THE CONTRACTOR WILL BE RESPONSIBLE FOR NOTIFYING THE PROPER ACSA OFFICIALS AT THE START OF THE WORK. 2. THE ALBEMARLE COUNTY SERVICE AUTHORITY SHALL HAVE ACCESS TO USE THE AIRSPACE ABOVE THE LOCATIONS OF CONSTRUCTION FOR THE FLIGHT OF UNMANNED AERIAL VEHICLES FOR THE PURPOSE OF IMAGERY COLLECTION. 3. THE LOCATION OF EXISTING UTILITIES ACROSS THE LINE OF THE PROPOSED WORK ARE NOT NECESSARILY SHOWN ON THE PLANS AND WHERE SHOWN ARE ONLY APPROXIMATELY CORRECT. THE CONTRACTOR SHALL, ON HIS OWN INITIATIVE, LOCATE ALL UNDERGROUND LINES AND STRUCTURES, AS NECESSARY. 4. THE CONTRACTOR SHALL BE RESPONSIBLE TO COMPLY WITH THE NO -LEAD REGULATION REGARDING BRASS FITTINGS EFFECTIVE JANUARY 4, 2014 (SENATE BILL 3874 WHICH AMENDS THE SAFE DRINKING WATER ACT). 12. THE SEWER LATERAL BEYOND THE CONNECTION AT THE SEWER MAIN SHALL BE PRIVATE. THE SEWER LATERAL STUB -OUT SHALL UNDERGO THE ACSA LOW-PRESSURE AIR TEST TO SATISFY COUNTY TESTING REQUIREMENTS. VISUAL INSPECTION OF THE SEWER LATERAL STUB -OUT SHALL BE WITNESSED BY THE COUNTY BUILDING INSPECTIONS DEPARTMENT. THIS INSPECTION SHALL OCCUR UNDER AN "OTHER PLUMBING" PERMIT WHICH MUST BE OBTAINED BY THE CONTRACTOR. 13. THE SEWER LATERAL BEYOND THE CONNECTION AT A MANHOLE SHALL BE PRIVATE. UNLESS OTHERWISE INDICATED, ALL VEGETATIVE AND STRUCTURAL EROSION AND SEDIMENT CONTROL PRACTICES SHALL BE CONSTRUCTED AND MAINTAINED IN ACCORDANCE TO THE MINIMUM STANDARDS AND SPECIFICATIONS OF THE VIRGINIA EROSION AND SEDIMENT CONTROL HANDBOOK. 2. ALL EROSION AND SEDIMENT CONTROL MEASURES ARE TO BE PLACED PRIOR TO OR AS THE FIRST STEP TO LAN',D DISTURBING ACTIVITIES AND SHALL BE MADE FUNCTIONAL BEFORE UPSLOPE LAND DISTURBANCE TAKES PLACE. 3. DURING DEWATERING OPERATIONS, WATER SHALL BE PUMPED INTO AN APPROVED FILTERING DEVICE. EFFLUENT SHALL BE DISCHARGED IN A MANNER THAT DOES NOT ADVERSELY EFFECT FLOWING STREAMS OR OFF -SITE PROPERTIES. 6. CONTRACTOR SHALL INSPECT ALL EROSION CONTROL MEASURES PERIODICALLY AND AFTER EACH RAINFALL EVENT. ANY NECESSARY REPAIRS OR CLEANUP TO MAINTAIN THE EFFECTIVENESS OR THE EROSION CONTROL DEVICES SHALL BE MADE IMMEDIATELY, AT NO ADDITIONAL COST TO THE CONTRACT. 7. AT THE COMPLETION OF CONSTRUCTION, ALL SEDIMENT CONTROLS SHALL BE REMOVED AND ALL AREAS SHALL BE STABILIZED. 8. UNLESS OTHERWISE NOTED, UNDERGROUND UTILITY LINES SHALL BE INSTALLED IN ACCORDANCE WITH THE FOLLOWING STANDARDS IN ADDITION TO OTHER APPLICABLE CRITERIA: 8.1. NO MORE THAN 300 LINEAR FEET OF TRENCH MAY BE OPENED AT ONE TIME. 8.2. EXCAVATED MATERIAL SHALL BE PLACED ON THE UPHILL SIDE OF TRENCHES. 8.3. RESTABILIZATION SHALL BE ACCOMPLISHED IN ACCORDANCE WITH THE CONTRACT 8.4. DOCUMENTS AND APPLICABLE STANDARDS. 