A subdivision plat was approved by the County on October 3rd that requires the ACSA jurisdictional area
GIS layer to be revised. Please see the attached plat and email from ACSA. The parcel is TMP 13600-00-
00-02800.
I am not sure if the new plat has been recorded yet, but as you know it will take a while before the
parcel layer reflects the new lot boundaries. ACSA has requested that GIS show that the new Lot A can
remain as "water only to existing structures." The residual parcel needs to be changed to "no service."

note. I have been here since 11/2016 and this was the first time I had
ever seen that note on a plat. When I spoke with Travis Davis from
VDH, he confirmed that other counties, such as Nelson County, have a
local ordinance requiring that note. Travis mentioned that he has never
signed plats for Albemarle though. When I showed the plat to my
colleagues who have been here longer than me, they also confirmed
that it was not a note we require on plats, and it would be best to
remove it before we sign it for approval.

I have good news — VDH has approved the soils report. I have attached
a copy of their approval letter above.
In order to approve the plat, I will need one last thing to be addressed:
On the Cover Sheet, please remove the three paragraphs of notes
regarding the septic system approval by VDH. Albemarle County does
not require those notes on our plats. The official approval letter from
VDH serves as the only approval we need for the subdivision, and they
don't sign the plats. They keep a copy at their office for reference
when you come in later to obtain a septic permit. I believe other
counties have local ordinances that require those notes on the actual
subdivision plat, but our ordinance does not.
Once the notes have been removed, please obtain owner signatures
that are notarized, and submit at least one copy to me. We can then
sign the plats and approve the application.

I received the soils report and VDH application yesterday and put it
in the Health Department mailbox here at the County office
building. VDH usually comes by on Mondays to check their mailbox
for any applications, so they haven't picked this one up yet.
One thing I noticed is that on the first page of the application, it
states that a copy of the subdivision plat must be included that
shows the locations of the proposed sewage disposal systems and
reserve absorption areas, and the location of the water supply on
each lot. The materials you sent did not have a copy of the plat
attached.
I can attach a copy of the subdivision plat with owner signatures
that was brought to me awhile back if you and Lisa are okay with
that. Otherwise, you can bring me a copy of the plat without owner
signatures that I can insert into the mailing package going to VDH. If
you want to submit another set of copies, please get it to me ASAP.

Thanks for removing the note. You can have the owners sign the plat and have it notarized and bring me
back at least one (1) copy of both sheets for County signatures.
I am not sure when Albemarle stopped requiring the Health Dept. note. I have been here since 11/2016
and this was the first time I had ever seen that note on a plat. When I spoke with Travis Davis from VDH,
he confirmed that other counties, such as Nelson County, have a local ordinance requiring that note.
Travis mentioned that he has never signed plats for Albemarle though. When I showed the plat to my
colleagues who have been here longer than me, they also confirmed that it was not a note we require
on plats, and it would be best to remove it before we sign it for approval.

I have good news — VDH has approved the soils report. I have attached a copy of their approval letter
above.
In order to approve the plat, I will need one last thing to be addressed: On the Cover Sheet, please
remove the three paragraphs of notes regarding the septic system approval by VDH. Albemarle County
does not require those notes on our plats. The official approval letter from VDH serves as the only
approval we need for the subdivision, and they don't sign the plats. They keep a copy at their office for
reference when you come in later to obtain a septic permit. I believe other counties have local
ordinances that require those notes on the actual subdivision plat, but our ordinance does not.
Once the notes have been removed, please obtain owner signatures that are notarized, and submit at
least one copy to me. We can then sign the plats and approve the application.

I received this plat which is creating one new lot, and it had no soils work with the application. I was
going to make this a comment to the applicant, so I didn't send a copy of the plat to VDH when I
distributed it a couple of weeks ago.
As I've been reviewing the plat over the past two days, I noticed the notes on the first sheet that say
that "this subdivision has been approved for on -site sewage systems in accordance with the Code of
Virginia and the SEWAGE HANDLING and DISPOSAL REGULATIONS." It also says that the subdivision was
submitted to VDH with a soil evaluation from Roger Nelson, AOSE #101.
I was wondering if you could confirm that VDH has received a copy of this plat and any associated soils
work. If not, I will tell the applicant to get the soils report to me so I can send it your way.
If VDH has received a copy of the soil evaluations, could you confirm if this site has been approved for a
septic system as noted on the plat?
Also: the property is located in the ACSA service area for water to existing structures only. The existing
building on the property will be located on the new 2.362 acre parcel, but the subdivision will leave a
21.54 residual parcel with no existing dwellings. County Code Section 14-416 requires the applicant to
meet all VDH requirements for private wells when the service authority can't provide public water. If
you have the soils evaluations, let me know if they meet all VDH requirements.

I have made some of the revisions requested, but have the following comments/questions.
1. 1 received the information for the prescriptive easement for route 726 from VDOT. There is no
deed book reference that I know of since it is a prescriptive easement.
2. The deed book/page I reference for the prope rty(469-409) is what the GIS shows. The other
one I saw was DB 342-255, which is older. What did I need to show?
3. 1 added the address of Ida Patterson
4. Removed A from parcel numbers
5. Check if what I changed is correct, never had to put this in the statement.
6. Added signature block for agent

I received this plat which is creating one new lot, and it had no soils work with the application. I was
going to make this a comment to the applicant, so I didn't send a copy of the plat to VDH when I
distributed it a couple of weeks ago.
As I've been reviewing the plat over the past two days, I noticed the notes on the first sheet that say
that "this subdivision has been approved for on -site sewage systems in accordance with the Code of
Virginia and the SEWAGE HANDLING and DISPOSAL REGULATIONS." It also says that the subdivision was
submitted to VDH with a soil evaluation from Roger Nelson, AOSE #101.
I was wondering if you could confirm that VDH has received a copy of this plat and any associated soils
work. If not, I will tell the applicant to get the soils report to me so I can send it your way.
If VDH has received a copy of the soil evaluations, could you confirm if this site has been approved for a
septic system as noted on the plat?
Also: the property is located in the ACSA service area for water to existing structures only. The existing
building on the property will be located on the new 2.362 acre parcel, but the subdivision will leave a
21.54 residual parcel with no existing dwellings. County Code Section 14-416 requires the applicant to
meet all VDH requirements for private wells when the service authority can't provide public water. If
you have the soils evaluations, let me know if they meet all VDH requirements.
