We received your WPO bond inspection request for WP0201800051. The bond cannot be released at
this time. Several items are missing from the county requirements listed here:

The following items must be completed before the bond can be released:
1) As part of the as -built drawings:
a. Provide verified drainage maps.
b. Show all easements with DB&PG.
c. Verify pond volumes are per design. Show bottom of ponds.
d. Verify all inverts, this includes the isolator row.
e. Based on changes to approved design, provide any updated calculations based on as -built
conditions.
2) Provide all applicable compaction reports.
3) Provide manufacturer's certification for propriety facilities such as the isolator row and the CDS
unit.
4) As part of the site visit:
a. Pond 4A:
i. Provide pipe video.
ii. Stabilize minor erosion near inlet grate.
b. Pond 4B
i. Micro -pool and aquatic bench not evident.
ii. Fix the channel for the outlet. It is eroding and sluffing off from 29.
iii. Please clean up material being stored in the facility area.
iv. Ensure it is vegetated per specifications.
c. Block retaining wall
i. There is a u shaped retaining wall with no apparent purpose. The wall is creating
unmanageable slopes and erosion.

5) Engineer's certification statement, as required by DEQ on the NOT.
6) On the NOT:
a. Please uncheck as -built - full-size paper as you provided them digitally
b. Please provide evidence that Adam Swartout is signatory authority for Brookhill
Apartments, LLC, who is the operator.
c. We will verify areas treated when the drainage area maps are provided on the as -built.
