This change is noted in the attached request and plan. This is the second Letter of Revision for this site. A
total of three LORs may be requested after which any further changes to the plan will require submission
in the form of a site plan amendment application.

2. CHANGES IN NEW PIPING FROM THAT SHOWN ON THE DRAWINGS, IN ORDER TO AVOID CONFLICTS WITH EXISTING
ELECTRICAL SYSTEMS, MECHANICAL SYSTEMS, EQUIPMENT, STRUCTURES, OR EXISTING PIPING, SHALL BE THE
RESPONSIBILITY OF THE CONTRACTOR. LIKEWISE, ALTERATIONS TO EXISTING ELECTRICAL SYSTEMS, MECHANICAL
SYSTEMS, EQUIPMENT, OR EXISTING PIPING IN ORDER TO ACCOMMODATE NEW PIPING AND EQUIPMENT SHALL BE THE

OF AIR POCKETS.
4. CONSTRUCTION LIMITS SHALL INCLUDE ALL DISTURBED AREAS. ALL DISTURBED AREAS SHALL BE SEEDED AND
PROVIDED WITH EROSION CONTROL DURING AND AT THE END OF CONSTRUCTION. TOTAL DISTURBED AREA FOR THIS

HEALTH REQUIREMENTS MANUAL EM 385-1-1, SECTION 25, AND WHERE INDICATED ON THE DRAWINGS TO
ADEQUATELY SUPPORT EXISTING SOIL AND ADJACENT STRUCTURES DURING EXCAVATION ACTIVITIES. THE CONTRACTOR 1S

RESTORED TO ORIGINAL LINES, GRADES OF ADJACENT SURFACES AND MATCH SURFACE MATERIALS. DENUDED AREAS
SHALL BE SEEDED IN ACCORDANCE WITH THE CURRENT EDITION OF THE VIRGINIA EROSION AND SEDIMENT CONTROL
HANDBOOK.
15. PRECAST CONCRETE UNITS SHALL HAVE A MINIMUM 28—DAY COMPRESSIVE STRENGTH OF 4000 PSI,
16. DESIGN STANDARD PRECAST CONCRETE UNITS TO WITHSTAND H2O DESIGN LOAD CONDITIONS IN ACCORDANCE WITH
APPLICABLE INDUSTRY DESIGN STANDARDS ACI 318, ACI 350, ASTM, ACPA DESIGN MANUAL, PCI MNL-120, AND
AASHTO. DESIGN MUST ALSO CONSIDER STRESSES INDUCED DURING HANDLING, SHIPPIN 7 AND INSTALLATION IN ORDER
TO AVOID PRODUCT CRACKING OR OTHER HANDLING DAMAGE. DESIGN LOADS FOR PRECAST CONCRETE UNITS SHALL
BE INDICATED ON THE SHOP DRAWINGS.
17. PRECAST CONCRETE UTILITY TRENCH SHALL BE ACQUIRED FROM OLDCASTLE PRECAST OR CONCRETE PIPE & PRECAST.
PRECAST CONCRETE UTILITY TRENCH SHALL BE PROVIDED WITH WATERTIGHT JOINTS. LEAKAGE SHALL NOT BE ALLOWED.
18. UTILITY TRENCH JOINTS SHALL BE PROVIDED WITH CONSEAL 102B (GASKET) AND CONSEAL 212 (JOINT WRAP).
19. UTILITY TRENCH SHALL BE INSTALLED PER MANUFACTURER'S RECOMMENDATIONS TO ENSURE WATERTIGHTNESS.
20. USE SLEEVES WHERE PIPES, VALVES, STEM EXTENSIONS, OR EQUIPMENT PARTS PASS THROUGH CONCRETE OR
MASONRY WALLS OR SLABS. SLEEVES SHALL BE SUFFICIENT SIZE TO ALLOW SEALING AROUND PIPES AND CLEARANCE
FOR VALVE STEM OR EQUIPMENT. EXTEND VERTICAL SLEEVES THROUGH SLABS 1—INCH ABOVE TOP SURFACE,
21. USE CAST IRON OR STEEL SLEEVES WITH INTERMEDIATE COLLARS TO ANCHOR AND PROVIDE WATER STOPS ON
SLEEVES THAT PASS THROUGH EXTERIOR WALLS BELOW GRADE. SEAL AROUND PIPES USING GROUTING COMPOUND OR
"LINK —SEAL" PIPE TO WALL CLOSURES MANUFACTURED BY THUNDERLINE CORPORATION, WAYNE, MICHIGAN. SEALS

OPENING. GROUTING COMPOUND SHALL BE MIXED AND PLACED IN ACCORDANCE WITH THE RECOMMENDATIONS OF THE
MANUFACTURER. MECHANICAL PIPE TO WALL SEAL SHALL BE INSTALLED IN ACCORDANCE WITH THE WRITTEN
