Alan, this is fine.
Please submit VSMP Application, 2 copies (Attached) + fee ($145 —this is half total; remaining half of the Application fee
is due at pre - construction meeting), Check blocks A. — H.
Block A. — Owners: include owner /s of property to receive Mitigation planting (owners must sign). List all TMPs affected
by site work, driveway, or Mitigation Plan.
Block B. —List total Ac. proposed to be disturbed, 0.98 Ac. Please confirm this value is accurate —shown as 42,500 SF on
plan.
Block C. — Complete Attached General VPDES Permit Registration Statement. [Not strictly required since <1.0 Ac., so
close, however, we request this form to be safe.] Submit with Application.
Block F. /G. — check, but include no documents. Geoff Elsie will work with builders, will require single - family SWPPP as
condition of building permit, each Lot —this addresses F. /G. requirements. May include note to this effect.
resources / stream are protected with SF /riprap culvert outfall protection during driveway construction, and are
protected from post - developed runoff via ELSs design.

1. I've tweaked the Culvert 1 inverts and calculations to match.
2. Added dimensions and a detail for rip rap outlet protection for Culvert 2.
3. Added the missing information for LS 4 on the plan.
What do we do next?

1. Culvert 1, INV Elev. DN =951.0 (culvert l.PDF); compare with plan — please revise plan.
2. Please dimension culv. 2 riprap outfall protection: L X W x D + class stone (rich detail, may have overlooked).
3. Please provide DA for LS 4 Qio — should not prove critical (for consistency). Design L sufficient for Q2 discharge
0.93 CFS).
4. Mitigation Plan is Acceptable; comments addressed.
Alan, design excels.
