Dear Mr. Woodward:
This letter, in conjunction with the approved plans and specifications (13 pages) dated June 19, 2013 ,
which are attached, constitutes your permit to install a Sewage Disposal System to serve an existing
church that produces no more than 300 gallons of sewage effluent per day.
The application for a permit was submitted pursuant to §32.1-163.5 of the Code of Virginia which
requires the Health Department to accept private soil evaluations and designs from an Onsite Soil
Evaluator (OSE) or a Professional Engineer working in consultation with an OSE for residential
development. The permitted site was certified as being in compliance with the Board of Health's Sewage
Handling and Disposal Regulations 12 VAC 5-610-20 et seq (and Regulations for Alternative Onsite
Sewage Systems 12 VAC 5-613-20 et seq if applicable)(and local ordinances if the locality has
authorized the local health department to accept private evaluations for compliance with local
ordinances)by the following named consultant. This letter is issued in reliance upon that certification.

The Board of Health hereby recognizes that the soil and site conditions acknowledged by this
correspondence, and documented by additional records on file at the local health department, are suitable
for the installation of onsite sewage disposal systems. The attached plat (or plats) shows the approved
areas for the sewage disposal systems. This letter is void if there is any substantial physical change in the
soil or site conditions where a sewage disposal system is to be located.
If modifications or revisions are necessary between now and when you construct your dwelling, please
contact the Onsite Soil Evaluator (OSE)or Professional Engineer(PE)who performed the evaluation and
design on which this permit is based. The name, address, and phone number of the OSE/PE appears on
the certification form attached to this permit. Should revisions be necessary during construction, your
contractor should consult with the OSE/PE that submitted the site evaluation or site evaluation and
design. The OSE or PE is authorized to make minor adjustments in the location or design of the system
at the time of construction provided adequate documentation is provided to the Albemarle County Health
Department.
This authorization is null and void if conditions are changed from those shown on the application or
conditions are changed from those shown on the attached construction drawings, plans and
specifications. No part of any installation shall be covered or used until inspected, corrections made if
necessary, and approved by the Albemarle County Health Department or unless expressly authorized by
the Albemarle County Health Department. Any part of any installation which has been covered prior to
approval shall be uncovered if necessary, upon the direction of the Department.
This Permit has been issued in accordance with applicable regulations based on the information and
materials provided at the time of application. There may be other local, state, or federal laws or
regulations that apply to the proposed construction of this onsite sewage system. The owner is
responsible at all times for complying with all applicable local, state, and federal laws and regulations.
If you have any questions, please contact me. This authorization to construct a sewage disposal system
expires on January 07,2015.
This Permit is NOT TRANSFERABLE to any other person or location.

THIS PERMIT ISSUED UNDER THE 2000 SEWAGE HANDLING AND DISPOSAL REGULATIONS
*Call MISS UTILITY at least 48 hrs. Before Proceeding with any Excavation On The Property*
Install 4 trenches,70 feet long,2 feet wide on contour,6 foot centers,with a maximum trench depth of 40 inches.Begin trenches
starting at point designated as distribution box on site sketch. Drain field area has been delineated and shown to scale on site
sketch. Have D.F.re-located if reference points on property do not allow for accurate relocation before installation.
Maintain the following separation distances.
1.Septic Tank/Treatment Chamber/Drainfield to building foundation—ten(10)foot minimum with crawl,twenty(20)foot min.with basement
2.Septic Tank/Treatment Chamber/Drainfield to property lines—five(5)foot minimum.
3.Septic Tank/Treatment Chamber/Drainfield to Class IIIC well—one hundred(100)foot minimum.
4.Septic Tank/Treatment Chamber/Drainfield to Class IIIB well-fifty(50)feet.
5.Septic Tank/Treatment Chamber/Drainfield to utilities—ten(10)foot minimum.
1. Protect the drainfield area during any land disturbance and/or building construction.Heavy equipment and trucks,especially,
will destroy the soil structure enough to make the sewage construction permit void.!Particularly during wet times of the year,be
aware of soil moisture conditions at all time when working on site. Do not allow vehicles to drive,park or store heavy construction
material within the designated drainfield area!
2. All trees should be removed from the drainfield area.Hydrophilic trees such as maples,cottonwoods,and willows,
must be removed from within 10 feet of the drainfield.An even greater distance is recommended.Tree roots interfere with the proper
functioning of the drainfield and may lead to early failure of the system. Tree stumps,if removed,must be done with a stump grinder,
not by bull-dozing or digging.
3. Pre-grading of the drainfield arealsite should only be done with the specific consent and guidance of the Health Department or
AOSE.The system is designed to be installed at a specific depth from the original ground surface.Removing the topsoil or
changing the contours of the site without permission may make the drainfield area unusable. This may void the permit!
This sewage disposal system is to be constructed as specified by the permit or attached plans and specifications:
This sewage disposal system permit is null and void if(a)conditions are changed from those shown on the application(b)conditions are
changed from those shown on the construction permit.
No part of any installation shall be covered or used until inspected,corrections made if necessary,and approved,by the designing AOSE
and local health department or unless expressly authorized by the health department.Any part or installation which has been covered prior

