Pollution Prevention and Spill Prevention Control Plan, Our Lady of the Angels Monastery, Minor
Site Plan Amendment, August 18, 2014, requires revision:
a. In addition to Concrete Wash -out Area, Fueling Area, Stockpile Area, and Porta -potty
labels, furnish specific prevention design information, including: secondary containment
for fueling area; concrete washout area located upstream of a sediment basin/trap; silt
fence/berm for portable sanitary facilities to prevent stream or environmental impact in
event of spill. Labels alone are not pollution/stormwater prevention design measures.

1. Prepare sheets titled Stormwater Management Plan that include requested /revised items listed below. Note:
submission received 20 -Aug includes: VaRRM spreadsheets (.xls); WPO plan, Jul -11 2014 (4 sheets: ESC
sheets, ESC notes, detail for roof drain infiltration system [ref. #8]); letter request for variation of entrance

max. gradient of 16 %; 8.5" x 11" color map of proposed development; comment response letter
(SDP201400035); Pollution Prevention and Spill Prevention Control Plan; PPP & SWPPP document with
original signature. This is relevant, useful information. In addition, collect items below on SWM sheets.

2. VaRRM requires revision. VSMP Application checklist, 7 Aug 2014, notes that the "acreage around the
building is managed turf, and cannot be considered preserved forest/open space in the computation." While
proper to use the re- development worksheet, it is inaccurate to list 0.00 Ac. forest /open space cover as the
pre - (re)developed condition, and to then list 1.62 Ac. forest /open space as the post - (re)developed condition.
Using these values, VaRRM .xls reports that total P load reduction required is 0.04 lb /yr, while phosphorus
reduction achieved via sheet flow to open space and roof drain infiltration pits is 0.95 lb /yr, or twenty times
the required level. Please consider if .xls input values appear reasonable. Refer to DEQ guidance on pre -
/post- (re)developed conditions. We are free to discuss the VaRMM, which may limit misunderstanding.
Since .xls values dictate design, we cannot evaluate whether proposed infiltration pits or sheet runoff to
open space will meet VaRRM total (P) load reduction requirements. (Rev. 1) Comment addressed.
3. VaRRM .xls printouts are unreadable for the most part. With revision ( #2, above), please ensure data is
readable (10 pt. +). Also, there is no need to include sheets for drainages B, C, D, and E, since the project
is a single drainage area. VaRRM printouts for drainage area A along with summary land cover and water
compliance (first -last) .xls sheets will likely meet review needs. (Rev. 1) Comment addressed.
4. VaRRM includes 0.12 and 0.11 Ac. impervious area runoff routed to infiltration and open space,
respectively. In plan view, please show /map how runoff is routed to each practice. Refine color map to
clearly indicate which areas measure 8,365sf, 4,986sf, 14,415sf, and 76,609sf, if any is relevant to SWM,
VaRRM, .xls, or compliance. (Rev. 1) Comment withdrawn.

Please furnish these items. Delineate and show open space preserved in covenant. Refer to design
guidance which is quite specific, and includes structural elements —level spreaders, for example.

Process:
After approval, plans will need to be bonded. The bonding process is begun by submitting a bond estimate request
form and fee to the Department of Community Development. One of the plan reviewers will prepare estimates and
check parcel and easement information based on the approved plans. The County's Management Analyst will
prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash,
certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County
Attorney and County Engineer. This may take 2 -4 weeks to obtain all the correct signatures and forms.
Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The
County's Management Analyst or other staff will prepare the forms and check for ownership and signature
information. The completed forms will need to be submitted along with court recording fees.
After bonding and agreements are complete, county staff will need to enter project information in a DEQ database
for state application processing. DEQ will review the application information based on local VSMP authority
approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest
processing, this is done electronically with the emails provided on the application. DEQ should notify applicants
with instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter.
This should be copied to the county.
After DEQ coverage is issued, via the coverage letter, the County can hold a pre - construction conference. Applicants
will need to complete the request for a pre - construction conference form, and pay the remainder of the application
fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be
checked by county staff, and upon approval, a pre - construction conference will be scheduled with the County
inspector. At the pre - construction conference, should everything proceed satisfactorily, a joint VSMP and grading
permit will be issued by the County so that work may begin.
