General
1. ALL WORK SHALL BE IN ACCORDANCE WITH THE CURRENT EDITION OF VIRGINIA WORK AREA PROTECTION
MANUAL, V.D.O.T ROAD AND BRIDGE STANDARDS AND SPECIFICATIONS, AND SOIL AND AGGREGATE COMPACTION
MANUAL.
2. THE "MISS UTILITY LAW" REQUIRES FOR THE CONTRACTOR TO CALL (1-800-552-7001) AT LEAST 3
WORKING DAYS IN ADVANCE OF THE PLANNED WORK TO ALLOW TIME FOR MARKING, THAT THE
MARKS BE RESPECTED AND PROTECTED, AND THAT EXCAVATION BE COMPLETED CAREFULLY.
3. CONTRACTOR SHALL BE RESPONSIBLE FOR SITE SECURITY AND JOB SAFETY. CONSTRUCTION
ACTIVITIES SHALL BE IN ACCORDANCE WITH OSHA STANDARDS AND LOCAL REQUIREMENTS.
4. AREAS DISTURBED DURING CONSTRUCTION AND NOT RESTORED WITH IMPERVIOUS SURFACES
(BUILDINGS, PAVEMENTS, WALKS, ETC.) SHALL FOLLOW DETAIL PROVIDED BY VIRGINIA EROSION AND
SEDIMENT CONTROL HANDBOOK FOR SITE SPECIFIC SEEDING MIXTURES IN ACCORDANCE WITH
STANDARD & SPECIFICATION 3.32.
5. UPON AWARD OF CONTRACT, CONTRACTOR SHALL MAKE NECESSARY CONSTRUCTION NOTIFICATIONS
AND APPLY FOR AND OBTAIN NECESSARY PERMITS, PAY FEES, AND POST BONDS ASSOCIATED WITH
THE WORK INDICATED ON THE DRAWINGS, IN THE SPECIFICATIONS, AND IN THE CONTRACT
DOCUMENTS. DO NOT CLOSE OR OBSTRUCT ROADWAYS, SIDEWALKS, AND FIRE HYDRANTS, WITHOUT
APPROPRIATE PERMITS.
6• TEMPORARY ROAD CLOSURES, TRAFFIC SIGNAGE AND PAVEMENT MARKINGS SHALL CONFORM TO THE
MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES (FHWA).
7. A TEMPORARY STREET CLOSURE PERMIT IS REQUIRED FOR CLOSURE OF SIDEWALKS, PARKING
SPACES AND ROADWAYS AND IS SUBJECT TO APPROVAL BY THE CITY TRAFFIC ENGINEER.
8. AREAS OUTSIDE THE LIMITS OF PROPOSED WORK DISTURBED BY THE CONTRACTOR'S OPERATIONS
SHALL BE RESTORED BY THE CONTRACTOR TO THEIR ORIGINAL CONDITION AT THE CONTRACTOR'S
EXPENSE.
9. IN THE EVENT THAT SUSPECTED CONTAMINATED SOIL, GROUNDWATER, UNFORESEEN UTILITIES,
SUBSURFACE CONDITIONS, ARCHAEOLOGICAL RESOURCES, AND OTHER MEDIA ARE ENCOUNTERED
DURING EXCAVATION AND CONSTRUCTION ACTIVITIES BASED ON VISUAL, OLFACTORY, OR OTHER
EVIDENCE, THE CONTRACTOR SHALL STOP WORK IN THE VICINITY OF THE SUSPECT AREA, AND
SHALL NOTIFY THE OWNER/ENGINEER IMMEDIATELY SO THAT THE APPROPRIATE TESTING AND
SUBSEQUENT ACTION CAN BE TAKEN.
10. CONTRACTOR SHALL PREVENT DUST, SEDIMENT, AND DEBRIS FROM EXITING THE SITE AND SHALL BE
RESPONSIBLE FOR CLEANUP, REPAIRS AND CORRECTIVE ACTION IF SUCH OCCURS.
10 DAMAGE RESULTING FROM CONSTRUCTION LOADS (E.G. ROADS, SIDEWALKS, CULVERTS, ETC.) SHALL
BE REPAIRED BY THE CONTRACTOR AT NO ADDITIONAL COST TO OWNER.
11 CONTRACTOR SHALL CONTROL STORMWATER RUNOFF DURING CONSTRUCTION TO PREVENT ADVERSE
IMPACTS TO OFF SITE AREAS, AND SHALL BE RESPONSIBLE TO REPAIR RESULTING DAMAGES, IF
ANY, AT NO COST TO OWNER.
12. THIS PROJECT DISTURBS MORE THAN ONE ACRE OF LAND AND FALLS WITHIN THE VIRGINIA
STORMWATER MANAGEMENT PROGRAM (VSMP), GENERAL CONSTRUCTION PERMIT (GCP) PROGRAM AS
ADMINISTERED BY THE VIRGINIA DEPARTMENT OF CONSERVATION AND RECREATION (DCR) UNDER THE
JURISDICTION OF THE EPA. PRIOR TO THE START OF CONSTRUCTION, THE CONTRACTOR SHALL FILE
A GCP NOTICE OF INTENT WITH THE DCR AND PREPARE A STORMWATER POLLUTION PREVENTION
PLAN IN ACCORDANCE WITH THE VSMP REGULATIONS.
Utilities
1. THE LOCATIONS, SIZES, AND TYPES OF EXISTING UTILITIES ARE SHOWN AS AN APPROXIMATE
REPRESENTATION ONLY. THE UTILITY INFORMATION SHOWN DOES NOT GUARANTEE THE ACTUAL
EXISTENCE, SERVICEABILITY, OR OTHER DATA CONCERNING THE UTILITIES, NOR DOES IT GUARANTEE
AGAINST THE POSSIBILITY THAT ADDITIONAL UTILITIES MAY BE PRESENT THAT ARE NOT SHOWN ON
THE PLANS. PRIOR TO ORDERING MATERIALS AND BEGINNING CONSTRUCTION, THE CONTRACTOR
SHALL VERIFY AND DETERMINE THE EXACT LOCATIONS, SIZES, AND ELEVATIONS OF THE POINTS OF

