Please find our responses to the comments below in bold lettering. Initial Site Plan Comments: Required Prior to Initial Site Plan Approval: 1. ZMA2005-3 Proffers: Compliance with the following proffer is needed prior to initial site plan approval: a. Proffer 9: Historic resources: Prior to preliminary plat, preliminary site plan, or an early grading permit in Tract 1, the Owner shall provide a survey to provide archival record of the existing buildings within Tract 1 and shall meet VDHR standards. On July 10, confirmation was received that DHR has approved the updated VCRIS entries. Required Prior to Final Site Plan Approval: 2. 18-4.6.1 Frontage: A new parcel for this proposal is needed, which will have required frontage on Loop Road. Approval of the road plan for Loop Road is needed before the final site plan can be approved. Acknowledged. 3. 18-4 Zoning overlays: Include the Airport Impact Area (AIA) Overlay District on Sheet C0.0. The AIA Overlay district has been added to Sheet C0.0. 4. 18-32.5.2(k): Easements: Verify that all necessary easements for proposed water, sewer, and any drainage facilities have been shown on the plan. Show and label existing and proposed easements on utility plan and indicate if any easements are to be dedicated to public use. Acknowledged. To maintain plan clarity, proposed stormwater management facilities easement has been provided on Sheet C6.3. Water and sewer easements on the proposed parcel are not required at this time. A 20’ ACSA easement has been provided around the fire line. 5. 18-4.1 Water supplies and sewer systems and Proffers per ZMA2005-3: This site is on public water and sewer (ACSA). Several of the proffers per ZMA2005-3 dealt with the Fire Station and connection to public sewer. The site plan appears to show existing connections to public sewer for the Fire Station. Please confirm if this connection exists, or label as future proposed connection. a. Proffers per ZMA 2005-3 (also attached): 8.1(D); 8.4; 8.5. The Fire Station connection the public sewer has been designed under a separate cover and shall be installed ahead of the start of construction for UVA Encompass Rehabilitation Hospital. See Sheets C2.0 & 4.0 for callout. 6. 18-4.12.6 Parking: Please provide a letter from the zoning administrator showing the approved parking requirements for this site, as the hospital use required spaces are determined by the zoning administrator. If this determination has not yet b een made, please submit a request for the number of spaces needed, under public use and hospital. Per email received by Kevin McCollum on 7/9/19, proposed parking tabulations have been accepted at this time. 7. 18-4.17 Lighting: Submit a lighting plan with the final site plan that meets the requirements of 18-4.17 and ARB requirements. Acknowledged. A lighting plan shall be provided with the final site plan submission. 8. 18-32.5.2: Contents of an initial site plan and 18 -32.6.2 Contents of a final site plan: a. Label the use on C0.0 as “Public Use: Hospital”. Sheet C0.0 has been updated accordingly. b. Include the following on Sheet C0.0: “Proffers per ZMA2005-3”. A reference to proffers ZMA2005-3 have been added to Sheet C0.0. c. Include the Magisterial District (Rio) on Sheet C0.0. Sheet C0.0 has been updated accordingly. d. Also include the Zoning District of adjacent parcels (along with owner and DB/PG). Adjacent parcel information has been provided on Sheet C0.0. e. Include the distance to the centerline of the nearest existing intersection for ingress and egress. A dimension has been provided on Sheet C4.0. 9. 18-32.7.9.4 Contents of a landscape plan: a. Sheet L2.1 shows 8 shrubs Sheet L2.0 shows 100 shrubs for a total of 108, however Sheet L3.0 lists 78 shrubs. Please revise. Shrub calculation has been revised. b. Include a Conservation Checklist. A Conservation Checklist will be signed and provided once Encompass purchases the parcel from UVA Foundation. 10. 18-32.7.9.5: Street Trees: a. Please see ARB comments as well for additional requirements. ARB comments have been reviewed and addressed. b. The Road Plan will need to include the tree information (including caliper) and count for the required street trees. The tree information is located on sheet L3.0 – Landscape Notes and Details. 