❑ _ Sweep the road of loose stones and debris prior to final inspection
❑ N/A All post larger than 4"x 4"shall be drilled with a hole configuration equivalent to VDOT standard 602.3

AS-Builts are Recuired to show existing grades,planned grades and finished grades of roadway along with drainage items
and easements.
1) Backfill and Stabilize all areas seed and straw behind the sidewalk and curb and gutter.
2) Straigten all stop signs that are loose and leaning. Tops should be cut to have 16"above the top of post only.
3) Clean and Sweep the road and curb and gutter. Flush all DI throats.
Curb and Gutter, Sidewalk will be re-inspected on areas of road where house construction is taking

5) See Attached Video Report that has been reviewed with clean out needed.
6) Need Quit Claims from Utility companies in right of way. Have Dominion Power.

11) Show and mark right of way line with an arrow and RJW.
Street: Misc. Items ALL STREETS
1)All stop signs will need to be 30'with 16"above the top of stop sign for VDOT route markers to be placed and 7'from
the bottom of stop sign to the ground for pedesrtian head clearence.
2) All speciality signs, plantings, irrigation equipment that is in the right of way will need to have a maintneance agreement
with a permit to keep in place and will need to be completed after the road officially is in the state system.
3) Otheryroblems that may exist or have occurred since thie review that is not on this 45 day punch list will be added.
Metal Post for State Signs will have a 7 gauge base and 10 gauge post. Wood post 4x4 can be used as well.
4)County 911 Signs are to be located to the left of the intersection on there own post.

1)All Subgrade Density Test and CBR Test HAVE
2)All Roller Patterns and Density Test for#21-A or B Stone. HAVE
3) All Roller Patterns and Density Test for Sm-9.5A. And BM-25.0 Have.
4)All Density test for Fills. HAVE
5)All Pipe Backfill test reports.HAVE
6)All CD and UD types and sizes will be required prior to final overlay to have a video camera inspection to check for problem
GENERAL NOTES: All asphalt repairs will be milled and paved at the existing grade of planned roadway design. Any se-
ction not marked during this review does not relieve the developer of the responsibility of other places that have settled or
intersections that have been overlayed above the designed grade of the remaining sections of roadway that have not been
disturbed for open cuts and overlayed with out being milled and paved properly. Developer will review and call if futher
review of a section or spot in the field is needed from VDOT to be marked between the final inspection.

Minor Joint issue appears to be present at bottom. This one can be
repaired with VDOT approved caulk to seal area or an approved epoxy

flushing the mud and rock from pipe. Re-video after cleaning and
caulking or epoxying the joint area. Joint is 95% closed, but want to
make sure the flow line is sealed.

Description: Site had been cleared and grubbed,was saturated from weekend rain. AG Dillard used bulldozer
to push off wet material. On site material being ripped and picked up by pans coming NE of Bee
Grove St and N of Ashlar Avenue consisted of mixed CLAY and weathered SHALE. Off site
CLAY being end dumped from tandem axle trucks. Sheepsfoot spread loose lift of 8 inches to 10
inches and compacted with multiple passes. Only one lift placed this date. Filled placement
procedures and compaction efforts were found to be satisfactory.

Description: F&R was onsite as scheduled to observe earthwork operations. Upon arrival the earthwork
contractor was observed ripping the subgrade materials that had frozen overnight. F&R informed
the contractor that frozen materials are not considered satisfactory for use as structural fill.
However,the contractor elected to re-roll the frozen materials and continue to place new fills on
top. One eight inch lift of structural fill was placed and compacted over a frozen subbase
approximately 4' below subgrade.
The frozen materials underling the new fill is considered to be a discrepancy with the project
specifications.

The purpos,e of this letter is to provide you with the results of the corrosion potential soil testing
conducted on five specimens collected from the above referenced project site. Soil samples were
collected along the alignment of the proposed waterline running within Blocks 28 & 29B. Samples were
tested in evaluation for ductile iron pipe (10-Point System) as required by the Albemarle County Service
Authority(ACSA). The laboratory test results are provided within the following table. ,

How to Identify Corrosive Environments
It is important to identify potentially corrosive environments prior to pipeline installation because,once a pipeline is installed.it is both cosby and
difficult to retrofit with corrosion-protection measures. Although Ductile Iron pipe possesses good resistance to corrosion and needs no
additional protection in most soils,experience has shown that external con wion protection is warranted in certain soil environments.Examples
3 include soils with low resistivities,anaerobic bacteria,differences in composition,and differential aeration around the pipe.Dissimilar metals and
external sb ay direct currents may also necessitate additional col I osion protection.Soils contaminated by coal mine wastes, cinders.refuse,or
salts also are generally considered corrosive.So are certain naturally occurring environments,such as swamps, peat bogs,expansive clays,and
alkali soils.And soils in wet,low-lying areas are generally considered more corrosive than those in well-drained areas.

101. Subgrades were found to be firm and stable, and exhibited adequate bearing.