2. WHERE AN EXISTING UTILITY IS FOUND TO CONFLICT WITH THE PROPOSED WORK, OR EXISTING
CONDITIONS DIFFER FROM THOSE SHOWN SUCH THAT THE WORK CANNOT BE COMPLETED AS
INTENDED, THE LOCATION, ELEVATION, AND SIZE OF THE UTILITY SHALL BE ACCURATELY DETERMINED
WTHOUT DELAY BY THE CONTRACTOR, AND THE INFORMATION FURNISHED IN WRITING TO THE
OWNER'S REPRESENTATIVE FOR THE RESOLUTION OF THE CONFLICT AND CONTRACTOR'S FAILURE TO
NOTIFY PRIOR TO PERFORMING ADDITIONAL WORK RELEASES OWNER FROM OBLIGATIONS FOR
ADDITIONAL PAYMENTS WHICH OTHERWISE MAY BE WARRANTED TO RESOLVE THE CONFLICT.

ELEVATIONS ON THE GRADING AND UTILITY PLANS.

5. -THE LOCATION, SIZE, DEPTH, AND SPECIFICATIONS FOR CONSTRUCTION OF PROPOSED PRIVATE
UTILITY SERVICES SHALL BE INSTALLED ACCORDING TO THE REQUIREMENTS PROVIDED BY, AND
APPROVED BY, THE RESPECTIVE UTILITY COMPANY (GAS, TELEPHONE, ELECTRIC, FIRE ALARM, ETC.).

