This form covers the following permit actions: new permit issuance, existing permit modification with an increase in acreage, existing permit modifications that result in a plan modification but do not result in an increase in disturbed acreage, and reissuance of an active permit coverage. Application type. Select NEW PERMIT ISSUANCE to obtain a new permit coverage. Modifications are for modifying an existing, active permit coverage. Select MODIFICATION WITH ACREAGE INCREASE when the previously approved acreage(s) increases (permit modifications are not performed for decreases in acreage unless they result in plan changes — see Modification WITHOUT Acreage Increase). Section I. Operator/Permittee Information. A. Construction Activity Operator (Permittee). The person or entity that is applying for permit coverage and will have operational control over construction activities to ensure compliance with the general permit. For companies, use the complete, active, legal entity name as registered with a state corporation commission. Entities that are considered operators commonly consist of the property owner, developer of a project (the party with control of project plans and specifications), or general contractor (the party with day-to-day operational control of the activities at the project site that are necessary to ensure compliance with the general permit). If an individual person is listed as the operator, that person (or a legal representative of) must sign the certification in Section V. An operator may be one of the following: 9VAC25-870-10. Definitions "Operator" means the owner or operator of any facility or activity subject to the Act and this chapter. In the context of stormwater associated with a large or small construction activity, operator means any person associated with a construction project that meets either of the following two criteria: (i) the person has direct operational control over construction plans and specifications, including the ability to make modifications to those plans and specifications or Ili) the person has day-to-day operational control of those activities at a project that are necessary to ensure compliance with a storm water pollution prevention plan for the site or other state permit or V5MP authority permit conditions (i.e., they are authorized to direct workers at a site to carry out activities required by the stormwater pollution prevention plan or comply with other permit conditions). In the context of stormwater discharges from Municipal Separate Storm Sewer Systems (MS4s), operator means the operator of the regulated M54 system. "Owner" means the Commonwealth or any of its political subdivisions including, but not limited to, sanitation district commissions and authorities, and any public or private institution, corporation, association, firm or company organized or existing under the laws of this or any other state or country, or any officer or agency of the United States, or any person or group of persons acting individually or as a group that owns, operates, charters, rents, or otherwise exercises control over or is responsible for any actual or potential discharge of sewage, industrial wastes, or other wastes or pollutants to state waters, or any facility or operation that has the capability to alter the physical, chemical, or biological properties of state waters in contravention of § 62.1-44.5 of the Code of Virginia, the Act and this chapter. "Person " means any individual, corporation, partnership, association, state, municipality, commission, or political subdivision of a state, governmental body, including a federal, state, or local entity as applicable, any interstate body or any other legal entity. B. May we transmit correspondence electronically? If you choose YES to this question and provide an email address in Section I. A., all correspondence, forms, invoices and notifications will be transmitted by email to the operator. This will also give the operator the ability to pay by credit card and to receive Dermit coverage aooroval letters immediately upon Dermit aooroval. Section II. Construction Activity Information. A. A site map indicating the location of the existing or proposed land -disturbing activities, the limits of land disturbance, construction entrances and all water bodies receiving stormwater discharges from the site must be included with the submittal of this form. Aerial imagery maps or topographic maps showing the required items are acceptable. Plan sheet sized site maps are not required. Please consult your VSMP authority if you have additional questions regarding site map requirements. B. Construction Activity Name and location. Provide a descriptive project name (it is helpful to use the same naming convention as listed on the Stormwater Management plans), 911 street address (if available), city/county of the construction activity, and the 6-digit latitude and longitude in decimal degrees format for the centroid, main construction entrance or start and end points for linear projects (i.e. 37.1234N/-77.1234W). C. Acreage totals for all land -disturbing activities, on- and off -site, to be included under this permit. Acreages are to be reported to the nearest one -hundredth acre (two decimal places; i.e. 1.15 acres). Provide the total acreage of the primary development site as approved on the Stormwater Management Plans and the primary on -site estimated acreage to be disturbed by the construction activity as approved under the Erosion and Sediment Control Plans. The off -site estimated area to be disturbed is the sum of the disturbed acreages for all off -site support activities to be covered under this general permit. Do not include the off -site acreage totals in the primary, on -site total and estimated disturbed acreage totals. Permit fees are calculated based on your disturbed acreage total for all on- and off -site areas being disturbed under this permit coverage (the sum of all on -site and off -site disturbed acreages). D. Property owner status. The status of the construction activity property owner. Any property not owned by a government entity or agency (i.e. federal, state or local governments) is PRIVATE. Choose the designation that best describes the post -construction use of this project (you may choose more than one). (i.e. Residential, Commercial, Industrial, Agricultural, Environmental, Educational, Oil and Gas, Utility, Transportation, Institutional, etc.). Describe the post -construction use of the project (i.e. Commercial — one new office building and associated parking and utilities; Transportation — Linear roads, sidewalks and utilities; Agricultural —3 Poultry Houses, etc.). F. Municipal Separate Storm Sewer System (MS4) name(s) if discharging to a MS4. If stormwater is discharged through a MS4 (either partially or completely), provide the name of the MS4(s) that will be receiving water from this construction activity. The MS4 name is typically the town, city, county, institute or federal facility where the construction activity is located. G. Estimated project dates. Provide the estimated project start date and completion date in Month/Day/Year or MM/DD/YYYY format (i.e. 07/30/2019). H. Is this construction activity is part of a larger common plan of development or sale? "Common plan of development or sale" means a contiguous area where separate and distinct construction activities may be taking place at different times on different schedules per 9VAC25-870-50. Definitions. Le. a subdivision, commercial development, business park, etc. I. 6th Order Hydrologic Unit Code (HUC) and associated Receiving Water Name(s). Provide all 6th order HUCs and receiving waterbody names, for the primary site and any Off -site areas included under this permit coverage, that could potentially receive stormwater runoff discharging from this activity. The HUC can be either a 12-digit number (i.e. 0208010101) or 2-letter, 2-number code (i.e. JL52). Include additional HUCs or receiving waters on a separate page. Off -site Support Activity Location Information. This general permit also authorizes stormwater discharges from support activities (e.g., concrete or asphalt batch plants, equipment staging yards, material storage areas, excavated material disposal areas, borrow areas) located on -site or off -site provided that (i) the support activity is directly related to a construction activity that is required to have general permit coverage; (ii) the support activity is not a commercial operation, nor does it serve multiple unrelated construction activities by different operators; (III) the support activity does not operate beyond the completion of the construction activity it supports; (iv) the support activity is identified in the Registration Statement at the time of general permit coverage; (v) appropriate control measures are identified in a SW PPP and implemented to address the discharges from the support activity areas; and (vi) all applicable state, federal, and local approvals are obtained for the support activity. Off -site activity name and location information. Provide a descriptive off -site project name, 911 street address (if available), construction entrance location (address or decimal degrees coordinates and description), city/county and the 6-digit latitude and longitude in decimal degrees (i.e. 37.1234N, 77.1234W) of all off -site support activities. Indicate whether the off -site support activity will be covered under this general permit or a separate VPDES permit. If excavated material (i.e., fill) will be transported off -site for disposal, the name and physical location address, when available, of all off -site excavated material disposal areas including city or county; 6-digit latitude and longitude in decimal degrees (i.e. 37.1234N, 77.1234W) and the contents of the excavated material. List additional off -site areas to be included under this permit coverage on a separate page. Off -site areas not included on this registration will need to obtain coverage under a separate VPDES permit. Section IV. Other Information. A. A stormwater pollution prevention plan (SWPPP) must be prepared prior to submitting the Registration Statement per 9VAC25-880. See 9VAC25-880-70. Part II. of the General Permit for the SWPPP requirements. B. If the Erosion and Sediment Control Plan for the estimated area to be disturbed listed in Section II. C. has been submitted to the VESC Authority for review and plan approval, choose YES. If you are submitting this application to reissue an existing permit coverage, please provide the date that the VESC Authority approved the Erosion and Sediment Control Plan for the estimated area to be disturbed. C. If land disturbance has commenced, choose YES. "Land disturbance" or "land -disturbing activity" means a man-made change to the land surface that may result in soil erosion or has the potential to change its runoff characteristics, including construction activity such as the clearing, grading, excavating, or filling of land per §62.1-44.15:24. Definitions. D. If this project is using approved Annual Standards and Specifications (AS&S), attach the completed AS&S Entity Form. The Meadowville Loam on -site is classified within hydrologic groups A, which can have moderate to rapid runoff potential when thoroughly wet. The Dyke Silt Loam, Hayesville Loam, and Unison Silt Loam on -site are classified within hydrologic group B, which can have moderate to rapid runoff potential when thoroughly wet. • See sheet ES 1.0 in Appendix M for soils map. Slopes (describe current slopes and note any changes due to grading or fill activities): • There are some areas of moderate slopes on -site within the project area (7-15% slopes) Drainage Patterns (describe current drainage patterns and note any changes due to grading or fill activities): • Pre -Construction: Stormwater from the site sheetflows across the site. Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property Erosion & Sediment Control BMPs: Minimize Disturbed Area and Protect Natural Features and Soil. Describe the areas that will be disturbed with each phase of construction and the methods (signs, fences, etc.) that you will use to protect those areas that should not be disturbed. Describe natural features identified earlier and how each will be protected during construction activity. Also describe how topsoil will be preserved. Include these areas and associated BMPs on your site map(s) also. Maximum disturbed area is detailed in the site plan. BMP Description: Topsoil Stockpiles Topsoil stripped from the immediate construction area will be stockpiled. The stockpile will be in an area that will not interfere with construction phases and at least 15 feet away from areas of concentrated flows of pavement. The slopes of the stockpile will not exceed 2:1 to prevent erosion. A silt fence will be installed around the perimeter of the stockpile. See E&S plan- Sheet ES2.0 in the Appendices for stockpile location. Installation Schedule: Topsoil stockpiles will be established during grading activities. Temporary stabilization will be applied immediately after the slopes of the stockpile have been graded and construction equipment transverses the slopes. Maintenance and Inspection: The area will be inspected weekly for erosion and immediately after storm events. Areas on or around the stockpile that have eroded will be stabilized immediately with erosion controls. Maintenance and inspection procedures for the silt fence. Responsible Staff: Responsible Land Disturber see page 2 Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property BMP Description: Tree Protection • Temporary Fencing: Install temporary fencing around tree protection zones to protect remaining trees and vegetation from construction damage. Maintain temporary fence and remove when construction is complete. Fencing should be the last item removed after completion of a project. This fencing will be erected at a radius of 1.50 feet away from the trunk of tree for every inch in stem diameter. Diameter is measured 4.5 feet above the ground. For trees 4 inches or less in diameter the measurement will be at 6 inches above the ground. This radial distance is termed the "critical root zone" and will serve as the Tree Protection Zone (TPZ). Fencing will consist of heavy-duty chain link. Fencing will be rigidly supported and maintained during all construction periods at a minimum height of 6.0 feet above grade. • Signs stating, "No Entry, Tree Protection Area," in both English and Spanish, are to be posted at 30-foot intervals. • Protect tree root systems from damage caused by runoff or spillage of noxious materials while mixing, placing, or storing construction materials. • Protect root systems from ponding, eroding, or excessive wetting caused by watering operations. • Mulch areas inside tree protection zones and other areas indicated. o Apply 4-inch thickness of organic mulch. Do not place mulch within 6 inches of the tree trunk to allow the trunk to breathe. • Do not store construction materials, debris, or excavated material inside tree protection zones. • Do not permit vehicles or foot traffic within tree protection zones; prevent soil compaction over root systems. • Maintain tree protection zones free of weeds and trash. Installation Schedule: Tree protection fencing should be installed prior to construction activities within the work area. Maintenance and Areas should be checked regularly to identify problems and Inspection: determine if additional controls (e.g., more training, more explicit signage, more obvious barriers) are needed. Inspectors should also look for signs of unintended consequences of construction activities on the natural areas, such as changes in hydrology, flooding, or accidental spills, and take appropriate actions to mitigate the damage. Staff: Personnel Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property TP TREE PROTECTION Z Control Stormwater Flowing onto and Through the Project: Describe structural practices (i.e., diversions, berms, ditches, storage basins) including design specifications and details used to divert flows from exposed soils, retain or detain flows, or otherwise limit runoff and the discharge of pollutants from exposed areas of the site. 3. Stabilize Soils: Describe controls (i.e., temporary seeding with native vegetation, hydroseeding, etc.) to stabilize exposed soils where construction activities have temporarily or permanently ceased. Also describe measures to control dust generation. Use of impervious surfaces for stabilization should be avoided whenever possible. BMP Description: Temporary Seeding The establishments of a temporary vegetative cover on disturbed areas by seeding with appropriate rapidly growing annual plants. This protects slopes exposed during construction until permanent vegetative cover can be established. Temporary seeding will apply to all areas on the site that will not be brought to final grade for periods of thirty (30) days to one year. Selection of the appropriate seed mixture depends upon the time of year it is to be applied see schedule on E&S details sheet — ES3.0. Installation Schedule: Temporary seeding shall be applied to all denuded areas prior to permanent stabilization in the form of either hardscape or permanent seeding. Temporary seeding shall be used on all fill slopes. If failure is noticed at the seeded area, the area will be reseeded immediately. After construction is completed at the site, permanently stabilized areas will be monitored until final stabilization is reached Responsible Staff: Responsible Land Disturber see page 2 BMP Description: Permanent Seeding Permanent seeding will be applied after the final design grades are achieved on portions of the site but no later than 14 days after construction activities have permanently ceased. After the entire site is