I attempted to call a couple of times for some answers but the first time I had to leave a message and the 2"d time after I pushed #3 the phone just kept ringing several times then hung up. We are purchasing this Massage Envy on November In and wanted to be sure we filled this application out correctly. As we couldn't wait any longer, I am hoping someone will call if we made any mistakes. This will be a change of ownership only. We are not making any changes to the location. We are unsure about page 3. We believe this is the owner of the shopping mall where the Massage Envy is located. We can mail them a copy as soon as this is confirmed and that we need to do so. Please let us know if you have any questions or if we need to make any changes to this application.