6. CONTRACTOR SHALL MAKE ARRANGEMENTS FOR AND SHALL BE RESPONSIBLE FOR PAYING FEES FOR
POLE RELOCATION AND FOR THE ALTERATION AND ADJUSTMENT OF GAS, ELECTRIC, TELEPHONE,
FIRE ALARM, AND ANY OTHER PRIVATE UTILITIES, WHETHER WORK IS PERFORMED BY CONTRACTOR
OR BY THE UTILITIES COMPANY.
7. STORM DRAINAGE PIPES SHALL BE RCP
8. ALL DRAINAGE AND SANITARY STRUCTURE INTERIOR DIAMETERS (4" MIN.) SHALL BE DETERMINED BY
THE MANUFACTURER BASED ON THE PIPE CONFIGURATIONS SHOWN ON THESE PLANS AND LOCAL
MUNICIPAL STANDARDS.
Layout and Materials
1. DIMENSIONS ARE FROM THE FACE OF CURB, FACE OF BUILDING, FACE OF WALL, AND CENTER LINE
OF PAVEMENT MARKINGS, UNLESS OTHERWISE NOTED.
2. ANY PROPERTY LINE MONUMENTATION DISTURBED DURING CONSTRUCTION SHALL BE SET OR RESET
BY A PROFESSIONAL LICENSED SURVEYOR.
3. PRIOR TO START OF CONSTRUCTION, CONTRACTOR SHALL VERIFY EXISTING PAVEMENT ELEVATIONS
AT INTERFACE WITH PROPOSED PAVEMENTS, AND EXISTING GROUND ELEVATIONS ADJACENT TO
DRAINAGE OUTLETS TO ASSURE PROPER TRANSITIONS BETWEEN EXISTING AND PROPOSED FACILITIES.
4. SYMBOLS AND LEGENDS OF PROJECT FEATURES ARE: GRAPHIC REPRESENTATIONS AND ARE NOT
NECESSARILY SCALED TO THEIR ACTUAL DIMENSIONS OR LOCATIONS ON THE DRAWINGS. THE
CONTRACTOR SHALL REFER TO THE DETAIL SHEET DIMENSIONS, MANUFACTURERS' LITERATURE, SHOP

DATA FILES THAT ARE OBTAINED FROM THE DESIGNERS, BUT SHALL VERIFY LOCATION OF PROJECT
FEATURES IN ACCORDANCE WITH THE PAPER COPIES OF THE PLANS AND SPECIFICATIONS THAT ARE
SUPPLIED AS PART OF THE CONTRACT DOCUMENTS.
1. CONTRACTOR SHALL REMOVE AND DISPOSE OF EXISTING MANMADE SURFACE FEATURES WITHIN THE
LIMIT OF WORK INCLUDING BUILDINGS, STRUCTURES, PAVEMENTS, SLABS, CURBING, FENCES, UTILITY
POLES, SIGNS, ETC.
2. EXISTING UTILITIES SHALL BE TERMINATED, UNLESS OTHERWISE NOTED, IN CONFORMANCE WITH
LOCAL, STATE AND INDIVIDUAL UTILITY COMPANY STANDARDS SPECIFICATIONS, AND DETAILS. THE
CONTRACTOR SHALL COORDINATE UTILITY SERVICE DISCONNECTS WITH THE UTILITY
REPRESENTATIVES.
1 CONTRACTOR SHALL DISPOSE OF DEMOLITION DEBRIS IN ACCORDANCE WITH APPLICABLE FEDERAL,
STATE AND LOCAL REGULATIONS, ORDINANCES AND STATUTES.
1• PRIOR TO STARTING ANY OTHER WORK ON THE SITE, THE CONTRACTOR SHALL NOTIFY APPROPRIATE
AGENCIES AND SHALL INSTALL EROSION CONTROL MEASURES AS SHOWN ON THE PLANS AND AS
IDENTIFIED IN FEDERAL, STATE AND LOCAL APPROVAL DOCUMENTS PERTAINING TO THIS PROJECT.
2. CONTRACTOR SHALL INSPECT AND MAINTAIN EROSION CONTROL MEASURES, AND REMOVE SEDIMENT
THEREFROM ON A IWEEKLY BASIS AND WITHIN TWELVE HOURS AFTER EACH STORM EVENT AND
DISPOSE OF SEDIMENTS IN AN UPLAND AREA SUCH THAT THEY DO NOT ENCUMBER OTHER
DRAINAGE STRUCTURES AND PROTECTED AREAS.
3. CONTRACTOR SHALIL BE FULLY RESPONSIBLE TO CONTROL CONSTRUCTION SUCH THAT
SEDIMENTATION SHALL NOT AFFECT REGULATORY PROTECTED AREAS, WHETHER SUCH SEDIMENTATION
IS CAUSED BY WAITER, WIND OR DIRECT DEPOSIT.
4. CONTRACTOR SHALIL PERFORM CONSTRUCTION SEQUENCING SUCH THAT EARTH MATERIALS ARE
EXPOSED FOR A MINIMUM OF TIME BEFORE THEY ARE COVERED, SEEDED OR OTHERWISE STABILIZED
TO PREVENT EROSIiON.
5. UPON COMPLETION OF CONSTRUCTION AND ESTABLISHMENT OF PERMANENT GROUND COVER,
CONTRACTOR SHALL REMOVE AND DISPOSE OF EROSION CONTROL MEASURES AND CLEAN SEDIMENT
AND DEBRIS FROM ENTIRE DRAINAGE AND SEWER SYSTEMS,