stabilized, any sediment that has accumulated will be removed and hauled off -site for disposal at the Ivy Material Utilization Center. Construction debris, trash and temporary BMPs (including silt fences, material storage areas, sanitary toilets, and inlet protection) will also be removed and any areas disturbed during removal will be seeded immediately. Seedbed Preparation a. In areas where disturbance results in subsoil being the final grade surface, topsoil will be spread over the fmished area at minimum depth of 2 to 6 inches. b. The seedbed will be free of large clods, rocks, woody debris and other objectionable materials. c. Fertilizer and lime will be applied to the seedbed according to the manufacturer's recommendations or soil tests. d. The top layer of soil will be loosened to a depth of 3-5 inches by raking, tilling, disking or other suitable means. Grass Selection/Application a. Common areas at the site will be stabilized with a mixture of Tall Fescue, Creeping Red Fescue and Redtop at an application rate of 30 pounds per acre or 0.95 pounds per 1,000 square feet. Lawns will be stabilized with a mixture of Kentucky Blue Grass and Creeping Red Fescue at an application rate of 100 pounds per acre or 2.3 pounds per 1,000 square feet. b. Seed will be applied uniformly by hydroseeding or broadcasting. Where broadcasting Installation Schedule: Portions of the site where construction activities have permanently ceased will be stabilized, as soon as possible but no later than 14 days after construction ceases. Maintenance and Inspection: All seeded areas will be inspected weekly during construction activities for failure and after storm events until a dense cover of vegetation has been established. If failure is noticed at the seeded area, the area will be reseeded, fertilized, and mulched immediately. After construction is completed at the site, permanently Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property stabilized areas will be monitored until final stabilization is reached. Responsible Staff: Responsible Land Disturber (see page 2) BMP Description: Dust Control Dust from the site will be controlled by using a mobile pressure -type distributor truck to apply potable water to disturbed areas. The mobile unit will apply water at a rate of 300 gallons per acre and minimized as necessary to prevent runoff and ponding. Installation Schedule: Dust control will be implemented as needed once site grading has been initiated and during windy conditions (forecasted or actual wind conditions of 20 mph or greater) while site grading is occurring. Spraying of potable water will be performed no more than three times a day during the months of May -September and once per day during the months of October -April or whenever the dryness of soil warrants it. Maintenance and Inspection: At least one mobile unit shall be available at all times to distribute potable water to control dust on the project area. Each mobile unit shall be equipped with a positive shutoff valve to prevent over watering of the disturbed area. Protect Slopes: Erosion control blankets and matting, tackifiers, etc. are not being proposed at this time for the scope of this project. BMP Description: Soil Stabilization Blankets & Matting Geotextile erosion control blankets will be used to provide stabilization for the trenches. The blanket will cover the entire area of the graded slope. The blanket will be installed by digging a small trench on the upside of the slope, 12 inches wide by 6 inches deep, and stapling the leading edge of the blanket in the trench. The blanket will be rolled down the slope slowly to maintain soil contact and stapled in 12-inch intervals. If the blanket cannot cover the entire slope, the blankets will be overlapped (minimum of 2 inches) and stapled at the overlapped edge. The erosion control blanket will always be installed according to manufacturer's instructions andspecifications. Installation Schedule: The erosion control blankets will be installed once the soil has reached final grade. Maintenance and Inspection: The erosion control blanket will be inspected weekly and immediately after storm events to determine if cracks, tears, or breaches have formed in the fabric; if so, the blanket will be repaired or replaced immediately. Good contact with the soil must be maintained and erosion should not occur under the blanket. Any areas where the blanket is not in close contact with the ground will be repaired or replaced. Protect Storm Drain Inlets: Describe controls (i.e., inserts, rock -filled bags, or block and gravel, etc.) including design specifications and details that will be implemented to protect all inlets receiving stormwater from the project during the entire duration of the project. BMP Description: Outlet Protection Structurally lined Outlet protection used for dissipating energy placed at the outlets of pipes or paved channel sections. Outlet protection is used to prevent scour at stormwater outlets and minimize downstream erosion. See ESC narrative (Section 4) for design details and placement. Installation Schedule: The outlet protection will be established when the inlet drainage pipe is installed. This form of protection shall be performed on all stormwater outlets by using ri ra . Maintenance and Inspection: Inspect to ensure against scour. Responsible Staff: Responsible Land Disturber see page 2 Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property 6. Establish perimeter controls and sediment harriers: Describe structural practices (i.e., silt fences or fiber rolls) including design specifications and details to filter and trap sediment before it leaves the construction site. BMP Description: Silt Fence Silt fences will be installed along the disturbed area as shown on the site plan. Silt fences will be installed by excavating a 12-inch-deep trench along the line of proposed installation. Wooden posts supporting the silt fence will be spaced 4 to 