[14-203] This application and review should be for a two lot subdivision, not a rural subdivision. The applicant paid $817.00 in fees for a rural subdivision whereas the fee for a two lot subdivision is $639.00. A total of $178.00 will be refunded to the credit card on file. Please contact staff via email to initiate refund. 2. [14-301(b)] On Sheet 2, please add "1 "=50"' to the scale. 3. [14-302 (a)(1)] On Sheet 1, please revise the title to "Athayde Two -Lot Subdivision Plat". 4. [14-302 (a)(11)] Instrument creating property proposed for subdivision. Please provide the deed book and page number of the instrument whereby the property was created, as recorded in the office of the clerk of the circuit court of the county. 5. [14-302 (a)(15)] Identification of all owners and certain interest holders. For the labelled Lot A, Lot B, and Lot D: please provide the name of the owner(s) as well as deed book and page number information. Please also label and provide the same information for TMP 48-18D. 6. [14-302 (b)(1)] Please add a date of last revision once the edits are completed. 7. [14-302 (b)(5)] Please add a note to indicate that the two parcels are in the entrance corridor overlay. 8. [General Comment] Please Note 10. This does not apply to this site. 9. [14-303 (a)] On Sheet 1 under "Owner's Approval", please revise "Rural Subdivision Plat" to "Athayde Two -Lot Subdivision Plat". 10. [14-308 (1)] Ground water assessment information. Results of the Tier II Groundwater Assessment are attached. 11. [14-309] Soils evaluations. The subdivider shall submit to the agent with each final plat the results of percolation tests or other methods of soils evaluation used to determine the suitability of the soil for subsurface drainfields, if conventional onsite sewage systems are proposed to be used in the development of the subdivision, and the results are requested by the agent. These results shall be forwarded by the agent to the health director. 12. [14-310] Health director approval of individual private wells and/or onsite sewage systems is required. Please provide the location of all existing buildings on the land to be subdivided within fifty feet of the proposed lot line. 14. [14-316] Approval of entrance onto public streets. VDOT review and approval is required. Comments and approvals will be forwarded to the applicant upon receipt by planning staff. 15. [14-403] Lot frontage. The frontage requirement for the rural area is 250 feet for public roads. Neither Lot 1 (approximately 197 ft) nor Lot 2 (approximately 122 ft) meet this frontage requirement. Both lots need to be widened so they meet the 250 feet along SR 20 or a new internal private street needs to be created to give both lots 250 feet of frontage. Virginia Department of Health - Travis Davis, travis.davis@vdh.virginia.gov - Review will begin once a soils report and location of individual private wells and/or onsite sewage systems are submitted.
