By signing this application as the owner,I hereby certify that all requirements of these plans and permits will be
complied with,and I have the authority to authorize the land disturbing activities and development on the subject
property. I hereby grant the County of Albemarle the right to enter upon the property as required to ensure
compliance with the approved plans and permits.

Notes:
1) The project alignment is predominantly contained within existing public(I/DOT)road rights-of-way.
2) In areas where private properties will be affected, permanent utility easements will be secured in the name of the
Albemarle County Service Authority,prior to construction. These easements will encompass all disturbances,
construction activities and erosion and sediment control features related to this project. By way of these permanent
utility easements,Albemarle County will have unrestricted access to private properties to ensure compliance with the
approved plans and permit,without the need for additional permissions. Refer to attached plats.
