CAO/county executive have executed the Deed(Attached). Please submit an original LS-sealed signed/dated notarized
plat,rev. date March 13,2019,that has Owner signature blocks for both Saurabh Godhwani and Dharma Patel. CDD is
positioned to approve the plat, and return plat and deed to Applicant for recordation. Please submit plat with transmittal
that reads: 'Not for approval, for county signature approval,'to my attention at your convenience. Thanks for your help
and patience.
Note:
2 original unexecuted versions of deed with sticky reading `DISCARD' will be returned with executed version of deed,
which should be recorded.

Kendra,
We cannot route notarized deed to the county attorney's office(CAO) in its current form(SKlVr364e-
En19060408500). There are 2 items: deed revision/plat revision:
1. CAO may request copy of corporate governance document for SIDDH LLC to establish that Dharma Patel is a
Managing Member. Recorded SWM Facility Maintenance Agreement lists only Saurabh Godhwani as a
Managing Member.

I've attached 2 .PDFs of th.;. vised deed: one lists both Messrs. Godhwai and Patel,the other lists only Dharma
Patel.
2. Please, before CAO requests revision to plat, as is likely, ask Meridian to revise easement plat Statement of
Consent(image,below)to read:
a.__(Version A):verbatim: Saurabh Godhwani,Managing Member/Dharma Patel,Managing_Member,:
ISIDDH LLC ---------- -__---- ---
b. (Version B): verbatim: Saurabh Godhwani,Managing Member, SIDDH LLC
Note: We urge that both plat/deed offer CAO a choice of whether both signatures more nearly meet county
recordation requirements, or whether only Mr. Godhwani need sign deed/plat.
Plat and deed signatures typically match, and also match the SWM Facility Maintenance Agreement. Please
ensure Tim Miller,LS, signs the plat prior to submittal for county approval.
Again, a cover letter to my or Christopher Perez's attention reading 'Not for review, for county signature
approval,' is helpful.
I cannot explain why I listed Mr.Patel as signatory to the deed, and not Saurabh Godhwani,but if both versions are
returned to us, signed and notarized, CAO may select whichever meets county needs.
We will return both the unused,unrecorded original deed and unrecorded original plat to you,to dispense with as you
will. We do need both plat and deed signed/notarized before routing to CAO.