11. Consistency with the Comprehensive Plan (CCP) for the use of the site. a. Per LOD2019-14, it was determined that this proposal is considered a “public use”. Staff also determined that a CCP application was needed. Tim Padalino (Senior Planner) has been coordinating with Elise Cruz, Fred Missel, and Todd Marshall at the UVA Foundation regarding this application. This application is reviewed and either approved by the Planning Commission or sent to the Board of Supervisors by the Planning Commission for final determination. Approval of the CCP is required prior to final site plan approval. Acknowledged. 12. ZMA2005-3 Proffers: Compliance with the following proffer is needed to prior to initial site plan approval: a. ZMA2005-3 Proffers: Compliance with the following proffers per ZMA2005-3 is needed prior to final site plan approval: a. Proffer 3.1: Square footage: Include a table, as shown on previous site plans under this rezoning, that shows total build out. A table has been provided, similar to previous site plans under this rezoning, on Sheet C0.0. b. Proffer 4.3: Wetlands: Label the location of the wetlands area for future open space. Wetlands have been shown on Sheet C2.0. c. Proffer 5.2: Internal roads must be dedicated for public use. Label Loo p Road as public and please confirm that this is a public street. Based on meeting between County Staff and Timmons Group, the Loop Road will not be required to be publicly maintained. d. Proffer 7.1: Landscaping and roads: All roads in the project must be landscaped according to “Exhibit D: UREF’s North Fork Street Tree Master Plan”, filed with Albemarle County on November 1, 1994. Ensure that the road plan for Loop Road meets these landscaping standards. Acknowledged. Loop Road landscaping to be addressed separately from SDP201900023 and shall comply with Proffer 7.1 accordingly. e. Proffer 8: Fire Station i. Proffer 8.1(D):The septic drainage field that was serving the Fire Station must be closed and removed once the Fire Station is connected to public sewer. Acknowledged. Drain field to be closed and removed once the Fire Station is connected to the public sewer. 1. The submitted plat (under review by Tim Padalino) cannot be approved until the connection to public sewer is made. The plat shows the drainfield off-site, and drainfields must be on the same parcel as the use they serve. Acknowledged. ii. Proffer 8.4: Prior to final plat or plan approval in this section of the rezoning per ZMA2005-3 (Tracts 1 and 2 on Exhibit J): “the applicant shall design and construct at its sole expense, or provide sufficient bond or other form of surety to the County in an amount sufficient to assure construction and acceptance by the appropriate authority, a public sewer to serve the Fire Station Parcel and, in conjunction with such construction and upon request by the developer of the Fire Station Parcel, shall install a lateral from the public sewer that connects the Fire Station to the public sewer. If such as request is made, the developer of the Fire Station Parcel shall reimburse the Applicant for its costs to design and install the lateral and shall pay all fees required for connecting the Fire Station to the public sewer system.” Acknowledged. 1. Label the SAN Lateral connection to the Fire Station as “Future Proposed SAN Lateral”. This connection has not yet been made. Callouts have been provided on Sheets C2.0 & C4.0. iii. Proffer 8.5: Once the Fire Station is connected to public sewer, the applicant shall dedicate the Fire Station Parcel to the County within 90 days. This area is approximately 2.55 acres and is shown on Exhibit B with the proffers approved with ZMA2005-2. Acknowledged. iv. Proffer 8.6: Road connection: Coordinate with Plan reviewers on the Road Plan to ensure this connection is satisfactorily made. Acknowledged. Coordination shall be made to ensure all connections are satisfactory. 