Please find attached a pdf copy of the revised plat for Laura G. Farrell-Rural Division & BLA.
The plat has been revised to reflect your comments dated August 2, 2019 as follows:
1. The approximate location of the 20' Drainage Easement between Parcel W and State Route 1248
(Mechums West Drive) is an existing easement taken from the plat recorded in D.B. 942-380 thru 383.
We did not create the easement. We do not know if it is public or private.
2. The building site note has been revised to include Revised T.M. 57-53E.
3. The setback requirements have been added to the land use notes.
4. The legal references for T.M. 57-52B on sheet 3 have been revised to include the recordation of the BLA
plat that was approved in June. •
5. A revised date has been added to each sheet of the plat.
Once you have had a chance to review the revised plat, please let us know if it is ready for owners notarized
signature and county signature.

accordance with.the.provisions of the Cod_a of Virginia;the Sewage Handling filing and Disposal Regulations,,

This request for"subdivision review was subniitted pursuant to the provisions of§32.1.-163.5 of the Code of
Virginia which requires the Health Department to accept private soil.evaluations and designs from an
Authorized Onsite Soil Evaluator(AOSE)or a Professional Engineer working in consultation with an AOSE for
residential development. This subdivision was certified as'being in compliance with the Board of Health's
Regulations by Steve Gooch;.Onsite Soil Evaluator Number 1940001284. This subdivision approval is

Pursuant to§ 360.of the Regulations this approval is not an assurance that Sewage Disposal System

unless all conditions and circumstances arc present at the time of application for a permit as arc present at the

The County of Albemarle has received an application for a Rural Subdivision and Boundary Line Adjustment
involving Tax Map Parcels 57-52B and 57-53E,with a proposed new"Lot 1"from the residue of TMP 57-52B.This
project requires Health Department approval before receiving final County approval.
The applicant has provided the following for Health Department review:

In order for VDH to process a subdivision application you must attach a plat of the property showing the location of the proposed onsite
sewage disposal systems and the reserve absorption areas(if required)and the location of the water supply system on each lot,if applicable.
Each plat or subsection of a subdivision shall be accompanied by specific soil information for each lot(absorption area and reserve area). If
not provided by the local subdivision ordinance,the district or local health department may require the plat to show streets,utilities,storm
drainage,water supplies,easements,lot lines,and original topographic contour lines by detail survey or other information as required.
When the AOSE site evaluations are reviewed,the property lines,building location and the proposed well and sewage system sites must be
clearly marked and the property sufficiently visible to see the topography,otherwise this application will be denied.
I give permission to the Virginia Department of Health(VDH)to enter onto the property described during normal business hours for the
purpose of processing this application and to perform quality assurance checks of evaluations and designs certified by an Authorized Onsite
Soil Evaluator(AOSE)or a Professional Engineer(PE)as necessary until the sewage disposal system has been constructed and approved.
