Rivanna Community Church —SWM Easement (BE#2346) Basil, There will be two documents that will be needed 1. The SWM maintenance agreement SWM maintenance agreements are formatted a specific way and are handled through Ana Kilmer(CC'd) She will be able to help you with this going forward I have updated her on the situation [NOTE The SWM maintenance agreement is executed recorded before the plat/deed] 2- Deed of easement This template is attached The deed of easement is recorded with the PLAT Since this is an odd situation this is how I recommend going forward in the process if you choose to do it the way described in your email 1_ Contact Ana to start work on the SL"Jw1 maintenance agreement —This document is created independently for both parcels the SWM easement falls on [NOTE. Only one is needed per parcel regardless of the number of SWM easements on the parcel so It will cover the extended SWM easement for the underground storage pipe without the need for an additional SWM agreement] a- Information for Ana to format the SWM agreements IMPS. 03100-00-00-03200: 031A0-00-00-000A0 ii. MS-4: NO 2. Complete the remaining items for PLAT review with the County 3- Concurrently circulate the Deed of Easement for signature by parties of both parcels and submit to the County a. Once received by the County the (signed) deed of easement will be sent to the County Attorney's office for review and signature by the County Executive/County Attorney b- The Deed of easement will be returned to you for record at the County Clerk's Office c_ Once recorded provide the receipt of record to the County as proof of record • 4- Final PLAT approval (signature; is granted for the existing SWM facility easement 5. Once the PLAT Is prepared for the extended easement. submit the PLAT for county review_ 6. Repeat step 3 above. with signatures only for Rivanna Community Church for the new SWM easement.
