This project is denied/approved. The rationale is given in the comments below. The VSMP application content requirements can be found in County Code section 17-401. A. Stormwater Pollution Prevention Plan (SWPPP) The SWPPP content requirements can be found in County Code section 17-405. A SWPPP must contain (1) a PPP, (2) an ESCP, (3) a SWMP, and (4) any TMDL measures necessary. 1. Sign Registration Statement. 2. Sign Sign Certification. 3. Complete other sections prior to pre -con. 4. Note: Engineering will attach the stamped plan to SWPPP. B. Pollution Prevention Plan (PPP) The PPP content requirements can be found in County Code section 17-404. 1. Provide the vehicle washing at construction entrance. 2. Provide a concrete washout detail. C. Stormwater Management Plan (SWMP) VSMP Regulation 9VAC25-870-108 requires the VSMP authority to approve or disapprove a SWMP. This plan is denied, and the reasons are provided in the comments below. The stormwater management plan content requirements can be found in County Code section 17- 403. 1. C1 & C4 - Please include'SWM Narrative' in title block and for the sheet index. 2. 1 would prefer that information on the type and location of stormwater discharges, predevelopment and postdevelopment drainage areas be shown on the plans rather than separately in calc. packet. 3. Provide % land covers on your drainage area pre -/post- dev. maps. 4. C4 - SWM Narrative summary: The plan should refer to the SWM Calculations Packet (i.e., see stamped sealed Calculations Packet submitted separately). 5. Calcs - Write/Solve the E.B. equation and show where values/variables are found. 9VAC25-870-66 B.3 6. Stamp seal the Calculations Packet. Engineering Review Comments Page 2 of 4 7. C3 -Show or callout the e.spillway. In the calcs for the pond report it calls for 10-ft e.spillway (x 2.00). What does x 2.00 mean & profile 4, may need to show? 8. Show the design WSE's for the pond in the profile. Provide stage -storage for the pond up to 100-yr design. State 'vegetative' e.spillway or the lining provided. 9. Provide barrel seepage control collar design spacing. 10.Off-site options, such as, purchasing credits should be considered only when PR practice cannot be implemented to the maximum extent. [DEQ GM 22-2012] a. A Wet Pond or Ext. Detention would be appropriate pollutant removal (PR) practice as it provides nutrient load reductions and should be considered where practicable. b. Consider F/OS protection since the site is forested. 11. Show SWM facility and access easement for pond. 12. Easement plat will need to be submitted and approved prior to WPO plan approval. 13. Deed of dedication will need to be recorded with the easement plat prior to WPO plan approval. 14. SWM agreement will need to be approved prior to WPO plan approval. 15. Nutrient credit agreement & purchase of credits will need to be completed prior to grading permit issuance. 16. Include A.C. As -built (VSMP) checklist. D. Mitigation Plan (MP) The mitigation plan requirements can be found in County Code Chapter 17-406. 1. 2:1 ratio mitigation planting area has not been met in the stream buffer (0.64ac disturbed, 1.28ac mitigation required, 0.64ac provided). This is mainly due to the size of the pond constructed relative to the available buffer area within parcel. It may need to be demonstrated why a pond this size is necessary because a reduced size pond can be constructed to meet 2:1 ratio mitigation planting requirement. (17-604 C.5] a. It is also an ESD practice to minimize soil disturbance to conserve the soils most conductive to infiltration & to maximize/retain forest cover. [DEQ GM 22-2012, p.41] 2. C7 - Add note that stream bed and banks shall be stabilized within seven (7) days from start of backfilling for the culvert.117-604 C.4) 4. Provide necessary permit for the stream disturbance.117-604 C.6) E. Erosion and Sediment Control Plan (ESCP) Virginia Code §62.1-44.15:55 requires the VESCP authority to approve or disapprove an ESCP. This plan is disapproved, and the reasons are provided in the comments below. The erosion control plan content requirements can be found in County Code section 17-402. 1. Provide County's paved construction entrance detail (ACDSM, p.9), and show sediment trapping device at or near entrance. 2. Provide DS, VESCH Spec. 3.26 for the excavated pond. 3. Show tree protection (TP) fencing around disturbed perimeter. 4. Note: Inspector may require additional/alternate on -site control measure beyond what the approved plan shows. The VSMP permit application and all plans may be resubmitted for approval when all comments have been satisfactorily addressed. For re -submittals please provide 2 copies of the complete permit package with a completed application form. Engineering plan review staff are available from 2-4 PM on Thursdays, should you require a meeting to discuss this review. Process: After approval, erosion and sediment control and stormwater management improvements must be bonded. The bonding process begins when the plan is approved, and the applicant submits a bond estimate request form and fee to the Department of Community Development. A plan reviewer will prepare estimates and check parcel and easement information based on the approved plans. The County's Management Analyst will prepare bond agreement forms, which will need to be completed by the owner and submitted along with cash, certificates or sureties for the amounts specified. The agreements will need to be approved and signed by the County Attorney and County Engineer. This may take 2-4 weeks to obtain all the correct signatures and forms. Stormwater Management Facilities Maintenance agreements will also need to be completed and recorded. The County's Management Analyst or other staff will prepare the forms and check for ownership and signature information. The completed forms will need to be submitted to the County, who will record the agreement. After bonding and agreements are complete, county staff enters project information in a DEQ database for state application processing, if a DEQ permit is required. DEQ reviews the application information based on local VSMP authority approval. At this time, the DEQ portion of the application fees will need to be paid directly to the state. For fastest processing, this is done electronically with the emails provided on the application. DEQ should notify applicants with Engineering Review Comments Page 4 of 4 instructions on how to pay fees. When DEQ approves the application, they will issue a permit coverage letter, which must be posted at the construction site. After DEQ coverage is issued, via the coverage letter, the County can hold a pre -construction conference. Applicants must complete the request for a pre -construction conference form and pay the remainder of the application fee. The form identifies the contractor and responsible land disturber, and the fee remaining to be paid. This will be checked by county staff, and upon approval, a pre -construction conference will be scheduled with the County inspector. At the pre - construction conference, should everything proceed satisfactorily, a joint VSMP and grading permit will be issued by the County so that work may begin.
