1) As-built. Provide drawing information:
a) Stationing along road centerlines.
b) Sheet 5, etc. Missing road section beyond Sta. 22+00 to phase line(23+30).
c) Provide at least 20 ft wide road EOP per design or as shown in as-built typical section.
d) Show remaining driveway and culvert locations.
2) Provide engineers letter certifying improvements are built to plan.
3) Provide signed certification form by owner/developer.
4) Provide copy of recorded plats.
5) Provide pipe trench reports.
6) Provide additional compaction/soil stabilization reports(proof rolling).
7) Provide CBR test locations map(1 every 0.1 mi. of road). Additional testing may be needed.
8) Provide core test report&locations(station) of road pavement thicknesses(surface/base). The
report didn't specify test locations(station). (2)minimum for each road.
9) Pipe video. For pipe runs(please re-video):
a) A10-A9 - Clean out rubble in pipe.
b) A9-A8—Re-video. Cannot see bottom half of pipe (underwater).
c) B6-B5—Please provide video.
d) E2-E1 —The pipe is 12-ft short per design (50-ft). There is rill erosion happening from outlet end
to sediment basin. Install additional length of pipe.
e) E3-E2—Video shows pipe deformation that starts about 60 ft in and ends at about 140 ft.
Recommend replacement.
There's medium to large gap between pipe joint at 100 ft, 120 ft and 140 ft into video. Seal the
joints at those locations.
f) E5-E4—Video shows deformation that's severe and likely to fail. Pipe segment will need to be
replaced. Deformation starts at about 5 ft in and ends at about 25 ft. Recommend replacement of
entire length.
There's medium joint separtation at 12.3 ft, and 51.8 ft into video. Seal the joints at those
locations.
Pipe deformation at about 71.5 ft. Recommend replacement.
10) Field report/inspection.
a) Parge lifting holes on inlet tops.
b) Install street trees.
c) Install surface asphalt.
d) Install remaining driveway culverts.
e) Install road cross drains&EW-12's.-not found
f) Drill 2" holes for the address signs (on each face).
g) Install end section at A8.-not found
h) Install pipe B6-B5, end section and outfall protection.-notfound
i) Provide at least 20 ft wide road EOP to match the typical section of plan.
j) Complete section of road(Sta. 22+00 to 23+30).
