This project is denied. The rationale is given in the comments below. The application may be resubmitted for approval if all of the items below are satisfactorily addressed. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. SWPPP not provided. Please provide SWPPP with all associated requirements. Rev. 1: Provide signed registration statement(section 1) and signed certification (section 9). 2. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. PPP not provided. If E&S plan is to be used, please note that and ensure all requirements in 17- 404 are shown on the plan. Rev. 1: Comment addressed. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is disapproved, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17-403. 1. Professional seal should be signed and dated. Calculations packet should also have a signed Engineering Review Comments Page 2 of 4 professional seal. Rev. 1: Comment not addressed. 2. [Sheet 5]Check contours between buildings in the southeastern corner of the site. Rev. 1: Comment addressed. 3. Bottom elevation of proposed storm pond should be lower than the proposed sediment basin. Rev. 1: Elevation appears to be 646 for both of them? 5. Provide easement around pond and access road labeled as SWM facility. Rev. 1: The proposed easement line is unlcear and hard to follow. 6. Pond must comply with quantity design requirements for #15 on https://www.swbmp.vwrrc.vt.edu/ 2:1 slopes are not allowed. A safety bench should be provided since the pond is so deep. Quality aspects do not apply (pre-treatment, filtering, etc). Let us know if you have any questions. Rev 1: Comment addressed. 7. The plan currently does not meet the energy balance requirement. Consider other means and methods to meet this requirement. Perhaps use underdrains without the 3" orifice? Or other runoff reduction practices on site. Rev. 1: The plan still does not meet the energy balance. 8. Nutrient credits must be purchased before construction can begin. Please contract Ana Kilmer (434-296-5832 ext 3246) to start this process. Do not buy credits before speaking with Ana. Rev. 1: Comment still applies. 9. A stormwater management maintenance agreement will need to be executed before the plan can be approved. Rev. 1: Comment still applies. 10. An easement plat with a deed will need to be approved and executed before the plan can be approved. Rev. 1: Comment still applies. 11. Update vicinity map to match current County GIS. Rev. 1: Comment addressed. 12. [Sheet 6] Show different staging water surface elevations on the section detail. Rev. 1: Comment addressed. 13. Add a note to the plan stating that the maximum slope is 2:1 (no slope shall be steeper than 2:1 or something similar). Rev. 1: Comment addressed. 14. Rev. 1: The stream buffer is not shown correctly. Per the county code 17-600: The stream buffer shall be no less than one hundred (100) feet wide on each side of any perennial stream and contiguous nontidal wetlands, measured horizontally from the edge of the contiguous nontidal wetlands, or the top of the stream bank if no wetlands exist D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. Proposed 2:1 slopes must specify a type of ground cover or grass that can withstand steep slopes. Rev. 1: Note should not be on Sheet 4, Phase 1. It needs to be specified on the sheet showing final grades. Limits of clearing and grading should not coincide with the property line or tree protection and silt fence, etc. Rev. 1: Comment addressed. Rev. 1: Sheet 3 still shows stone construction entrance detail in addition to the paved detail. 4. [Sheet 3] Provide outlet protection detail and calculations for sizing based on proposed flow/velocity. Rev. 1: Detail was provided, however calculations/sizing were not. The calcualtions in the booklet are sufficient, however the sizing must be provided on the plans for the contractor. 5. [Sheet 4] Proposed diversion dike to sediment basin will not function. There is a low point in the southeastern portion of the site. The diversion cannot turn 90 degrees north. It must have a positive grade to the outlet. Rev. 1: Comment addressed. 6. Provide safety fence alone the western property boundary to protect the existing single family houses. Rev. 1: Location of safety fence is not evident on the plans. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process: After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted along with court recording fees. After bonding and agreements are complete, county staff will need to enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants will need to complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