6 feet apart and driven securely into the ground: a minimum of 18 to 20 inches deep. The silt fence will be fastened securely to the wooden posts with wire ties spaced every 24 inches at the top, mid -section, and bottom of the wooden post. The bottom edge of the silt fence will be extended across the bottom of the trench and the trench will be backfilled and compacted to prevent stormwater and sediment from discharging underneath the silt fence. Installation Schedule: The silt fence will be installed before construction begins at the site and silt fence locations are illustrated on the plan Section 4. Maintenance and Inspection: Silt fences will be inspected weekly and immediately after storm events to ensure it is intact and that there are no gaps where the fence meets the ground or tears along the length of the fence. If gaps or tears are found during the inspection, the fabric will be repaired or replaced immediately. Accumulated sediment will be removed from the fence base if it reaches one-third the height of the silt fence and hauled off -site for disposal. If accumulated sediment is creating noticeable strain on the fabric and the fence might fail from a sudden storm event, the sediment will be removed more frequently. Before the fence is removed from the project area, the sediment will be removed. The anticipated life span of the silt fence is 6 months and will likely need to be replaced after this period. Responsible Staff: Responsible Land Disturber see page 2 Design Specifications 1. The silt fence will be constructed long enough to extend across the expected flow path. 2. The support posts will be a minimum of 4.5 feet and driven a minimum of 18 to 20 inches in the ground. Posts will be spaced a maximum of 6 feet apart. Fabric will be securely fastened to posts with half -inch staples or 16-gauge wire ties spaced a maximum of 6 inches. 3. A 12-inch trench will be excavated along the uphill side of the silt fence posts. The bottom edge of the fabric will extend across the bottom of the trench. The trench will be backfilled to 4 inches above ground and compacted to bury and secure the bottom of the filter fabric. Stormwater Pollution Prevention Plan (SWPPP) Bundoran Farm: Lot F 11 Property 7. Retain Sediment On -Site and Control Dewatering Practices: Describe sediment control practices (i.e., sediment trap or sediment basin), including design specifications and details (volume, dimensions, outlet structure) that will be implemented at the construction site to retain sediments on -site. Describe dewatering practices that will be implemented if water must be removed from an area so that construction activity can continue. 8. Establish Stabilized Construction Faits: Describe location(s) of vehicle entrance(s) and exit(s), procedures to remove accumulated sediment off -site (i.e., vehicle tracking), and stabilization practices (i.e., stone pads and/or wash racks) to minimize off -site vehicle tracking of sediments and discharges to stormwater. BMP Description: Temporary Stone Construction Entrance A stone entrance shall be constructed utilizing existing driveways, as identified on the site map, to prevent off -site transport of sediment by construction vehicles. The stone entrance will be at least 70 feet long and a minimum of 10 feet wide. This stone area shall be used for washing sediment from vehicles exiting the site. Where sediment is transported onto a public road surface, the road shall be cleaned thoroughly at the end of each day. Sediment shall be removed from the roads by shoveling or sweeping and transported to a sediment control disposal area. Street washing shall be allowed only after sediment is removed in this manner. Installation Schedule: The stabilized exits will be installed before construction begins and shall be adjacent to Carpenter Drive as shown on the Erosion & Sediment Control plan ES2.0 . Maintenance and Inspection: The exit will be inspected weekly and after storm events or heavy use. The exit will be maintained in a condition that will prevent tracking or flowing of sediment onto Carpenter Drive. All sediment tracked, spilled, dropped, or washed onto Carpenter Drive will be swept up immediately and hauled off -site for disposal at the landfill. Broken road pavement as a result of construction activities on roadways immediately adjacent to the project site will be repaired immediately. Alternate Method: The County inspector may, at their discretion, require the use of a paved construction entrance as an alternate to the stone construction entrance. Additional BMPs: Describe additional BMPs that may not fit into the above categories. BMP Description: Surface Roughening This erosion and sedimentation control measure involves creating horizontal depressions on slopes or leaving slopes in a roughened condition by not fine -grading them. This is done to aid in the establishment of vegetative cover or seed, to reduce runoff velocity and increase infiltration, and to reduce erosion and provide for sediment trapping. Stair -step grades or groove -cut slopes will be used for gradients steeper than 3:1. Stair -step grading on any erodible material that is soft enough to be ripped with a bulldozer. Also, it is well suited for slopes consisting of soft rock with some subsoil. Make the vertical cut distance less than the horizontal distance, and slope the horizontal portion of the step slightly toward the vertical wall. Keep individual vertical cuts less than 2 feet deep in soft materials and less than 3 feet d ep in rocky materials. Installation Schedule: Roughening shall occur after vegetation has been removed from the slope after grading. Maintenance and Inspection: The surface roughening will be inspected to ensure that the proper sloping is achieved after stair -stepping concludes. Responsible Staff: Responsible Land Disturber see page 2 E E 0 EROSION AND SEDIMENT CONTROL NARRATIVE Bundoran Farm Lot F-11 Site Plan: Erosion and Sediment