14. ZMA2005-3 Proffers: Compliance with the following proffers is needed in future stages of development. This is for informational purposes: a. Proffers 5.3 and 5.4: All road improvements required on the application plan (Exhibit M) for Phase I must be completed or bonded before the maximum build - out of Phase I, based on square footage of uses. Or, VDOT must determine that the intersections will still function at an acceptable level, as described in these proffers. This includes the intersection of SR 649 (Airport Road) and Road A (Lewis and Clark Drive). Acknowledged. i. These improvements are described in detail in Proffer 5.4(B). Acknowledged. ii. These improvements must meet the landscaping and buffer standards in Proffer 7.2. Acknowledged. Landscape and buffer requirements shall meet Proffer 7.2. b. Proffer 6.1: Recreation space: Must be completed prior to maximum build-out of Phase 1. Acknowledged. 15. Additional applications: The following applications and approvals are required prior to final site plan approval: a. 18-32.7.4.2 and 18-32.7.5.3: Easement Plats: It appears no private easements or public non-ACSA easements are proposed. Any non-ACSA or private easements must be included on an easement plat. Acknowledged. An ACSA easement has been provided on Sheet C4.1. b. 18-32 and 14: Subdivision Plat: A subdivision plat is needed prior to final site plan approval. This plat has been submitted and is under review. Compliance with all applicable ZMA2005-3 proffers is also needed prior to final site plan approval. Acknowledged. c. 18-32.7.2 and 14: Approval of road plans is required prior to final site plan approval. Proffer 5.2 refers to internal roads being public roads. Acknowledged. Per meeting with County Staff and Timmons Group, the Loop Road can remain private. d. 18-32.7.4.1: Approval of VSMP Plans is required prior to final site plan approval. Acknowledged. VSMP approval shall be obtained prior to final site plan approval. Engineering (Matt Wentland): 1. The UVA Research Park Loop Road Plans will need to be approved before the Final Site Plan can be approved and both will need to be bonded prior to issuance of a grading permit for this plan. Acknowledged. 2. The VSMP plans will need to be approved before Final Site Plan approval. Acknowledged. VDOT (Adam Moore): 1. See attached review comments. Fire/Rescue (Shawn Maddox): 1. Due to the height of the building exceeding 30’ the travel way must be 26’ clear travel width. Building height tabulations have been revised on Sheet C0.0. The maximum building height (to the bottom of the parapet at the main entry) is 28’ while the top of the parapet. Building elevations were provided on 6/24/19 for use and review. 2. A knox box will be required. Please add a note indicating this requirement and that location can be coordinated with the fire marshal’s office. Acknowledged. See note #1 on Sheet C4.1 & C4.2. 3. The canopy shown at the main entrance should have a minimum clear height of 13’6. The canopy shall maintain a minimum of 13’-6”. 4. A fire flow test of the new hydrants will be required prior to final acceptance of the site. Acknowledged. ACSA (Richard Nelson): 1. Submit 3 copies to ACSA for review. Three (3) hard copies will be provided for ACSA review. 2. Final approval will not be given until final approval of the UVA Loop Road. Acknowledged. 3. Final approval will not be given for Lewis and Clark designs until ACSA receives an updated master plan. Acknowledged. 4. Confirm if the second water meter is a redundant feed to the hospital. If so it will need to be relocated to the other side of the site. The second water line is a redundant feed to the hospital. This line has been relocated to the other side of the site. See Sheet C4.1 & C4.2. 5. RWSA Sewer capacity certification will be required. Acknowledged. Sewer capacity letter has been provided to RWSA for their use and review. 6. Submit 3 copies to ACSA for review. Three (3) hard copies will be provided for ACSA review. 7. North Fork South Zone Special Rate Fees apply. Acknowledged. RWSA (Dyon Vega): 1. See attached review comments. Sewer capacity letter has been provided via email. Health Department (Alan Mazurowski): 1. No objection – see attached. Inspections (Michael Dellinger): 1. No objections. Architectural Review Board (Margaret Maliszewski): 1. Administrative ARB approval. See attached review comments. Acknowledged. E911 (Andy Slack): 1. No objections.
