Purge Unnecessary Files: folder to contain either digital, or hardcopies...not both. Digital files
should all be in CV Documents under that WPO with the exception of pipe, and other videos
which may be on flash drives and kept in the folder.

As-Built drawings:
• Signed &dated professional seal of the engineer/surveyor, and the name &address of
the firm on the cover page;
• Construction/inspection records by operator to verify dimensions, mtls, & installation of
the facility including:
Inspection records/photos for pipe trenches, bedding, underdrains;
Video of any pipes that are not accessible/viewable;
Updated location of all facilities using geo coordinates;
Verified drainage area maps for area treated, and areas to facilities;
Current surveys of all earthen structures. Ponds shall include a survey to
verify pond volumes are pursuant to design;
Plantings: location and type;
Plans& profiles for all culverts, pipes, risers, weirs and drainage structures
including type of structure, culvert/pipe,weir, inlet&outlet end
sections, inlet&outlet protection, alignment& invert elevations
compared to the plan;
Computations for significant deviations from the plan. Computations are
required to verify as-built conditions meet, or exceed design criteria;
Ditch Lines and Channels shall be displayed as constructed and shall include
typical sections and linings;
Easements of any platted easements with deed book and page provided.
Copies of recorded documents are required;
Guardrail,fences&other safety provisions constructed shall be identified on
the plan with type, length and end treatments compared to the plan;
Materials: provide bioretention/filter media type and depth (bio media to be
state-approved mix), stone layers, sand layers, keyways and cores must
be verified as to type and depth. Inspection reports, boring/test pit
reports, and material certifications;
Access Roads: show location, surface treatment, drainage, etc;
Compaction Reports are required to verify fill compaction in dams;
Manufacturer's Certifications for proprietary BMPs certifying installation and
functionality pursuant to the plan

• Verify fields have been filled out accurately;
• If site utilizes SWM Facility from larger CPD, but is not a "regional" facility, note quality
is satisfied by the VAR10#of that CPD permit, and place under"comments" on the
N.O.T.
- Erosion Erosion Control,or SWM Performance Bond Release:
For BIESC the inspector verifies project completion, updates CV review status to
"approved/complete".
For BISWM the inspector verifies project completion and obtains county engineer's approval
before updating CV review status.
Inspector completes the bond release form and attaches to file,then places on manager's desk
to process. It is the inspector's responsibility to verify all required documents included
in this checklist are either in hard copy form and in the folder, or in CV under the WPO#.
Manager reviews folder for accuracy, scans the release form and forwards to Ana &cA-e/J}-
EAria notifying them that the WPO qualifies for bond release and to process.
.- Inspection Reports: A minimum of 2 county inspection reports shall be included. Reports may either
be digital and found in CV, or may be hard copy form. One of the reports must be the final bond
release inspection report verifying completeness.
Outstanding Fees: Applicant is responsible for any/all outstanding fees prior to bond release. Fees
shall include re-inspection fees, annual permit maintenance fees, etc. Applicant shall be notified
by inspector prior to performing final inspection that there "may" be outstanding fees, but bond
shall not be released until all fees have been paid.