Control Plan The County of Albemarle, Virginia PROJECT DESCRIPTION The scope of this E&S narrative and plan is for the purpose of developing soil in preparation for a single-family residence, select utility work,and a new septic drainfield. A total of approximately 1.8 acres will be disturbed during construction. EXISTING SITE CONDITIONS The existing topography for the site is hilly with vegetation throughout. A portion of the site is being used as a vineyard. A portion of the site lies within the stream buffer. ADJACENT AREAS The project site is located along Carpenter Drive. The surrounding properties are also zoned RA residential. OFF -SITE AREAS All land disturbing activities for this plan shall occur on -site and within the limits of disturbance shown on the plans. There are no off -site borrow and waste locations established. If applicable, separate erosion control plans will be required for off -site land disturbing activities. SOILS This site is located in a geologic area referred to as the Piedmont Physiographic Province. The soils in this geologic area are weathered from the underlying metamorphic bedrock. The naturally occurring soils at this site were found to be moderately to highly plastic silts weathered from the Catoctin formation which consists of basic lava flows, schist, and gneiss composed of chlorite, plagioclase, amphiobole, and epidote. The soils observed here appear to be weathered from metamorphosed sedimentary soils such as phyllite and schist. The map units on the detailed soil map depicted on the Erosion and Sediment Control Narrative, represent the soils in the survey area. The map unit descriptions, along with the soil maps, can be used to determine the suitability and potential of a soil for specific uses, and, precautionary erosion and sedimentation control measures which must be adhered to. They also can be used to plan for the management needed for those uses. The profile of the soil classifications follows: Dyke Silt Loam (25B): This deep, gently sloping (2-7%), well drained soil is on broad convex foot slopes and colluvial fans. Typically, the surface layer is dark reddish brown silt loam about eight (8) inches thick. The upper thirty-five (35) inches of the subsoil is dark red clay loam and clay, and the lower part to a depth of sixty (60) or more is dark red clay. Permeability and available water capacity are moderate. Shrink -swell potential is moderate. Runoff is medium, and the hazard of erosion is moderate. Bedrock is generally at a depth of five (5) feet or more. Dyke Silt Loam (25C): This deep, strongly sloping (7-15%), well drained soil is on convex side slopes and colluvial fans. Typically, the surface layer is dark reddish brown silt loam about eight (8) inches thick. The upper thirty-five (35) inches of the subsoil is dark red clay loam and clay, and the lower part to a depth of sixty (60) or more is dark red clay. Permeability and available water capacity are moderate. Shrink -swell potential is moderate. Runoff is rapid, and the hazard of erosion is severe. Bedrock is generally at a depth of five (5) feet or more. Hayesville Loam (36D): This deep, moderately steep (15-25%), well -drained soil is on side slopes that border small drainageways. Typically, the surface layer is brown and strong brown loam about seven (7) inches thick. The subsoil is red clay and clay loam about fifty-one (51) inches thick. The substratum is mostly multicolored sandy clay loam. Permeability and available water capacity are moderate. Shrink -swell potential is low. Runoff is rapid, and the hazard of erosion is severe. Bedrock is generally at a depth of five (5) feet or more. Meadowville Loam (56B): This deep, gently sloping (2-7%), well -drained to moderately well -drained soil is on footslopes, in depressions, and along small drainageways. Typically, the surface layer is dark brown loam about forteen (14) inches thick. The subsoil is brown loam and clay loam about twenty-three (23) inches thick. The substratum is brown loam. Permeability is moderate to moderately rapid, and available water capacity is high. Shrink -swell potential is moderate. Runoff is medium and the hazard of erosion is moderate. Bedrock is generally at a depth of five (5) feet or more. Meadowville Loam (56C): This deep, strongly sloping (7-15%), well -drained to moderately well -drained soil is on footslopes, in depressions, and along small drainageways. Typically, the surface layer is dark brown loam about forteen (14) inches thick. The subsoil is brown loam and clay loam about thirty-two (32) inches thick. The substratum is brown loam. Permeability is moderate to moderately rapid, and available water capacity is high. Shrink -swell potential is moderate. Runoff is rapid and the hazard of erosion is severe. Bedrock is generally at a depth of five (5) feet or more. Unison Silt Loam (89B): This deep, gently sloping (2-7%), well -drained soil is on broad colluvial fans and mountain foot slopes. Typically, the surface layer is dark brown silt loam about six (6) inches thick. The subsoil is reddish brown and yellowish red silty clay loam and clay about fifty-two (52) inches thick. The substratum is strong brown cobbly clay loam. Permeability and available water capacity are moderate. Shrink -swell potential is moderate. Runoff is medium and the hazard of erosion is moderate. Bedrock is generally at a depth of five (5) feet or more. CRITICAL AREAS There are a few areas of steep slopes on -site that are classified by the County of Albemarle as "Critical Slopes." A portion of the property lies within a stream buffer and a portion of the site is within the 100-Year Storm floodplain. EROSION AND SEDIMENT CONTROL MEASURES The contractor shall provide, construct and maintain erosion and sediment control measures on the site, in accordance with the Virginia Department of Transportation and the County of Albemarle erosion and sediment control measures (refer to the Erosion and Sediment