Engineering has reviewed the easement plat and offers the following comments. Additional comments
may be forthcoming from other reviewers.
1. Sheet 1: There is a typo, "ownwer" versus "owner."
2. Sheet 1: Restate "watersupply" as "water supply watershed" in the 15' note, bottom right of the
sheet [14-302.B(7)].
3. Sheet 1: Correct the floodplain note. This property is located within Zones X and AE. Show the
flood hazard overlay district boundaries and the flood elevation based on the attached LOMR
[item 9 on checklist and 14-302.B(9)]. Add to the floodplain note the County application number
[FDP202100005] and the FEMA LOMR-FW Case No. 21-03-1595A.
4. Sheet 2: Show the stream buffer [item 10 on the checklist and 14-302.B(10)].
5. Sheet 2: Include the stormwater discharge pipe (STR 3 to headwall) in the SWM facility & access
easement. Also, the easement should extend 10-ft easement on each side of the pipe (20-ft total),
from the facility to 10-ft beyond the headwall, including through the parking lot. The easement
should be centered on the pipe, but may be adjusted slightly off -center as needed so that it
remains entirely on the subject property and is 20-ft for the entire length [14-431.A].

This is in response to your request for a Letter of Map Amendment for the property referenced above
The Federal Emergency Management Agency (FEMA) uses detailed application/certification forms
for revision requests or amendments to the National Flood Insurance Program (NFIP) maps. The
forms provide step-by-step instructions for requestors to follow, and are comprehensive, ensuring that
the requestors' submissions are complete and more logically structured. Therefore, we can complete
our review more quickly and at lower cost to the NFIP. While completing the forms may seem
burdensome, the advantages to requestors outweigh any inconvenience.
The following forms or supporting data, which were omitted from your previous submittal, must be
provided:
Requests for a portion of property must not intersect or coincide with the footprint of any
existing or intended structures. The request should be submitted with the intent to obtain a
determination for those buildable portions of the property where structures currently or may
exist as opposed to attempting to revise or eliminate the existing Special Flood Hazard Area
(SFHA) as it is specifically shown on the effective Flood Insurance Rate Map (FIRM).
Because the MT-1 process for which you have applied will not physically revise or redraw the
SFHA depicted on the FIRM, the areas should instead be drawn with the intent to obtain a
determination stating whether or not any structures built within the described area would carry
the mandatory flood insurance requirement. Please review the attached excerpt from the MT-1
Technical Guidance document.
Please note that if all of the required items are not submitted within 90 days from the date of the first
Request for Additional Data letter that was sent on 5/20/2021, any subsequent request will be treated
as an original submittal and will be subject to all submittal procedures.
When you write to us concerning your request, please include the case number referenced above in your
letter. All required items for your request are to be either uploaded through the Online LOMC tool, for
requests initiated online, or mailed to the Engineering Library, 3601 Eisenhower Ave Ste 500, Alexandria,
VA 22304-6426, for requests initiated through the mail.
