Notable differences, all of which have been discussed in the field and to Bohler's knowledge as of the date of this document, obtained required approvals, from the field condition to the approved plans, including Field Revisions, are listed below: Proposed storm pipe material has been revised from RCP to HDPE Proposed endwall structure C-03 removed to allow for the continuation of HDPE pipe to structure C-02 as part of the approved Site Plan for Block 8A, named "200430 Senior Living Facility (Block 8A) FSP Rev 8 — Approved" and dated 4/30/2020. Level spreaders proposed at Polo Grounds Road station 16+00 and 22+50 have been removed as SWM is being treated and accounted for upstream as part of proposed design plans under separate cover, not prepared by Bohler. w .BohlerEngineeiing.com BOHLER Brookhill - Road Improvement Plans BE # V 152000 7/21 /20 Page 2 of 2 Pavement Sections have been revised to include for Stella Lane (shown as Archer Avenue on Design Plans) to increase the thickness of IM-19A from 1.5" to 2" and decrease the thickness of Base BM-25A from 4" to 3.5" Pavement Sections have been revised to include for Archer Avenue (shown as Stella Lane on Design Plans) to increase the thickness of IM-19A from 1.5" to 3" and decrease the thickness of Base BM-25A from 5" to 4" Lastly, Bohler is in receipt of the following documents which Froehling & Roberson (F&R) prepared, summarizing the onsite construction activity throughout the construction process for Polo Grounds Road, Archer Avenue, and Stella Lane: • Soil Report • CBR's and Pavement Design Worksheet • Concrete Testing Reports • Stone and Asphalt Testing Inspections Based solely upon the above -referenced one (1) site visit, the As Built Data, and the content of the field reports which F&R provided, as referenced above, to the best of Bohler's actual knowledge, as of the date of this letter, and to the extent same is within Bohler's scope of services as the Civil Engineer, the road improvements, which are within Bohler's scope of services for the Brookhill Development and which were visually observable, have been installed in general accordance with the Brookhill Road Improvements for State Route 643 Polo Grounds Road, Stella Lane and Archer Avenue (approved Date, and last revised 10/30/18) which Bohler prepared, and associated field revisions. All discovered discrepancies have been reported and were either corrected on the project site or brought to the attention of the appropriate licensed design Professional Engineer of Record for resolution (no discrepancy log is attached). Dillard Summary: Roudabush Pipe Inspection Services conducted a storm sewer video inspection video inspection between July and August 2020 at Brookhill apartments in Albemarle County at the request of A.G. Dillard The purpose of this inspection was to verify proper installation and function of the newly installed storm pipe system to VDOT standards. The facility was inspected with a Pearpoint P350 wheeled crawler camera with a pan/tilt/zoom Camera. Wincan pipe inspection software was used to document the inspections and produce an inspection report of the SWM facility. The inspection standards used were NASSCO PACP and VDOT VTM 123. Findings: During the initial inspection in July 2020, several pipes were identified as requiring cleaning due to sediment ingress from adjacent construction. These pipes were cleaned by a Jetvac cleaning system by RKM Environmental in July 2020. A follow up inspection was conducted in August 2020. Following cleaning, all pipes appear to be in acceptable condition in accordance with VTM 123 and VDOT Road and Bridge Specification. 302.03(d).