Control Plan sheet). All vegetative and structural erosion and sediment control practices shall be constructed and maintained according to minimum standards and specifications of the most current version of the Virginia Erosion and Sediment Control Handbook. Permanent or temporary soil stabilization shall be applied to denuded areas within seven (7) days after final grade is reached on any portion of the site. Temporary soil stabilization shall be applied within seven (7) days to denuded areas that may not be at final grade but will remain dormant (undisturbed) for longer than 30 days. Permanent stabilization shall be applied to areas that are to be left dormant for more than one year. Soil stabilization refers to measures that protect soil from the erosive forces of raindrop impact and flowing water. Applicable practices include vegetative establishment, mulching, and the early application of gravel base on areas to be stone. Soil stabilization measures should be selected to be appropriate for the time of the year, site conditions, and estimated duration of use. Sediment basins and traps, perimeter dikes, sediment barriers and other measures intended to trap sediment shall be constructed as a first step in any land -disturbing activity and shall be made functional before upslope land disturbance takes place. Stabilization measures shall be applied to earthen structures such as dams, dikes and diversions immediately after installation. Where construction vehicle access routes intersect paved public roads, provisions shall be made to minimize the transport of sediment by vehicular tracking onto the paved surface. Where sediment is transported onto a public road surface, the road shall be cleaned thoroughly at the end of each day. Sediment shall be removed from the roads by shoveling or sweeping and transported to a sediment control disposal area. Street washing shall be allowed only after sediment is removed in this manner. This provision shall apply to individual subdivision lots as well as to larger land -disturbing activities. The following structural practices will be implemented according to the requirements set forth by the Virginia Erosion and Sediment Control. Safety Fence (3.01) A protective barrier consisting of polyethylene web secured to a conventional metal post driven into the ground a minimum of 18 inches. Posts should be spaced at 6-foot centers. Used to prevent undesirable use of an erosion control measure by the public. To prevent access to an erosion control measure. Specific Application., Safety fence will be used at areas where pedestrians may come into contact with the planned work. Stone Construction Entrance (3.02) A seventy (70) feet stone entrance shall be placed at the point of vehicular ingress and egress on the construction site. This area shall be used for washing sediment from vehicles exiting the site. Where sediment is transported onto a public road surface, the road shall be cleaned thoroughly at the end of each day. Specific Application: A construction entrance shall be utilized where shown on plan. cL Silt Fence (3.05) A temporary sediment barrier consisting of synthetic filter fabric stretched across and attached to supporting posts and entrenched. Used to prevent sediment from leaving the site. To decrease the velocity of sheet flows. Specific Application: Silt fence will be used where shown on the plan. dam. Inlet Protection (3.07) A temporary sediment filter around a storm inlet or curb inlet, in order to prevent sediment from entering storm drainage systems prior to permanent stabilization of the disturbed area. Specific Application: Inlet protection will be used around storm drainage inlets and curb inlets. 1L Outlet Protection (3.18) Structurally lined aprons or other acceptable energy dissipating devices placed at the outlets of pipes or paved channel sections. To prevent scour at stormwater outlets and minimize downstream erosion. Specific Application: This form of protection shall be performed on all stormwater outlets by using riprap. Rock Check Dam (3,20) Small temporary stone dams constructed across a swale or drainage ditch to reduce the velocity of concentrated stormwater flows. Specific Application: This practice is limited to use in small open channels that drain 10 acres or less. g).Surface Roughening (3.29) This erosion and sedimentation control measure involves creating horizontal depressions on slopes or leaving slopes in a roughened condition by not fine -grading them. This is done to aid in the establishment of vegetative cover or seed, to reduce runoff velocity and increase infiltration, and to reduce erosion and provide for sediment trapping. Specific Application: This form of protection shall be performed on all slopes 3:1 or greater as shown on the accompanying E&S Plan. Slopes less steep than 3:1 should have the soil lightly roughened and loose to a depth of 24" prior to seeding. h).Temporary Seeding (3.31) The establishment of a temporary vegetative cover on disturbed areas by seeding with appropriate rapidly growing annual plants. This protects slopes exposed during construction until permanent vegetative cover can be established. Temporary seeding will apply to all areas on the site that will not be brought to final grade for periods of thirty (30) days to one year. Selection of the appropriate seed mixture depends upon the time of year it is to be applied (See schedule on E&S details sheet.) Specific Application: Temporary seeding shall be applied to all denuded areas prior to permanent stabilization. Temporary seedingshall be used on all fill slopes and diversions. P D. Soil stabilization Blankets & Matting (3.36) Stabilizing disturbed areas of a steep slope, channel, or a shoreline by applying protective covering on a prepared area. The matting will protect the young vegetation and promote its establishment while increasing the permissible velocity of turf grass stands. Specific Application: Where