Based on the information filed with you application and attempts by the Department's representative(s)to conduct soil and site evaluations,we are unable to process your application for a Sewage Disposal System Construction Permit for one or more of the following reasons: Unable to identify property boundary lines—property lines need to be located/flagged. Unable to identify site contours due to excessive underbrush—drainfield area needs to be cleared of underbrush. Proposed house location not adequately located/identified—proposed house location needs to be staked. Unable to perform soil evaluation with hand auger—need to have backhoe pits dug(call our office to coordinate time for EHS to meet on site with backhoe). X_ Other: Need written variance from Albemarle County for drainfield to be installed on slopes that exceed 25%. When written copy of variance is obtained it may then be possible to issue septic and well permit. , Please refer to the information provided to you at the time of your application regarding site preparation and minimum requirements for an onsite sewage disposal site and soil evaluation. The Albemarle County Environmental Health Office is committed to providing a timely and professional evaluation of your site as well as the sites of other customers whose sites are ready for evaluation. In order to proceed with your on-site sewage disposal system evaluation, a new application must be submitted. If you reapply within 90 calendar days (May 2,2006), no additional fees are required. In accordance with the Virginia Administrative Process Act, Chapterl.l:l and Title 32.1-164.1 of the Code of Virginia,this letter is to inform you of your right to appeal the decision of the local health department,which is specified in this letter. If you desire to pursue your right of appeal,you may at your discretion be represented by counsel. The first step in the appeal process is to submit within 30 days to Dr. Susan McLeod,Thomas Jefferson Health District Director, a written request detailing and outlining all the facts and such other data or information which would support your request for an appeals review of the decision that has been rendered.
