This Written Warning is being issued because of the following deficiencies: Deficiency: At time of inspection observed slopes are showing signs of scour damage and inadequate vegetative cover Corrective Action: Lime, fertilizer, and seed needed. Recommend a seed mix for severe slopes Deficiency: Observed areas that were not stabilized with an adequate ground coverage. Sign of erosive flow. Corrective Action: Deficiency: Cover or grade all piles of removed sediment. Corrective Action: Stabilize or grade out to prevent sediment from returning to the traps it was removed from. Deficiency: Observed areas that are in need of stabilizing Corrective Action: Amend and seed Deficiency: All traps are in need of stabilizing on the inner and outer walls. They also need to be cleaned out to ensure adequate wet and dry storage. At visit, the top most trap is almost completely filled with sediment. The existing roadside conveyance channel is inadequate as it is currently, needs to be widened and lined with rip -rap, check dams need to be installed at the required spacing to help reduce velocity of the Stormwater. Traps need to be maintained better to prevent sediment from being released without adequate settling time. Establish a good cover of vegetation on th inner and outer walls to prevent scour.