DISCHARGE OF POLLUTANTS TO THE SURFACE WATERS OF THE STATE, OR IF THE STORM WATER POLLUTION
PREVENTION PLAN PROVES TO BE INEFFECTIVE IN ELIMINATING OR SIGNIFICANTLY MINIMIZING POLLUTANTS
FROM THE CONSTRUCTION ACTIVITY.
4. DOCUMENTATION OF THE DATES WHEN MAJOR GRADING ACTIVITIES OCCUR, WHEN CONSTRUCTION ACTIVITIES
TEMPORARILY OR PERMANENTLY CEASE ON A POR 110N OF THE SITE, AND WHEN STABILIZATION MEASURES
ARE IMPLEMENTED.
5. INSPECTIONS OF DISTURBED AREAS OF THE CONSTRUCTION SITE AND AREAS USED FOR STORAGE OF
MATERIALS THAT ARE EXPOSED TO PRECIPITATION THAT HAVE NOT BEEN FINALLY STABILIZED, STRUCTURAL
CONTROL MEASURES, AND LOCATIONS WHERE VEHICLES ENTER OR EXIT THE SITE SHALL BE CONDUCTED AT
LEAST ONCE EVERY SEVEN CALENDAR DAYS AND WITHIN 24 HOURS OF THE END OF A STORM EVENT THAT
PRODUCES SURFACE RUNOFF AS REQUIRED BY THE GENERAL PERMIT REGULATIONS. REPORTS SUMMARIZING
THE INSPECTIONS SHALL BE MADE IN ACCORDANCE TO THE REGULATIONS AND RETAINED AS PART OF THE
STORM WATER POLLUTION PREVENTION PLAN.
6. ALL CONTRACTORS AIND SUBCONTRACTORS SHALL SIGN A COPY OF THE CERTIFICATION STATEMENT AS
DESCRIBED IN THE GEENERAL PERMIT BEFORE CONDUCTING ANY PROFESSIONAL SERVICES AT THE SITE
IDENTIFIED IN THE POLLUTION PREVENTION PLAN. ALL CERTIFICATIONS SHALL BE INCLUDED IN THE STORM
WATER POLLUTION PREVENTION PLAN.
7. ALL DOCUMENTS, RECORDS, REPORTS, AND OTHER INFORMATION RELEVANT TO THE GENERAL PERMIT
REGULATIONS SHALL BE GIVEN TO THE OWNER FOLLOWING FINAL STABILIZATION OF THE SITE.
8. WHERE THE SITE HAS BEEN FINALLY STABILIZED AND ALL STORM WATER DISCHARGES FROM CONSTRUCTION
ACTIVITIES THAT ARE AUTHORIZED BY THE GENERAL PERMIT ARE ELIMINATED, A NOTICE OF TERMINATION
SHALL BE PREPARED FOR THE OWNER TO SIGN AND SUBMIT TO THE VIRGINIA DEPARTMENT OF
ENVIRONMENTAL QUALITY, WATER DIVISION.
9. THE STRUCTURAL STABILITY OF THE PERMITTED FACILITIES SHALL BE ENSURED.

1. PRIOR TO ANY CONSTRUCTION WITHIN ANY EXISTING PUBLIC RIGHT-OF-WAY, INCLUDING
CONNECTION TO ANY EXISTING ROAD, A PERMIT SHALL BE OBTAINED FROM THE VIRGINIA
DEPARTMENT OF TRANSPORTATION (VDOT). THIS PLAN AS DRAWN MAY NOT ACCURATELY
REFLECT THE REQUIREMENTS OF THE PERMIT. WHERE ANY DISCREPANCIES OCCUR THE
REQUIREMENTS OF THE PERMIT SHALL GOVERN.
2. ALL MATERIALS AIND CONSTRUCTION METHODS SHALL CONFORM TO CURRENT SPECIFICATIONS
AND STANDARDS (OF VDOT UNLESS OTHERWISE NOTED.
3. EROSION AND SILITATION CONTROL MEASURES SHALL BE PROVIDED IN ACCORDANCE WITH THE
APPROVED EROSION CONTROL PLAN AND SHALL BE INSTALLED PRIOR TO ANY CLEARING,
GRADING OR OTHER CONSTRUCTION.
4. ALL SLOPES AND DISTURBED AREAS ARE TO BE FERTILIZED, SEEDED AND MULCHED.
5. THE MAXIMUM ALLOWABLE SLOPE IS 2:1 (HORIZONTAL: VERTICAL). WHERE REASONABLY
OBTAINABLE, LESS6ER SLOPES OF 3:1 OR BETTER ARE TO BE ACHIEVED.
6. PAVED, RIP -RAP OR STABILIZATION MAT LINED DITCH MAY BE REQUIRED WHEN IN THE
OPINION OF THE COUNTY ENGINEER, OR DESIGNEE, IT IS DEEMED NECESSARY IN ORDER TO
STABILIZE A DRAINAGE CHANNEL.
7. ALL TRAFFIC CONTROL SIGNS SHALL CONFORM WITH THE VIRGINIA MANUAL FOR UNIFORM

