RE: Review of Proposed Subdivision Plat and attached Soils Information for Individual Onsite
Sewage Systems as part of a division of Tax Map 72 Parcel 19 located in Albemarle
County, Virginia (SUB2021000017),
Dear Mr. Langille:
On June 4, 2021, the Comity of Albemarle requested the Virginia Department of Health (via the Albemarle
County Health Department) review the proposed subdivision plat identified above. This letter is to inform you
that the above referenced subdivision plat is approved for individual Onsite Sewage Systems in
accordance with the provisions of the Code of Virginia, the Sewage Handling mud Disposal Regulations,
and lord ordinances.
This request for subdivision review was submitted pursuant to the provisions of § 32.1-163.5 of the Code oJ'
Virginia which requires the Health Department to accept private soil evaluations and designs from an
Authorized Onsite Soil Evaluator (AOSE) or a Professional Engineer working in consultation with an AOSE for
residential development. This subdivision was certified as being in compliance with the Board of Health's
Regulations by: Steven K. Thomas, CPSS It3401000006. This subdivision approval is issued in reliance
upon that certification,
Pursuant to § 360 of the Regulations this approval is not an assurance that Sewage Disposal System
Construction Permits will be issued for any lot in the subdivision identified above unless that lot is specifically
identified on the above referenced plat as having an approved site for an onsite sewage disposal system, and
unless all conditions and circumstances are present at the time of application for a permit as are present at the
time of this approval. This subdivision may contain lots that to do not have approved sites for onsite sewage
systems.
This subdivision approval does pertain to the requirements of local ordinances.

Dear Mr. Gatobu:
On July 31st, 2007, the County of Albemarle requested the Virginia Department of Health (via the Albemarle
County Health Department) review the proposed subdivision plat identified above. This letter is to inform you
that the above referenced subdivision plat is approved for individual onsite sewage systems in accordance with
the provisions of the Code of Virginia, and the Sewage Handling and Disposal Regulations (12 VAC 5-610-10
et seq., the "Regulations"), (and local ordinances if the locality has authorized the local health department to
accept private evaluations for compliance with local ordinances). This request for subdivision review was
submitted pursuant to the provisions of § 32.1-163.5 of the Code of Virginia which requires the Health
Department to accept private soil evaluations and designs from an Authorized Onsite Soil Evaluator (AOSE) or
a Professional Engineer working in consultation with an AOSE for residential development. This subdivision
was certified as being in compliance with the Board of Health's regulations by:

This subdivision approval is issued in reliance upon that certification.

"traditional systems," however actual system designs may be different at the time construction permits are
issued.

19 located at 381 Miller School Rd. near Crozet in Albemarle County, Virginia.
The purpose of this investigation was to locate 'suitable soils for the installation of septic tank
absorption field reserve area for an existing single-family dwelling. The reserve area will
support a three bedroom home.
Soils that meet the minimum standards as set forth in the Sewage Handling and Disposal

Regulations, revised July. 2000, were located in the vicinity of the flagged areas.
Please refer to the soil evaluation forms, abbreviated design forms, plat or drawing, and J
comments page for specifics concerning the drainfreld areas.

COMMENTS AND GUIDELINES
All interpretations are based on the Sewage Handling and Disposal Regulations, revised July 2000
Class 3C wells (private wells) must be kept at least 100 ft up slope from drainfields. Class 3B wells are noted on
the plat or drawing.
Class 2B wells are required for public drinking water. A well lot 100-ft. by 100-fl. must be shown on the plat and
recorded.
Building sites located down slope from drainfields will require a pump system. Pump systems are noted on the plat
or drawing
Lateral groundwater movement interceptors (LGMI) or french drains, if necessary, are noted on the plat or drawing
Blue flags mark the approximate comers of the dtainfields and pink flags mark the location of the auger holes and/or
backhoe pits, unless otherwise noted.
If this investigation is for a private residence, dminfields are sized for three bedroom homes unless otherwise noted.
