floodplain. Development includes buildings or other structures, mining, dredging, filling, grading, paving, excavation or
drilling operations or storage of equipment or materials. The form responses will be used to determine whether a
floodplain impact plan or special use permit is required.

Please describe the nature of the development activity proposed within the floodplain (e.g. new structure, grading for a
playing field, driveway stream crossing, installation of a fence, etc.):
Construction of public safety improvements including VDOT GR-2 guardrail and stone rip -rap along one side of existing boat
launch pavement. A small amount (6.19 CY) of gabion stone fill material is proposed for the project. Given the large size of the
reservoir watershed there will be no measurable impact on the existing floodway conditions.

CONSTRUCTION NOTES:
1) ALL WORK SHALL BE PERFORMED TO MEET THE LATEST EDITION OF THE VDOT ROAD AND BRIDGE
STANDARD PLANS AND SPECIFICATIONS.
2) A SURVEY, BY A LICENSED SURVEYOR, WAS NOT PREPARED FOR THIS PROJECT. SITE PLAN MAPPING
IS BASED ON COUNTY AND OTHER GIS MAPPING SOURCES AND IS REPRESENTED ON THE PLANS AS
SHOWING APPROXIMATE LOCATIONS ONLY. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR
DETERMINING EXACT FIELD CONDITIONS RELATED TO ALL ASPECTS OF THE CONSTRUCTION WORK.
