samples, become the property of Albemarle County. Applicants are encouraged to maintain duplicate
copies of all submittal items in their own files.
• Only complete application packages will be scheduled for ARB review. The application package is not
complete without the appropriate checklist, completed, signed, and included with the required submittal
materials indicated on the checklist.

A. Written description of the proposal

2 GZ Explain how the proposal is compatible with the surrounding area and the Entrance Corridor.
B. Site plan showing the approved site conditions and the proposed changes, as follows (drawn to the scale of

1 ® Location(s) of existing and proposed building(s) on the site.
2 Location of proposed parking, travelways, walkways and other improvements.
3 Mechanical equipment, .trash containers, loading and service areas, other similar features and improvements, and
associated screening.
4 Existing and proposed topography drawn with contour intervals of 5-feet or less, and with sufficient off -site
topography to describe prominent and pertinent off -site features and physical characteristics, but in no case less
than 50-feet outside of the site.
5 Location, size, identification of type and extent of existing and proposed utilities and easements.
6 Stormwater management plan.

4 ❑ Photometric plan addressing all fixtures and indicating that lighting does meet the requirements of Section 4.17 of

5 ❑ Coordination of lighting with landscaping and other site elements.
E. Appearance of the building(s):
1 Dimensioned architectural elevations of the existing building and proposed addition. Elevations must be drawn
to the scale of 1/8"=1'-0". Include a building materials schedule and key on the elevation drawings.
2 ❑ One set of all building materials / colors.
3 ❑ A floor plan adequate to show exterior walls, windows and doors.
F. Additional material
1 ❑ Provide labeled, color 8-1/" x 11" photos of the site as seen from both directions on the EC.
2 ❑ Site sections that clarify proposed changes in topography and illustrate the visibility of the proposed development
from the Entrance Corridor. Site sections shall indicate the finish floor elevation(s) and roof height(s) relative to
the natural elevations along the Entrance Corridor.
3 ❑ Color perspective images (for example: sketches, renderings, photosimulations, etc.) that show the proposed
development as seen from the Entrance Corridor are often useful for gaining a clear understanding of the
proposal. The ARB may require such images in certain cases when other submitted documents do not answer all
questions of building form, building details, materials, context, topography, and/or visibility.
4 ❑ Sheet number, total number of sheets, drawing date, date and description of the latest revision, and contact information
for the firm preparing the drawings in the title block on all drawings. For revised drawings, clearly identify revisions
made.
5 ❑ Any additional material that will make the review more productive. Clarification of topography, visibility, utilities,
landscaping, or other unique or unusual conditions are welcome..

RE: Verizon - Shadwell (Easton Property) - Tier II PWSF Application
To whom it may concern:
The application and information enclosed with this cover letter are being submitted in support of
Verizon's request for approval of a Tier II Personal Wireless Service Facility. Please note that a similar
application was submitted for review at the same location in 2017, and that application
(SDP201700055) has been deemed "withdrawn" due to inactivity.
Based on visibility of the balloon during the balloon test and the Architectural Review Board's finding
at its September 18, 2017 meeting that the location would not sufficiently minimize the visibility of the
monopole from the Entrance Corridor, Verizon had the surveyors remeasure the heights of the trees
using an alternate method an determined that the March 27, 2017 zoning drawings submitted with the
previous application contained an error with respect to the height of the reference tree upon which the
monopole's proposed height was based. This application is being submitted with revised drawings
noting the correct reference tree height and a lower monopole height:
In accordance with requirements set forth in the County of Albemarle's Zoning Ordinance, sixteen (16)
separate packages are being submitted for review and each collated set contains the following
information in support of the above -mentioned project:
1. Memorandum addressing the Site `Review Committee's memo for SDP201700055, dated
November 10, 2017
2. Revised project narrative.
3. Photos taken from the man -basket during the remeasuring of tree heights.
4. Specification sheets for the proposed antennae, remote radio head and fiber optic junction boxes.
5. Site Plan and Construction Design Drawings reflecting corrected tree heights, revisions made in
response to the Site Review Committee's memo for SDP201700055, and updated Verizon
equipment.
