As -built Plan — Provide drawing information: a. Stationing along road centerlines Road centerline stationing now provided on plans. b. Sheet 5, etc. missing road section beyond Sta. 22+00 to phase line (23+30) The phase line is at 22+00, not 23+30. c. Provide at least 20 ft wide road FOP per design or as shown in as -built typical section Minimum road width of 20-ft shown. d. Show remaining driveway and culvert locations Driveway & culverts shown. 2.) Provide engineer's letter certifying improvements are built to plan. Engineer's certification letter provided. 3.) Provide signed certification form by owner/developer Signed certification of completion form provided. 4.) Provide copy of recorded plats Copy of the recorded plats provided with this submission. 5.) Provide pipe trench reports Pipe trench certification letter by contractor provided. 6.) Provide additional compaction/soil stabilization reports (proof rolling). The Geo-Terrain report dated June 28, 2019 includes a statement that the entire section of Road A in Phase 2 (19+75 to 22+25) & the entire Road C were proof rolled along with the in place density tests for each road at subgrade and -1' from subgrade. 912 E. CBR test locations map is provided. 8.) Provide core test report & locations (station) of road pavement thickness (surface/base). The report didn't specify test locations (station). (2) minimum for each road. Pavement core reports are provided. 9.) Pipe Videos. For pipe runs (please re -video): a. A10-A9 — Clean out rubble in pipe. A10-A9 cleaned out. b. A9-A8 — Re -video. Cannot see bottom half of pipe. (underwater) Video provided with this submission. c. 136-135 — Please provide video. Video provided with this submission. d. E2-El — The pipe is 12-11 short per design (50-ft). There is still erosion happening from outlet end to sediment basin. Install additional length of pipe. Additional pipe length added. e. E3-E2 — Video shows pipe deformation that starts about 60-ft in and ends at about 140-ft. Recommend replacement. There is medium to large gap between pipe joint at 100-ft, 120- ft and 140-ft into video. Seal the joints at these locations. E3-E2 was replaced entirely. f. E5-E4 — Video shows deformation that sever and likely to fail. Pipe segment will need to be replaced. Deformation stars at about 5-11 in and ends at about 25-11. Recommend replacement of entire length. There is medium joint separation at 12.3-ft, and 51.8-ft into video. Seal the joints at those locations. Pipe deformation at about 71.5-ft. Recommend replacement. Video inspections have been reviewed by the manufacturer, ADS. Their engineer has provided a certification letter that all discrepancies are within their tolerance and no repairs are required. Please find attached the PE Certification from Advanced Drainage Systems. 10.) Field report / Inspections a. Parge lifting holes on inlet tops. Parging of the lifting holes has been completed. b. Install street trees. Street trees are installed. c. Install surface asphalt. Surface asphalt installed. d. Install remaining driveway culverts. Driveway culverts installed. e. Install road cross drains & EW-12s —not found No EW-12s were installed but all cross drains are run to manholes. Drill 2" holes for the address signs (on each face) Comment not relevant. g. Install end section at A8 — not found End section installed and survey shown on plans. h. Install pipe 136-135, end section and outfall protection— not found Pipe B6-B5 and end section are installed with outlet protection. Provide at least 20-ft wide road EOP to match the typical section of plan. Minimum 20-ft wide road shown on plans. j.
