This project is approved, but please note items at SWPPP, 2.c./d., required for a Grading Permit. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. SWPPP 1. Please ensure SWPPP cover includes reference to WP02021-000 (Rev. 1) Addressed. 2. Feel free to call if any questions. b. Sec. 6.A.: PPP Exhibit: Please show initial location of: (Rev. 1) Addressed. i. Rain gauge. ii. Portable sanitary facilities (porta-john), as required. iii. Covered non -hazardous waste dumpster, if required. iv. Vehicle wash waters, draining to trapping measure (Not a sediment trap design, per se, but shallow depression — 1-2 backhoe buckets. Avoid direct drainage to Ex, storm system, or pond/s.) v. Concrete wash -out. vi. On -site fuel, if required. vii. Paint, stucco, chemical storage, if required. c. Sec. 6E: List named individual responsible for PPP measures. (Rev. 1) Persists. Please list a named individual: required for Grading permit, but VSMP /WPO plan may be approved without. d. Sec. 8: Although county personnel will inspect, Applicant is required to retain qualified (contractor/third-party) E&S inspection personnel to perform ESC inspections and evaluate compliance relative to VESCH, Yd Edition, 1992. (Rev. 1) Addressed. B. Pollution Prevention Plan (PPP) —see SWPPP item 2.b.. above (Rev. 1) Addressed. The PPP content requirements can be found in County Code section 17-404. Engineering Review Comments Page 2 of 3 C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is approved. 1. Please revise plan set title to: Worrell Drive Improvements VSMP IWPO2021-00070 (Rev. 1) Addressed. 2. Sheets C8.0, C8.01, C12.00, C12.01, C12.02, C13.00, included with the ROAD plan, should be removed from the VSMP/WPO plan. (Rev. 1) Addressed. 3. Sheet 28 of 35-sheet plan set, titled Topeka's Steakhouse (sheet 10 of 10), appears intended as an exhibit but Worrell Drive lies at elevation higher than drainage areas X, Y, Z, it is not immediately clear that this past design is relevant to Worrell Drive Improvements. Please place sheet 10 exhibit in context of Worrell Drive Improvements. (Rev. 1) Addressed. 4. Please revise CI 1.04, CI 1.05 HydroCad subcatchment labels to correspond with Pond 1, 2, 3, 4 drainage areas, or easily -identifiable identifiers from CI 1.01 so routings may be more -easily interpreted in context of DAs. Additional comments possible. Note: A HydroCad schematic/flow diagram with subcatchment areas should be included as index to guide review. (Rev.1) Addressed. 5. It is unclear if this VSMP /WPO plan proposes to rely on 9VAC25-870-47 or 9VAC25-870-48 (Criteria IIB; prior -approved plan; existing VAR10). Provide stonnwater quality and quantity narrative/s explaining: design, criteria, compliance regime (IIB or IIC design criteria, as applicable). Provide VaRRM.xls, as needed. Provide channel /flood protection calculations. Additional comments are possible. (Rev. 1) Addressed. 6. C9.00, Storm sewer 5-1 to 5-2: Provide a high -flow bypass design for level spreader (LS) in fill section. If bypass channel is located in earth fill, armor bypass against erosion. Provide channel lining design for LS high -flow bypass (stone (below LS), EC-2, EC-3). (Rev. 1) Addressed. 7. Note: An Approved WPO plan is required prior to receiving a Grading Permit. (Rev. 1) NA, plan is approved, but please see items at SWPPP, 2.c./d., above, also required for a Grading permit. D. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is approved. 1. C4 series a. Provide ESC legend. (Rev. 1) Addressed. b. Label Area of disturbance (LOD = x.xx Ac.). (Rev. 1) Addressed. c. Provide SAF, as needed, to protect pedestrians using Ex. path/s from construction activities, and to discourage trespass. (Rev.1) Addressed. 2. C5.00: Provide note discussing disposition of export /demo material: `Waste asphalt to landfill,' for example. (Rev. 1) Addressed. 3. C6.00: Label reading `Maintain access to Martha Jefferson Dr and Worrell Drive at all times' appears at odds with MOT, which appears to indicate episodic, brief closure of Worrell Drive. (Rev. 1) Addressed. 4. C7.00. C7.01 a. Provide L x W dimensions for outfall riprap. At Str. 3-1, 4-1, 5-1 dimensions appear to reference LS design. If this is the case, provide LS section/s to help correlate dimension labels (C7 series) with LS design. Several pipe outfalls without level spreaders have Vol. (cy) labels, which are helpful, but L x W x D (EC-1 Type IA) labels will aid plan review, bond estimate, installation, and inspection. (Rev.1) Addressed. b. Provide grade contours at 2-2 (or spotshot) to ensure positive drainage to grate. (Rev. 1) Addressed. c. Provide pond 2 WSEL and structure 6-1 INV out label on this plan sheet. (Rev. 1) Addressed. Engineering Review Comments Page 3 of 3 Process: After approval, plans will have to be bonded. The bonding process is begun by submitting a bond estimate request form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will have to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will have to be approved and signed by the County Attorney and County Engineer. This may take 24 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements, if required (required if SWM facilities are approved), will also have to be completed and are recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will have to be submitted along with court recording fees. After bonding and agreements are complete, county staff will enter project information in a DEQ database for state application processing. DEQ will review the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will have to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter. This should be copied to the county. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants must complete the request for a pre -construction conference form, and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre -construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
