Remove all trash and unnecessary erosion control devices. Remove all advertising signs and construction mailboxes. ❑ All reconstructed, repaired, and bare/denuded areas within right -of- way have been seeded & stabilized. Fill in behind Curb ❑ Mow all areas within the right-of-way including all drainage easements. Cut back all brush and trees within drainage easement areas. ®Complete All post larger than 4"x4" shall be drilled with a hole configuration equivalent to VDOT standard 602.3. ❑ Grade all shoulders and ditch lines to the approved plan cross section. - Ditch line at entrance holding water. Investigate and remediate ❑ Inlet and outlets of all culverts are to be graded and cleared of debris to provide positive drainage. - Clean debris from all DI's remove any unnecessary inlet protection. Visually inspect pipe to ensure no construction debris has entered the pipes. ❑ Inlet and outlets have ladders and invert shaping per design standard. Lifting eyes patched and all brick areas parged. - These should be inspected, any issues should be remediated. All manholes will be opened and inspected during the final inspection. ® Complete All bent or damaged culverts to be repaired or replaced. ❑ Utility In -Place -Permits and Quit claims of any companies with facilities in the right of way. Contact the utility companies who will complete and submit forms to VDOT. Recommend requesting copies for your files. • Have: Century Telephone, Ting, Dominion • Don't Have: Charlottesville Gas, ACSA ® Complete Ensure all valves and manholes are flush with the grade. ® Complete A video camera inspection is required for all storm sewers and culverts that are deemed inaccessible to VDOT inspections and are to be accepted into VDOT's maintenance program and ownership. The video inspection shall be conducted in accordance with VDOT's video camera inspection procedure. Accepted based on VDOT review of submitted materials and letter provided by Roudabush, Gale, & Associates. ❑ Under Drain Report - I don't see this in the binder. Were any UD's or CD's installed/called for. ® Received Recorded subdivision plat for this section: two normal size paper copies and digital copy. ❑ As -built Plan: two normal size paper copies and digital copy. - Need approved As -built from the County ❑ All agreements must be finalized and copies submitted prior to Final Acceptance: - Landscape Maintenance Agreement and LUP ❑ All stop signs require 16" above for state route markers. ❑ Signing and Pavement Markers - Replace all damaged signs - Additional "No Parking" signs will be needed to make the no parking zone clear - Sign foundations should be < 4", STP-1 ❑ CG-3 Curb in median should have a reveal <1". Curb reveal is not consistent along the median. Pleas provide additional information about this issue. ❑ Reverse curb north of Roundabout will push water into the roadway and hold water over top of the joint instead of in the gutter. Additionally, it appears there will be drainage issues in this location with the commercial entrance pushing water into the center of the intersection. See CG-11 Standard for location of flow line across a Commercial Entrances. There is currently no DI to accept this drainage. Please investigate this area and provide information regarding this concern. Until this concern is addressed this segment will not be considered for acceptance.
