I called you to go over this but am following up with this email. I am trying to assist David with this
project since he has 14 other plan reviews in his que. If I've missed something, please forgive me and let
me know.
Referring to David's comments (copied below) emailed on May 14th. Your email reply is included below
also further down in the chain.
As -Built:
'14. Shew the 48-in (UD) pipe and pipe eutfa# in an appmpr4ateiy named 15L44UfioGX
5• Sheets 6.03-6.05 from the design plan
should be included in as -built. -Verify siring calcs meet or exceed the design.
6. Show 48-in (UD) pipe was installed correctly per design. Show profile, weir location &
{ dimensions. -Sheet 5.04 from the design plan should be shown on the as -built. Verify
proper dimensions, materials and installation.
Additional items needed for a complete bond release:
7. ' Owner/Operator needs to update the DEQ termination form for the permanent control
measures (see attached VAR10).

need to be videoed. -This would include storm str. pipe runs: STM S6-S3-S4-SS and S8-S9.
My comments after reviewing everything to this date:
The following comments have not been addressed. I've added a little detail to help clarify. If you'd like
to review/discuss, please give me a call.
Comment 5: Roudabush still hasn't certified or addressed the information shown on Sheet C6.03
(area of biofilter, depth of biofilter mix/gravel, installation of fabric/underdrain, etc.). The as -built
drawings specifically state in Note 8.c that "this review does not imply in any way that ... to any
element or structure not visible or depicted on the physical survey." Typically, the engineer of
record (Devin Keeler in this instance) certifies the facility meets design requirements (including
certifying that the information shown on Sheets 6.04 and 6.05 haven't changed). The
owner/contractor can also provide documentation (photos) confirming the facility meets the design
requirements, or someone (F&R or Roudabush) can probe the biofilter after the fact to confirm
depths of layers. The biofilter areas should be shown on the as -built plans.
Comment 6: The as-builts still do not show the weir, orifice, or trash rack. David's comments asked
to provide as-builts of the detail shown on Sheet C5.04. These items need to be included in the as -
built drawings.
Comment 7: The termination form is still not completed. Permanent control measure #2 is not filled
out completely. Please complete all blanks on the form for the 48" underground detention system
on page two.
Addressed.
Comment 10: With all sincerity, have you looked at the videos? I cannot see anything except for the
bottom of the pipe. The purpose of the video is to confirm there are no gaps, deflection, debris, etc.
in the pipe. I will make a visit to the site in the next week and try to visually inspect this myself, as
the pipe is large enough I think to do this. If I can visually confirm, we won't need another video. In
the future, if we can't visually inspect, videos are required. I am not sure who did your video but it
appears they used a camera meant for small pipes (4 to 15 inch pipes) on the end of a cable.
Typically, a camera mounter on a crawler is used on pipes this size so the entire pipe can be seen,
the joints can be inspected, deflection can be seen (if any), and the camera is off the bottom of the
pipe so it isn't dragged through water or mud (like this one was).
I'll let you know when 1 or someone else visits the site to check the pipe. If all information is received,
reviewed and approved by the 2111, we will be able to terminate the permit before July 1.

I performed a site inspection today and noted the following:
- The riprap outlet protection at the outlet STM S9 has not been installed. This will need to be
shown on the as -built drawings. Shelbeck to install riprap between September 15th and 20th.
David please plan to re -inspect the week of September 23`d. Now that we are all working off the
same approved plans, Roudabush to include in final as -built drawings.
- The 6-inch storm drain that is supposed to collect the barn roof doesn't appear to be installed.
The inlet in the manhole has plastic covering the hole (see photo). I forgot to confirm if the
other building collector drains were installed (you'll need to confirm if these were installed).
Your engineer will need to confirm if these were actually installed, and if not, if they were
required for quantity/quality requirements. If so, they'll need to be installed or your engineer
will need to show how those requirements will be met. This should be shown on the as -built
drawings unless your engineer certifies they aren't needed. See Devin Keeler's response
attached which includes:
o Agreeing the 6" SD should have been labeled UD for underdrain.
o Approving the deletion of UD for structural soil of the Fire Lane
- There are two trees planted over the 48-inch detention facility. These need to be relocated
outside of the stormwater easement. See attached Landscape plan showing the relocation of
these tree to take place in the Fall at the same time as replacements are installed. The Fall is the
recommended time for planting and transplanting of trees.
- The 48-inch pipe looked fine except it has some gravel/mud that needs to be removed. Shelbeck
to clean a second time between September 15th and 20th. David please plan to re -inspect the
week of September 23`d
- The mulch layer was not installed in the biofilter as required on the approved plans. Shelbeck to
install mulch layer between September 15' and 20th. David please plan to re -inspect the week
of September 23`d.
- The earth is eroding on the downstream side of the overflow weir. This needs to be corrected.
Shelbeck to address erosion between September 15th and 20th. David please plan to re -inspect
the week of September 23`d
- Once all of these items are addressed, an engineer (typically the engineer of record) will need to
write a letter certifying that the facilities have been installed in accordance with the plans.
Dewberry was our Engineer of record, but Craig Kotarski of Timmons will certify all facilities are
operating as designed.
David James will be your contact from now on Please route all correspondence through him as I will
not have access to emails over the next two weeks. I have not reviewed the most recent as -built
submittal, David will do this, but since there are changes needed, I am going to suggest he wait until
corrections are made. David - I will confirm after all items have been addressed by the site contractor
and officially request a re -inspection. Roudabush has submitted several as-builts already, should they
provide final as -built drawings prior to your inspection or after? Which would be more efficient?
I apologize for the timing of inspections and review of as-builts. We typically review as-builts with a site
visit, and do not issue comments until both are completed so there isn't more than one set of
comments. But in the effort to help you close this project before the end of the month, we went ahead
and reviewed the as -built drawings without competing the field inspection, assuming everything was
completed in the field correctly. We should have waited and next time I will make sure to do so. Sincere
apologies for additional comments on the as -built drawings.
