RE: Verizon — Moores Creek (Canody Property) — Building Permit after Tier 11 or
Tier II Approval with Special Exception Request
To Whom It May Concern,
The application and information enclosed with this cover letter are being submitted
in support of Verizon's request for approval to modify Section 5.1.40.b(2)(c) —
Projection and Section 5.1.40.b(2)(b) in order to allow Verizon's proposed antennas
to be install in a modified flush mount array and the size of one (1) of the new
proposed antenna. The initial Tier II tower was approved by the Planning
Commission on December 22Id, 2003 (SP200300066).
In accordance with requirements set forth in the County of Albemarle's Zoning
Ordinance, two (2) copies of the following information is being submitted for the
above mentioned project:
1. Personal Wireless Service Facility Application.
2. Special Exception Request Letter — Antenna Standoff.
3. Special Exception Request Letter — Proposed Antenna Size.
4. Structural Analysis dated 3/10/22, verifying the structural viability of the
existing tower regarding the proposed equipment modification.
5. Construction Drawings dated .
6. Equipment Specification Documents.
7. Recorded Memorandum of Lease and Survey of Property.
8. Previous approval history documents.
