RE: Private Street Request for 5`s Street Commercial

Attached is an application and plans for a preliminary subdivision plat for 5' Street Commercial. Below
are responses to Section 14-234 (A)(1)(a) to authorize a private street easement.
(a) The request shall include:
(i) A map of the subdivision having contour intervals of not greater than twenty (20) feet
showing the horizontal alignment;
✓ S-101: Contours shown on existing conditions plan.
(ii) Field -run profiles and typical cross -sections of the proposed streets;
✓ S-102: Road profile and typical section provided.
(iii) The maximum number of lots to be served by each private street; and
✓ The private street will serve TMP 07600-00-00-055A, TMP 07600-00-00-055C,
and Lots 1 and 2.
(iv) Documentation explaining how the perpetual maintenance of the private street including,
within the development areas, the curb, curb and gutter, sidewalks, and planting strip
landscaping will be funded, and identifying the person or entity that will be responsible for
maintaining the improvements.
✓ Perpetual maintenance will be the responsibility of FTV Investments, LLC