8.5. APPLICABLE SAFETY REGULATIONS SHALL BE COMPLIED WITH. 9. IN GENERAL, ALL EROSION AND SEDIMENT CONTROL MEASURES SHALL BE CHECKED DAILY AND AFTER EACH SIGNIFICANT RAINFALL. THE CONTRACTOR SHALL PERFORM THE DETAILED MAINTENANCE INSTRUCTIONS OUTLINED IN THE HANDBOOK FOR EACH STRUCTURAL AND VEGETATIVE MEASURE PROVIDED. THE FOLLOWING ITEMS SHALL_ BE CHECKED IN PARTICULAR: 9.1. EROSION CONTROL STRUCTURES SHALL BE CLEANED WHEN THEY REACH 50% CAPACITY. 9.2. SEEDED AREAS SHALL BE CHECKED REGULARLY TO ENSURE THAT A GOOD STAND IS MAINTAINED. AREAS SHOULD BE FERTILIZED, RESEEDED AND MULCHED AS NEEDED. 9.3. ALL ADJACENT ROADWAYS SHALL BE KEPT CLEAN. FILL SHALL NOT BE SPILLED INTO THE ROADWAY. 9.4. THE CONTRACTOR SHALL PROVIDE DUST CONTROL AS REQUIRED. 9.5. THE AREAS OF CONSTRUCTION SHALL REMAIN IN STABLE CONDITIONS AT THE CLOSE OF EACH DAY, EROSION CONTROLS SHALL BE MONITORED AT THIS TIME, MAINTAINED AND REPAIRED OR REPLACED IF NECESSARY. 9.6. AN ADEQUATE RESERVE OF EROSION CONTROL MATERIALS SHALL BE ON SITE AT ALL TIMES FOR EMERGENCY OR ROUTINE REPLACEMENT AND SHALL INCLUDE MATERIAL TO REPAIR SILT FENCES AND OTHER DEVICES USED TO STABILIZE THE SITES. 9.7. THE CONTRACTOR SHALL PROTECT EXISTING VALVES, MANHOLES, ETC. FROM BEING COVERED. IT IS A VIOLATION OF LAW FOR ANY PERSON, THIS DRAWING WAS PREPARED AT THE SCALE INDICATED. CONTRACTOR SHALL, AT HIS EXPENSE, MAINTAIN THE WORK SITE IN A CLEAN AND ORDERLY APPEARANCE AT ALL TIMES. ALL DEBRIS AND SURPLUS MATERIAL COLLECTED SHALL BE DISPOSED OF OFF THE WORK SITE BY CONTRACTOR, AT HIS EXPENSE. 12. EXISTING LAWNS, TREES, SHRUBS, FENCES, UTILITIES, CULVERTS, WALLS, WALKS, DRIVEWAYS, POLES, SIGNS, RIGHT-OF-WAY MONUMENTS, MAILBOXES AND THE LIKE SHALL BE PROTECTED FROM DAMAGE DURING THE WORK. ANY DAMAGE CAUSED TO SUCH ITEMS SHALL BE REPAIRED OR REPLACED BY CONTRACTOR AT NO ADDITIONAL COST. ALL PROPERTY PINS DISTURBED BY CONTRACTOR, WHETHER OR NOT THEY ARE DEPICTED IN THE SITE BASEMAP, SHALL BE RESTORED BY A VIRGINIA LICENSED SURVEYOR AT CONTRACTOR'S EXPENSE. 13. CONTRACTOR SHALL EMPLOY EROSION CONTROL DEVICES AND METHODS AS REQUIRED TO MEET THE REQUIREMENTS AND INTENT OF THE SERVICE AUTHORITY EROSION CONTROL ORDINANCE AND SHALL COMPLY WITH THE DRAWINGS CONTAINED HEREIN. CONTRACTOR SHALL PROVIDE THE NECESSARY DIVERSION DITCHES, DIKES, TEMPORARY CULVERTS, AND SILT FENCE REQUIRED TO PREVENT MUD AND DEBRIS FROM BEING WASHED ONTO THE STREETS OR PROPERTY. CONTRACTOR'S VEHICLES SHALL BE KEPT CLEAN TO PREVENT MUD OR DUST FROM BEING DEPOSITED ON STREETS. '14. CONTRACTOR SHALL CLEAN UP, RESTORE, SEED AND MAINTAIN ALL DISTURBED AREAS IMMEDIATELY UPON COMPLETION OF WORK ON EACH SITE. TOPSOIL, SEED, FERTILIZER AND MULCH SHALL BE PLACED IN ACCORDANCE WITH THE VIRGINIA EROSION AND CONTROL SEDIMENT CONTROL HANDBOOK, 3RD EDITION 1992 AND CITY STANDARDS ON ALL DISTURBED AREAS. A PERMANENT STAND OF GRASS ADEQUATE TO PREVENT EROSION SHALL BE ESTABLISHED ON ALL AREAS NOT RECEIVING PAVEMENT PRIOR TO FINAL ACCEPTANCE. 