ANY DAMAGE CAUSED TO SUCH ITEMS SHALL BE REPAIRED OR REPLACED BY CONTRACTOR AT NO ADDITIONAL COST. PROPERTY PINS DISTURBED BY CONTRACTOR THAT ARE NOT SHOWN ON THE PLANS TO BE DISTURBED SHALL BE RESTORED BY LICENSED SURVEYOR AT THE CONTRACTOR'S EXPENSE. 6. MEASURES TO CONTROL EROSION AND SEDIMENT SHALL BE PROVIDED PURSUANT TO AND IN COMPLIANCE WITH CURRENT FEDERAL, STATE AND LOCAL REGULATIONS. THE INFORMATION CONTAINED IN THE CONSTRUCTION PLANS AND/OR APPROVAL OF THE PLANS SHALL IN NO WAY RELIEVE THE CONTRACTOR OR HIS AGENT OF ANY LEGAL RESPONSIBILITY WHICH MAY BE REQUIRED BY THE CODE OF VIRGINIA OR ANY ORDINANCE ENACTED BY THE COUNTY OF ALBEMARLE. CONTRACTOR SHALL PROVIDE THE NECESSARY DIVERSION DITCHES, DIKES, OR TEMPORARY CULVERTS REQUIRED TO PREVENT MUD AND DEBRIS FROM BEING WASHED ONTO THE STREETS OR ADJACENT PROPERTY. CONTRACTOR'S VEHICLES SHALL BE KEPT CLEAN TO PREVENT MUD OR DUST FROM BEING DEPOSITED ON STREETS. NO AREA SHALL BE LEFT DENUDED FOR MORE THAN SEVEN (7) CALENDAR DAYS. 7. CONTRACTOR SHALL CLEAN UP, RESTORE, SEED AND MAINTAIN ALL DISTURBED AREAS IMMEDIATELY UPON COMPLETION OF WORK. TOPSOIL SEED, FERTILIZER AND MULCH SHALL BE PLACED IN ACCORDANCE WITH COUNTY OF ALBEMARLE STANDARDS ON ALL DISTURBED AREAS. A PERMANENT STAND OF GRASS ADEQUATE TO PREVENT EROSION SHALL BE ESTABLISHED PRIOR TO FINAL ACCEPTANCE. ALL EROSION AND SEDIMENT CONTROLS SHALL BE REMOVED WITHIN 30 DAYS AFTER THE PROJECT IS STABILIZED. (MS-18) 8. FOR FURTHER REQUIREMENTS AND DETAILS OF TREE PRESERVATION, PLANTING, EROSION AND SEDIMENT CONTROL, REFER TO THE VIRGINIA EROSION AND SEDIMENT CONTROL HANDBOOK. 9. AN EROSION AND SEDIMENT CONTROL CERTIFIED RESPONSIBLE LAND DISTURBER (CRLD) IS REQUIRED FOR ALL LAND DISTURBANCE ACTIVITIES. 10. THE CONTRACTOR SHALL PROPERLY INSTALL AND MAINTAIN EROSION AND SEDIMENT CONTROLS FOR THE LIFE OF THE PROJECT; AND ROUTINELY CHECK CONTROL DEVICES BEFORE, DURING AND AFTER STORM EVENTS. 1. PRIOR TO CONSTRUCTION OR EXCAVATION, THE CONTRACTOR SHALL ASSUME RESPONSIBILITY OF LOCATING ANY AND ALL UNDERGROUND UTILITIES (PUBLIC OR PRIVATE) THAT MAY EXIST WITHIN OR CROSS THROUGH THE AREA OF CONSTRUCTION WHETHER OR NOT THEY ARE SHOWN ON THE PLANS. PRIOR TO ANY CONSTRUCTION ACTIVITY, THE CONTRACTOR SHALL CALL 'MISS UTILITY OF VIRGINIA' AT 1-800-552-7001. THE CONTRACTOR SHALL BE RESPONSIBLE FOR REPAIRING, AT HIS SOLE EXPENSE, ANY EXISTING UTILITY DAMAGED DURING CONSTRUCTION. 2. THE PLAN DOES NOT GUARANTEE THE EXISTENCE, NONEXISTENCE, SIZE, TYPE, LOCATION, ALIGNMENT, OR DEPTH OF ANY OR ALL UNDERGROUND UTILITIES OR OTHER FACILITIES. WHERE SURFACE FEATURES (MANHOLES, CATCH BASINS, VALVES, ETC.) ARE UNAVAILABLE OR INCONCLUSIVE. INFORMATION SHOWN MAY BE FROM UTILITY OWNER'S RECORDS AND/OR ELECTRONIC LINE TRACING, THE RELIABILITY OF WHICH IS UNCERTAIN. BACKFILL SHALL BE CAREFULLY PLACED AND COMPACTED TO PREVENT FUTURE DAMAGE OR SETTLEMENT TO EXISTING UTILITIES. ANY UTILITIES REMOVED AS PART OF THE WORK, AND NOT INDICATED TO BE REMOVED OR ABANDONED, SHALL BE RESTORED USING MATERIALS AND INSTALLATION EQUAL TO THE UTILITY'S STANDARDS. 4. CONTRACTOR SHALL NOTIFY LANDOWNERS, TENANTS AND THE ENGINEER A MINIMUM OF 72 HOURS PRIOR TO THE INTERRUPTION OF ANY SERVICES. SERVICE INTERRUPTIONS SHALL BE KEPT TO A MINIMUM. 5. Prior to any construction within any existing public right-of-way, including connection to any existing road, a permit shall be obtained from the Virginia Department of Transportation (VDOT). This plan as drawn may not accurately reflect the requirements of the permit. Where any discrepancies occur the requirements of the permit shall govern. 2. Construction inspection of all proposed roads within the development will be made by the County. If a subgrade soil CBR of 10 or greater is not obtainable, a revised pavement design shall be made by the design engineer and submitted with the test results for approval. 6. All slopes and disturbed areas are to be fertilized, seeded and mulched. The maximum allowable slope is 2:1 (horizontal:vertical). Where reasonably obtainable, lesser slopes of 4:1 or better are to be achieved. 7. Surface drainage and pipe discharge must be retained within the public right-of-way or within easements prior to acceptance by the County. All drainage outfall easements are to be extended to a boundary line or a natural watercourse. 8. Guardrail locations are approximate. Exact length, location and appropriate end treatments will be field determined at the time of construction. Additional guardrail may be required at locations not shown when, in the opinion of the Director of Engineering, or the Directors designee, it is deemed necessary. When guardrail is required, it shall be installed four (4) feet offset from the edge of pavement to the face of guardrail, and roadway shoulder widths shall be increased to seven (7) feet. A minimum clear zone of eight (8) feet from the edge of pavement shall be maintained on all residential subdivision streets. 