Part C: Description of Proposal
Describe your proposal. Attach a separate sheet if more space is needed.
Pavilion IV is the second of three buildings to be built as part of the site plan amendment. Pavilion IV varies
slightly from the original site plan amendment ARB submittal. The large central gable remains, but the porches
and windows are now the same as Pavilion II, and the entry element is wider and taller. All materials and finishes
will match Pavilion II.
Part D: Applicant Agreement
Applicant must read and sign:
• Only complete application packages will be scheduled for ARB review. The application package is not complete
without the appropriate checklist - completed, signed, and included will# the required submittal materials indicated
on the checklist ABB checklists are available at 1_vrvw.albemarle.oM at the Community Development Forms
Library.
• Digital submittals are encouraged, but paper submittals are accepted.
• Paver submittals: Each application package must contain 1 folded copy of all plans and documents being submitted,
and 1 set of building material samples (if indicated on the corresponding submittal checklist.) All submittal items,
including building material samples, become the property of Albemarle County. Applicants are encouraged to
maintain duplicate copies of all submittal items in their own files. A pdf copy of all submittal items is requested.
• Digital submittals: One set of building material samples may be required. See the corresponding submittal checklist
for requirements. All submittal items, including building material samples, become the property of Albemarle County.

Note: Submittal packages must contain 2 collated and folded copies of all information unless otherwise stated.
A. Written description of the proposal

2 GZ Explain how the proposal is compatible with the surrounding area and the Entrance Corridor.
B. For Minor Site Plan Amendments, provide a plan showing the approved site condition and the
following (drawn to the scale of f "=20, clearly legible):
1 ❑ Location and identification of all proposed changes, including new site features and features to be
demolished such as but not limited to:
a ❑ parking, travelways, walkways
b ❑ mechanical equipment, trash containers, loading and service areas, and associated
screening
c ❑ proposed grading changes drawn with contour intervals of 5-feet or less
d ❑ proposed utilities and easements or changes to utilities and easements, identify easement
type and extent
e ❑ changes to stormwater management facilities
f ❑ retaining walls with top and bottom elevations, maximum wall heights, proposed materials,
and material and color samples
g ❑ changes to site lighting (see the lighting checklist)
h ❑ existing landscaping and changes to landscaping, including landscaping to be removed
with location, size and species; proposed landscaping with a landscape schedule
indicating plant species with common and botanical names, quantity, size at planting, and
symbol used; location and height of above -ground utilities and associated easements, and
location of below -ground utilities and associated easements; tree protection fencing and
limits of work on the landscape, grading and E&S plans.
C. For Minor Amendments to Architectural Plans, provide the following:
1 Dimensioned architectural elevations of the existing condition and the proposed changes.
Elevations must be drawn to the scale of 1/8"=V-0". Include a building materials schedule
