easement. Please provide a cross section for the grass channels on the plan. 9. The booklet shows kenmare north, laurel north and laurel south. Please label these on the plan. 10. Booklet shows drainage area to "point a", but does not show drainage area breakdown for each facility. Please provide a drainage map showing the drainage to each facility. 11. Provide maintenance requirements for the SWM facilities on the plan. This can be found in section 9 of the BMP specifications. 12. Forest & open space was used in the runoff reduction spreadsheet, therefore, it will need to be in an easement labeled as SWM forest & open space. An easement plat application can be found in comment 6 above. 14. You should include the following note on the plans and the easement plat, "The SWM Forest and Open Space Easement is subject to the guidance set forth by DEQ in the Virginia Stormwater Management Program. The areas will remain undisturbed in a natural, vegetated state, except for activities as approved by the local program authority, such as forest management, control of invasive species, replanting and revegetating, passive recreation (e.g., trails), and limited bush hogging to maintain desired vegetative community (but no more than four times a year)." 15. It is unclear how you are proposing to meet 9VAC25-870-66. Please clarify. You must meet channel protection and flood protection for each concentrated stormwater flow. D. Erosion and Sediment Control Plan (ESOP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. There are no perimeter controls for the proposed waterline and sanitary line installation. 2. There are no perimeter controls for the permeable paver construction. 3. Provide dust control on the E&S plans. 4. Please show a stockpile location and staging/parking area. 5. Please show that silt fence meets the 1/4 ac drainage per 100 ft silt fence requirement. 6. Ensure there is outfall protection (OP) at each outfall. Show the drainage area, flow and required type and dimensions of OP. 7. Sheet C 1.0 shows a stone construction entrance (in the legend), however the detail on Sheet C 1.1 shows paved. The county requires a paved construction entrance with a WR. 8. What is tenax fence? It is not in the legend. Is it a safety fence? 10. Sheet C 1.0 notes number 1 reference a Sheet C 1.6. There is no Sheet C 1.6. Please revise. 11. Sheet C 1.2 notes a waterline in off -site areas. Clearly show this on the plan. Provide permission to work in this off -site area. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process: After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