15. AS DETERMINED BY THE SERVICE AUTHORITY, ANY DEFECTIVE, FAULTY, CRACKED, OR BROKEN SIDEWALKS, DRIVEWAYS, HANDICAP RAMPS OR CURB & GUTTER AS A RESULT OF THIS CONSTRUCTION SHALL BE REMOVED AND REPLACED PRIOR TO FINAL ACCEPTANCE. NO ADDITIONAL PAYMENT WILL BE MADE FOR SUCH WORK. EARTHWORK AND SITE CONDITIONS 1. UNLESS OTHERWISE NOTED ON THE PLANS OR IN THE SPECIFICATIONS, ALL FILL MATERIALS SHALL BE COMPACTED TO 95% OF THEORETICAL MAXIMUM DENSITY AS DETERMINED BY AASHTO T-99 METHOD A, WITHIN PLUS OR MINUS 2% OF OPTIMUM MOISTURE, FOR THE FULL WIDTH AND DEPTH OF THE FILL. ALL FILL MATERIAL UNDER PAVED AREAS SHALL BE VDOT #21 B CRUSHED STONE COMPACTED IN 6 INCH DEEP LIFTS AS DIRECTED BY PROJECT INSPECTOR OR ACSA PROJECT MANAGER. 2. ALL MATERIALS AND INSTALLATION DETAILS SHALL CONFORM TO THE ALBEMARLE COUNTY SERVICE AUTHORITY STANDARDS AND ALL OTHER APPLICABLE SERVICE AUTHORITY ORDINANCES. 3. ANY UNUSUAL OR UNANTICIPATED SUBSURFACE CONDITIONS SHALL BE IMMEDIATELY REPORTED TO THE SERVICE AUTHORITY. 4. CONTRACTOR SHALL VERIFY ALL DIMENSIONS, ELEVATIONS AND LOCATIONS PRIOR TO BEGINNING WORK, AND IMMEDIATELY NOTIFY THE ACSA PROJECT MANAGER OR HIS DESIGNEE IN THE EVENT THERE ARE ANY DISCREPANCIES BETWEEN SUCH CONDITIONS AND THOSE SHOWN ON THE PLANS AND SPECIFICATIONS.. 5. BENCH MARKS ARE AS NOTED ON DRAWINGS. HORIZONTAL AND VERTICAL DATUM ARE NAD 83 AND NAVD 88, US SURVEY FEET. 6. COMPACTION TESTING OF THE SOILS IN ROAD & PARKING RIGHT OF WAY SHALL BE CONDUCTED BY AN INDEPENDENT AGENCY AT THE CONTRACTOR'S EXPENSE, WITH THE RESULTS SENT TO THE ACSA PROJECT MANAGER. CONCRETE AND ASPHALT 1. ALL MATERIAL INSIDE FORMS SHALL BE CLEAN AND FREE OF ALL ROCKS AND OTHER LOOSE DEBRIS. SUB -BASE MATERIAL SHALL BE COMPACTED BY MECHANICAL MEANS. REMOVE ALL STANDING WATER FROM AREA INSIDE FORMS. 2. CONCRETE SHALL NOT BE PLACED UNLESS THE AIR TEMPERATURE IS AT LEAST 40' FAHRENHEIT IN THE SHADE AND RISING. DISPOSAL OF ALL EXCESS MATERIAL IS THE RESPONSIBILITY OF CONTRACTOR, AND SHALL BE PERFORMED IN ACCORDANCE WITH ALL APPLICABLE FEDERAL, STATE, AND LOCAL REGULATIONS. 7. LIMITS FOR REPAVING ARE DEPICTED ON THE PLANS. CONTRACTOR SHALL MILL AND OVERLAY ENTIRE ROADWAY WIDTH, ALL AFFECTED STRIPING SHALL BE REPLACED IN ACCORDANCE WITH VDOT STANDARDS. CONTRACTOR SHALL EXERCISE CARE, ESPECIALLY AT INTERSECTIONS AND GUTTER LINES, TO PROVIDE POSITIVE DRAINAGE. ANY AREAS WHERE WATER IS IMPOUNDED AS A RESULT OF THIS CONSTRUCTION SHALL BE CORRECTED BY CONTRACTOR AT NO ADDITIONAL COST. POSITIVE DRAINAGE OF ALL ROADWAY AREAS TO THE STORM DRAIN INLETS OR OTHER ACCEPTABLE DRAINAGE CHANNELS SHALL BE MAINTAINED AT ALL TIMES. 2. CONTRACTOR SHALL MAINTAIN EXISTING STREAMS, DITCHES, DRAINAGE STRUCTURES, CULVERTS AND FLOWS AT ALL TIMES DURING THE WORK.
