Good morning Kris,
It appears that all outstanding comments have been addressed for SUB2019-00027 and SUB2019-00028; however,for
SUB2019-00026,there is a frame shed shown on the BLA plat that was recorded earlier this year that is not shown on
this subdivision plat. Show and label this shed on SUB2019-00026. You can provide me with a PDF version when that
revision is made, rather than a hard copy,for me to review.
Once that revision has been adequately addressed,the subdivision plats and private street maintenance agreements can
be signed and notarized, and submitted to CDD for signature by the Agent of the Board of Supervisors.

RE: SUB201900028 TMP 56-31 —Final Plat(Two-Lot Subdivision)
Dear Mr. Kirtley:
The County of Albemarle has received an application for a Two-Lot Subdivision involving Tax Map Parcel 56-31,
with proposed new lots 1 and 2.This project requires Health Department approval before receiving final County
approval.
The applicant has provided the following for Health Department review:
• One(1)final subdivision plat(dated 01/28/2019);
• One(1)VDH Application for Subdivision Review(dated 02/02/2019); and
• Two(2)OSE/PE Reports for Subdivision Approval(dated 03/02/2019).
Please review the proposals for suitable subsurface drainfields which comply with the provisions of Chapter 18,
Sections 4.2.2,4.2.3,4.2.4, and Chapter 14, Sections 14-309, 14-310,and 14-416 of the Albemarle County Code.
Your review comments on the subdivision plat(and corresponding soils reports) are requested by email and/or in
County View by March 29,2019. Should you have any questions,requests for assistance,or topics of discussion,

Additional description of subdivision
Overview of soils and geology(optional but encouraged)
In order for VDH to process a subdivision application you must attach a plat of the property showing the location of the proposed onsite
sewage disposal systems and the reserve absorption areas(if required)and the location of the water supply system on each lot, if applicable.
Each plat or subsection of a subdivision shall be accompanied by specific soil information for each lot(absorption area and reserve area). If
not provided by the local subdivision ordinance,the district or local health department may require the plat to show streets,utilities,storm
drainage,water supplies,easements,lot lines,and original topographic contour lines by detail survey or other information as required.
When the AOSE site evaluations are reviewed,the property lines,building location and the proposed well and sewage system sites must be
clearly marked and the property sufficiently visible to see the topography,otherwise this application will be denied.
I give permission to the Virginia Department of Health(VDH)to enter onto the property described during normal business hours for the
purpose of processing this application and to perform quality assurance checks of evaluations and designs certified by an Authorized Onsite
Soil Evaluator(AOSE)or a Professional Engineer(PE)as necessary until the sewage disposal system has been constructed and approved.
