The mitigation plan is hereby approved assuming no additional encroachments into the stream buffer are necessary. Please let me know if you have any questions regarding the review. Thanks, Phil 1 ,ekof COUNTY OF ALBEMARLE Department of Community Development 401 McIntire Road Charlottesville,Virginia 22902-4596 Phone(434)296-5832 Fax(434)972-4126 Project: Miller School New Entrance Road(WPO-2012-00049) Plan preparer: Mr.Brian Smith,PE Owner or rep.: The Miller School of Albemarle Plan received date: 25 May 2012 Date of comments: 22 June 2012 Reviewer: Phil Custer The engineering review of the ESC plan for the new entrance drive at the Miller School, received on 25 May 2012,has been completed. The plans can be approved after the following comments are addressed. 1. Please show and label all areas of pavement and gravel demolition(1-4). The letter from July 7th,2010 referred to by the applicant on page C9.0 could not be located in the county files. This work will be bonded as part of the Stormwater Management/Mitigation Bond and is, therefore,a critical part of the plan set. 2. Please provide the approval letter from the Army Corps of Engineers for the proposed disturbance to the streams. 3. The design of Trap 1,being undersized for a period of time towards the end of construction, is acceptable with a few modifications to the plan. On sheet C7.0,please extend the diversion north of Trap 2 to the ditch, label this diversion as a RWD,and provide a note referencing this diversion that states"This diversion must remain in place directing runoff to Trap 2 until the contractor is ready to apply asphalt paving to the travelway." It may be better to replace this diversion with a temporary culvert so vehicles may more easily pass it. Please also provide a blowup detail of trap 1 (without the Phase 1 Construction Entrance),preferably at 10 scale, within the set. In the detail, please clarify how a wet elevation of 612 can be provided with the rim invert of the junction box at 613.50. 4. The bottom dimensions for Traps 2 and 3 are mislabeled on Sheet C7.2. 5. Please provide a note on Sheet C7.0 near the construction entrances that states that the grading for the travelway from Sta. 11+50 to Sta. 17+00 cannot begin until the culvert is installed and sediment trap 1 is graded. 6. Please use the latest edition of the General Construction Notes from Erosion and Sediment Control Plans, available in the Design Manual online. 7. To receive a bond amount, please provide a completed Bond Estimate Request Form to the County Engineer after all comments have been addressed.
