FLOODPLAIN DEVELOPMENT PERMIT
This form is intended to aid the Department in documenting and characterizing the nature of development proposed in the
floodplain. Development includes buildings or other structures, mining, dredging, filling, grading, paving, excavation or
drilling operations or storage of equipment or materials. The form responses will be used to determine whether a
floodplain impact plan or special use permit is required. Fees are listed in County Code §18-35.I f.9 & 10.

This Floodplain Development Permit application is in reference to WP0202200037.
The proposed Innisfree Mitigation Bank - Phase 2 project includes the restoration, enhancement, and preservation of
stream reaches, as well as the associated buffer plantings and preservation areas. The bank includes unnamed tributaries
to Doyles Creek. Phase 2 includes approximately 66,666 linear feet of stream channel.