CONSTRUCTION NOTES&DETAILS:
Design Specifications:Install all products to manufacture specifications.Contact OSE/Engineer to schedule a pre-
Construction conference before beginning installation of the system.
1. Inspect existing 1250 gal.septic tank to determine if it is water tight and structurally sound.Replace with
equivalent top seam septic tank if necessary.Inspect existing inlet and outlet tees proper installation and function.
2. Install(1)1250 gallon top seam concrete tank to be used as the pump/dosing tank(top seam concrete tank required),
seal secondary access and outside top seam with hydraulic cement,Pump tank must be watertight.Tank location may
be adjusted to maintain required fall in building sewer from existing tank to pumpldosing tank,maintaining
required set-backs and proper landscape position..Hydraulically seal/cement around all inlets and outlets to the tank.
3. Install PF2005,20 GPM,115 V,1/2 HP,Orenco HH Effluent Pump or equivalent.
Install simplex control panel capable of time dosing and acting as the master disconnect within sight of pump tank.
3. Time dose panel to operate at 5 minutes-ON/240 Minutes-OFF
4. Install 2"sch.40 PVC conveyance line from pump tank to D.Box with 90 degree turn down into Splitter Box.

INTO THE SEPTIC TANK'*(This could cause excessive hydraulic overload of the drainfield area and septic system of
which the system is not designed.)*
11. Concrete tanks are to be installed on uniformly firm and stable compacted soil or undisturbed soil.Number 57 stone is
recommended to provide uniform support to tank bottom and at least 6"of number 57 stone is required if rock is
encountered in bottom of tank hole,graded and level before tanks can be set.
12. Septic tank and/or pump tank shall be water tight(Top Seam)and conform to applicable local and state Sewage Handling
and Disposal Regulations.
System should be installed only in dry conditions.Call designer if there is any question in this regard.
13. Backfill with suitable loose material free of large or damaging objects.Backfill and compact soil around tanks in 6
to 8 inch lifts to reduce settling.Ensure that final grade provides positive drainage over and around the tank
area and drainfield area and is adequate to prevent any infiltration of surface water.Divert any storm water run-
off from up-slope and/or any guttering from structures,away from the treatment system and away from the septic
tanks.Installer is responsible for proper compaction around tanks and any settling that may take place after
installation for up one year.Installer will be required to correct any settling that may occur in order to maintain
proper positive drainage away from tanks and drainfield.
14. Installer is responsible for D.F.area and tank area to be seeded,straw mulched,and fertilized immediately
after backfill and final grading.(Homeowner is responsible for proper watering of new grass and maintaining
of proper cover around tanks and drainfield area).
15. Call designing AOSE @ number listed below at least 48 hours before beginning any excavation begins to
arrange pre-construction meeting if there are questions about the exact drainfield location.If the drainfield
location is not still marked/staked or cannot be located according to the platted survey from known property
corners,DO NOT BEGIN INSTALLATION OF ANY PART OF THE SYSTEM and CALL THE DESIGNING AOSE
IMMEDIATELY.(FAILURE TO CONTRACT THE DESIGNING AOSE OR HEALTH DEPT.IN THIS
CIRCUMSTAINCE COULD RESULT IN REJECTION OF THE PERMIT AND DENIAL OF THE DRAINFIELD AREA