13. Compliance with the minimum pavement width, shoulder width and ditch sections, as shown on the typical pavement section detail, shall be strictly adhered to. Exceptions will not be allowed. 14. Upon completion of the roadway construction, street name signs shall be installed by the developer at all street intersections. Signs and their installation shall conform to "Albemarle County Street Name Sign Requirements". The developer shall be responsible for maintenance of signs until roads have been accepted. 15. Road plan approval is subject to final subdivision plat validation. Should the final plat for this project expire prior to signing and recordation, then approval of these plans shall be null and void. 16. Traffic control or other regulatory signs or barricades shall be installed by the developer when, in the opinion of the Director of Engineering, or the Director=s designee, they are deemed necessary in order to provide safe and convenient access. The items in this list must be completed in the order given. 1. Tests and Inspections: Have all necessary tests and inspections performed by a professional engineer or geotech, or VDOT certified inspector. This should include, at a minimum, a. California Bearing Ratio (CBR) tests, b. stone depth inspections, including installation of under -drains and cross -drains, c. fill compaction tests, d. pipe and drainage structure video inspections or equivalent, e. base and surface pavement inspections, documenting materials, thickness and compaction f. concrete tests g. structural and related inspections for any bridges and foundations Compile a report of these tests, certified by a professional engineer, and submit this report with the as -built plans. 2. As -built plans and plats: Provide drawings of the constructed improvements according to the Albemarle County As -built Road Plan Policy. Provide copies of all recorded drainage easements, sight easements, and right-of-way plats. Plats should be copies of the actual recorded documents from the Clerk's office. Before completing as -built drawings, make any corrections necessary to ensure all improvements are within right-of-ways and easements. 3. Bond inspection: Request a bond reduction or release inspection from the County, which will reduce or release your bond, indicating completion or partial completion, or generate a letter indicating items in need of documentation or completion. The following documents will be needed with your bond inspection request; A. Completed Bond Inspection Request form and fee A form and fee are required for each bond, such as for a project with multiple phases and bonds. B. As -built documents per items 1 and 2 above. An as -built plan prepared in accordance with the County's As -Built Road Plan Policy is required. For any releases or reductions, the minimum requirements of this policy must be met, if only to ensure that all improvements are in right-of-way and easements. C. Signed and sealed letter from a professional engineer listed and certifying that completed improvements are built according to plan. Please do not e-mail or fax copies of this letter. An original signature and certifying seal are required. Please do not use qualifying statements such as "it appeared", or "to the best of my knowledge", or "generally in accord". Improvements were either built to plan, or they were not. As -built measurements, construction inspections, and other field verifications should be cited and included. Tolerances should be noted. The items not built according to plan must be listed, with explanations. Deviation in pavement materials and thicknesses must be listed. If street trees or sidewalks, or other items were moved, or added, this should be noted. If drainage changed, by addition or deletion of culverts, inlets, or re -alignments of pipes or grades, this should be verified by revised computations and attached to the letter. If there are outstanding items or omissions, these should be listed. Graphics are helpful. In short, please provide more than a statement of opinion or assurance. Please demonstrate that the improvements and construction have been inspected, investigated and documented, and are certified in detail, and county staff will not be in the position of discovering discrepancies and deficiencies in the field. D. Completed Certificate of Completion This form must be received for release of a bond. It is available on the county website. In the case of reductions, a letter listing the outstanding items that need to be completed before signing the certification is acceptable. It must be signed by the owner. This form says all subdivision improvements have been completed. It also says that all construction conforms to approved plans and any discrepancies have been approved by the County.