WETLAND CONSTRUCTION ACTIVITIES SHALL INCLUDE: REMOVAL AND TEMPORARY STORAGE OF TOPSOIL AND SOIL MATERIAL DOWN TO ELEVATION 311.0, COMPACTION OF EXISTING SUBGRADE AND
INSTALLATION OF A CLAY CUT-OFF TRENCH, REPLACEMENT OF TOPSOIL, GRADING OF MITIGATION CHANNELS, AND PLANTING DISTURBED AREAS WITH A VARIETY OF NATIVE TREES, SHRUBS, AND

5. JURISDICTIONAL WETLANDS AND STREAMS OUTSIDE BUT ADJACENT TO THE PROJECT LIMITS OF DISTURBANCE SHALL NOT BE IMPACTED. THE CONTRACTOR SHALL CLEARLY MARK THESE AREAS IN THE
FIELD WITH ORANGE SAFETY FENCE
6. ALL TEMPORARY SOIL EROSION AND SEDIMENT CONTROL (SESC) MEASURES SHALL BE CONSTRUCTED AND MAINTAINED IN ACCORDANCE WITH THE VIRGINIA EROSION AND SEDIMENT CONTROL (VAESCH)
HANDBOOK, CURRENT EDITION,
7. ALL AREAS DISTURBED BY CONSTRUCTION WILL BE STABILIZED WITH PERMANENT SEEDING IMMEDIATELY FOLLOWING FINISH GRADING ACTIVITIES. SEEDING AND SEEDED AREA MAINTENANCE SHALL BE

1. THE EXISTING CONDITIONS SHOWN ON THIS PLAN WERE PROVIDED BY KIRK HUGES AND ASSOCIATES, 'WITH ADDITIONAL FIELD SURVEY IN THE LOCATION OF THE STORM SEWER EXTENSION BY VHB. THE
CONTRACTOR IS RESPONSIBLE FOR FIELD VERIFYING ALL EXISTING CONDITIONS AND DETERMINING THE EXTENT TO WHICH EXISTING CONDITIONS MAY AFFECT CONSTRUCTION OPERATIONS.
2. THE CONTRACTOR SHALL CONTACT "MISS UTILITY" AT 1-800-552-7001 FOR MARKING THE LOCATIONIS OF EXISTING UNDERGROUND UTILITIES (I.E. WATER, SEWER, GAS, TELEPHONE, ELECTRIC, AND
CABLE TV) AT LEAST 72 HOURS PRIOR TO ANY EXCAVATION OR CONSTRUCTION. THE CONTRACTOR SHALL ALSO IDENTIFY AND PROTECT ALL OTHER UTILITIES FOUND IN THE WORK SITE AREA

12. PERFORM STEP 4 & STEPS 6 THROUGH 11 FOR THE REMAINING 2 CELLS OF THE PROPOSED MITIGAT11ON SITE.
13, INSTALL THE PROPOSED STORM SEWER PIPES AND STRUCTURES. START AT THE OUTFALL END, INSTALLING THE HEADWALL AND OUTFALL APRON AS INDICATED ON SHEETS C-4 AND C-9 OF THE

1, THE PLAN APPROVING AUTHORITY MUST BE NOTIFIED ONE WEEK PRIOR TO THE PRE -CONSTRUCTION CONFERENCE, ONE WEEK PRIOR TO THE COMMENCEMENT OF LAND DISTURBING ACTIVITY, AND ONE WEEK PRIOR TO THE FINAL INSPECTION.
