County Code section 17-410 and Virginia Code §62.1-44.15:34 requires the VSMP authority to act on any
VSMP permit by issuing a project approval or denial. This project is denied for reasons listed below. The
VSMP application content requirements can be found in County Code section 17-401.
A. Stormwater Pollution Prevention Plan (SWPPP)
The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain
(1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary.
1. VSMP Application d. 5/13/20, Item G., p. 2, indicates SWPPP is included. SWPPP not received. Please
submit SWPPP using county template; link to template:

2. Engineering needs two (2) print copies, once Plan is approved.
B. Pollution Prevention Plan (PPP) —A., above. Also, please ensure SWPPP includes I I" x 17" inserts of
ESC and SWM Plan, once comments addressed (Sec. 4. 5) and I I" x 17" SWPPP PPP Exhibit (Sec. 6.A.).
The PPP content requirements can be found in County Code section 17-404.
C. Stormwater Management Plan (SWMP)
VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This
plan is disapproved for reasons listed below. The stormwater management plan content requirements can be
found in County Code section 17-403.
1. General: Revise plan and calc. packet titles to include WP0202000030. When SWPPP is submitted, please
include reference to WPO202000030.
2. Ensure final site plan and WPO plan sets are consistent.
3. Notes:
a. Since there is no subdivision, WPO plan approval requires SWM Facility (dry retention basin) and
SWM Facility Access easements be recorded prior to WPO plan approval.
b. WPO plan approval is required prior to Site Plan approval.
4. Revise p. 18, Calc. packet to show and identify approximate location of 1% limit of analysis. Also, please
report Vi_y, within Ex. 15" CMP at 12.78cfs. CMP is likely less resistant to erosion than concrete pipe.

The total discharge to the point of analysis is 12.78 cfs, which is a conservative assumption
as it assumes the peak flows from subareas A-D occur simultaneously. The reduced
post -development 2-year flow of 12.78 cfs to the point of analysis is not erosive to the
existing 15" corrugated metal pipe outfall. The LOA point per the 1% rule is within the existing
manmade stormwater conveyance system, thus channel adequacy is provided in that the
post -development peak flow from the two-year storm is improved from existing and non -erosive.
5. Engineering appreciates (sheet 8) permeable paver installation General Notes. In addition, please review
and include any installation steps, guidance, etc. found at Virginia DEQ Stormwater Design Specification

6. Include Attached.PDF document, VSMH, Ch. 9, July 2013, 9-C.8.0 Permeable Pavement: O&M Checklist
on the plans.
7. Include visual inspection findings of condition of existing on -site pipe systems. Any existing on -site pipe
system proposed to be integrated with post -developed storm conveyance requires visual verification and
photographs of bottoms of pipe. CMP typically weathers and is subject to corrosion which may completely
consume the floor of corrugated metal pipe. Condition of pipe must be rated good or better, without visible
gaps, pipe penetration, deformation, loss of capacity, etc., for existing pipes to be approved as part of post -
developed storm conveyance system proposed with this WPO plan.
8. Please see Engineering final site plan review comments d. 6/10/20 (SDP202000044). Site plan review
comments that bear on storm conveyance should be addressed with subsequent WPO and site plans.
9. Please include table of contents to index sections of 41-p. Calculation packet.
10. Confirm Calculation packet includes and reflects offsite storm runoff that enters and transits site via pipe
beneath CSX Transportation RR tracks. Make explicit reference to off -site runoff. Revise calculations or
SWM facility design, as needed.

D. Erosion and Sediment Control Plan (ESCP)
Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan
is disapproved for reasons listed, below. The erosion and sediment control plan content requirements can
be found in County Code section 17-402.
1. Sheet 3, ESC Notes and Narrative, project description lists dates (years /months) that appear inconsistent
with anticipated completion within 6 months. Timeline appears to approach 15 months. Please revise
description for internal consistency. Please describe commercial redevelopment, along with parking and
circulation improvements. Please list 2.52 Ac. LOD using black text (more easily readable than grayscale).
2. Sheet 3: Include Sec. 8.1. of VA DEQ Stormwater Design Specification No. 7 on the ESC Plan. Link:

3. Show /label existing contours at 1' intervals, or interval sufficient to evaluate direction of flow during
phase 1 development.
4. Provide spot elevations along proposed diversion dike west of 2415 building. County GIS appears to
indicate elevation gain as diversion leads north to the back side of the site, crossing a higher contour.
5. Make Phase I ESC measure symbols dark text, for example: IP at DI to be lowered from 560.7 to 560.4,
which leads to pipe labeled `Existing storm shall remain to convey sediment trap outflow.'
6. Recommend notes and linework preserve a portion of existing pavement to be removed to serve as
temporary paved construction entrance. It seems intuitive to make use of existing pavement at edge of U.S.
Rt. 250 as a paved construction entrance. Engineering is not opposed to this.
7. Provide spot shot elevations that indicate grade change that occurs to decrease area to STl and increase
area to ST2, between phase I and 11. Clearly label ST drainage divides in Phase I, Phase II.
8. Provide SAF, especially in vicinity of U.S. Rt. 250.
9. Coordinate SAF /other VDOT-approved barriers required to make improvements within U.S. Rt. 250 RW.
10. Obtain VDOT land use permit for activity within VDOT RW.
11. To extent practical, provide pedestrian transit of site during as many phases of site development as is safe,
or practical. No pedestrian facilities exist within VDOT RW, nor do pedestrian facilities exist along EBL,
U.S. Rt. 250. The existing paved walk is internal to site, and there are phases of construction when it must
be closed, but may be times it may remain safely open for pedestrian use.

12. Recommend revise Sheet 3 Notes to describe /define phases of construction during which existing paved
walk may remain open. Include Notes directing contractor to provide signs alerting pedestrians of
construction activity, risk /hazard, or No Trespassing.
13. Provide SAF that corresponds with Ex. paved walk open or partially -open periods. Paved walk: image,
below.
14. Provide `Construction Ahead' signs on plans as needed for land disturbance or construction activity within
VDOT RW (water lateral replacement, for example)
15. Location of construction parking /staging with CRS at SW comer of 2415 building appears unworkable.
Access to this location is obstructed on all sides (diversion dikes, ST2, Ex. tree to remain). Please revise.
16. Provide RW diversion at paved construction entrance (PCE)-U.S. Rt. 250 to limit amount of material that
tracks onto U.S. Rt. 250. Divert RWD runoff to a suitable trapping measure. Plan note indicates PCE with
onsite wash rack down gradient from R/W diversion but linework does not appear to indicate RWD.
Existing contours do not indicate where a wash rack might be located in a narrow, pre -developed space.
17. Label /identify feature between ST2 and existing inlet in parking lot. Define feature /purpose.
18. Sheet 5, 6: Label improvements (linework) between 2415 building and RWSA Ex. W.
19. Sheet 4, 7: Recommend increase volume of STI, if possible. STI weir outlets to an Ex. MIL and thence to
existing piped conveyance along U.S. Rt. 250. This existing conveyance skirts U.S. 250 (either WBL or
EBL' oath 1, 2, below) before daylighting to parallel U.S. 29 SBL on -ramp. A stream eventually winds and
enters a residential community, discharging to a pond. Any upset at 2415 Ivy Road will be visible in this
private residential community pond. CE is reminded of imperative to avoid off -site downstream impacts
during construction. Proposed wet and dry volumes of STI are just within specification. A high rainfall
event may lead to near -immediate visible off -site impact, complaints, and urgent need to respond. GIS

20. Provide ST design for smaller STI, ST3.
STI, ST3 exist in larger and revised, smaller sizes.
21. In each view of sediment traps, label floor dimensions.
22. In each plan view label STI, ST2, ST3 outfall weir L x W x D.
The VSMP permit application and all plans may be resubmitted for approval when all comments have been
satisfactorily addressed. Please provide digital version of revised plan /SWPPP with a completed application foam.
Process:
After approval, plans will have to be bonded. The bonding process is begun by submitting a bond estimate request
foam and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and
check parcel and easement information based on the approved plans. The County's Management Analyst will
prepare bond agreement forms, which will have to be completed by the owner and submitted along with cash,
certificates or sureties for the amounts specified. The agreements will have to be approved and signed by the County
Attomey and County Engineer. This may take 24 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also have to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will have to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will enter project information in a DEQ database for state
application processing. DEQ will review the application information based on local VSMP authority approval. At
this time, the DEQ portion of the application fees will have to be paid directly to the state. For fastest processing,
this is done electronically with the emails provided on the application. DEQ should notify applicants with
instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This
should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference.

Applicants must complete the request for a pre -construction conference foam, and pay the remainder of the
application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid.
This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the
County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and
grading permit will be issued by the County so that work may begin.