concentrated stormwater flows overland to the existing basin. Where deemed necessary by the Responsible Land Disturber or County Inspector. Dust Control (3.39) Reducingsurface air movement of dust during land disturbances demolition or construction activities in areas subject to dust 9 1 problems in order to prevent soil loss and reduce the presence of potentially harmful airborne substance. Specific Application: Dust control shall be applied as necessary. PERMANENT STABILIZATION All areas disturbed by construction will be stabilized with permanent seeding immediately following finish grading. All seeds will be approved by the "Virginia Crop Improvement Association," Blacksburg Virginia. All seeding, excluding the stormwater management basin will be done with the following: Two (2) parts Kentucky Bluegrass (Poa pratensis), by weight; minimum 98 % purity and 90% germination; maximum 1 % weed seed. Three (3) parts perennial Ryegrass (Labium perrene) by weight, minimum 85% purity and 90% germination; maximum 1 % weed seed. Fifteen (15) parts Falcon Fescue; minimum 98% purity and 90% germination; maximum 1 % weed seed. Note: See the Stormwater Management Plan for special seeding of the basin. Permanently seeded areas shall be protected during establishment. Mulch shall consist of oat or wheat straw, free from weeds, foreign matter detrimental to plant life, and dry. Hay or chopped cornstalks will not be permitted. Mulch and/or lime shall be applied at a rate of two (2) tons per acre. Specifications for fertilizer are FS 0-F-241 C, Type 1, solid form; Grade A or B; recommended for grass, with one hundred (100) percent of the elements derived from organic sources of the following properties: a) Nitrogen 12.0% b) Phosphoric Acid 8.0% c) Soluble Potash 8.0% With at least fifty (50) percent of the nitrogen from a non -water soluble organic source. The following measures shall be incorporated during fertilization: Apply fertilizer at a rate of 1000lbs./acre. Apply after smooth raking of topsoil. Do not apply fertilizer at same time or with same machine as will be used to apply seed. Till fertilizer into upper two (2) inches of topsoil. BMP Description: Waste Materials All waste materials will be collected and disposed of into metal trash dumpsters in the materials storage area. Dumpsters will have a secure watertight lid, be placed away from stormwater conveyances and drains, and meet all federal, state, and municipal regulations. Only trash and construction debris from the site will be deposited in the dumpster. No construction materials will be buried on -site. All personnel will be instructed, during tailgate training sessions, regarding the correct disposal of trash and construction debris. Notices that state these practices will be posted in the office trailer and the individual who manages day -today site operations will be responsible for seeing that these practices are followed. Construction dumpsters shall be covered and secured during every rain event and before vacating the site for more than 24 hours. Installation Schedule: Trash dumpsters will be installed once the materials storage area has been established. Maintenance and Inspection: The dumpsters will be inspected weekly and immediately after storm events. The dumpster will be emptied weekly and taken to the Van Der Linde facilities. Hazardous waste materials will be stored in appropriate and clearly marked containers and segregated from other non - waste materials. Secondary containment will be provided for all waste materials in the hazardous materials storage area and will consist of commercially available spill pallets. Additionally, all hazardous waste materials will be disposed of in accordance with federal, state, and municipal regulations. Hazardous waste materials will not be disposed of into the on -site dumpsters. All personnel will be instructed, during tailgate training sessions, regarding proper procedures for hazardous waste disposal. Notices that state these procedures will be posted in the office trailer and the individual who manages day-to-day site operations will be responsible for seeing that these procedures are followed. Installation Schedule: Shipping containers used to store hazardous waste materials will be installed once the site materials storage area has been installed. Maintenance and Inspection: The hazardous waste material storage areas will be inspected weekly and after storm events. The storage areas will be kept clean, well -organized, and equipped with ample cleanup supplies as appropriate for the materials being stored. Material safety data sheets, material inventory, and emergency contact numbers will be maintained in the office trailer. Installation Schedule: The portable toilets will be brought to the site once the staging area has been established Maintenance and Inspection: All sanitary waste will be collected from the portable facilities a minimum of three times per week. The dumpster will have a secure watertight lid, be placed away from stormwater conveyances and drains and meet all local and state solid -waste management regulations. Only solid recyclable construction scraps from the site will be deposited in the dumpster. All personnel will be instructed, during tailgate training sessions, regarding the correct procedure for disposal of recyclable construction scraps. Notices that state these procedures will be posted in the office trailer, and the individual who manages day-to-day site operations will be responsible for seeing that these procedures are followed. Installation Schedule: Designated recycling dumpsters will be installed once the combined staging area has been established. BMP Description: Materials Storage Area Construction equipment and maintenance materials will be stored at the combined staging area and materials storage areas. Gravel bag berms will be installed around the perimeter to designate the staging and materials storage area. A watertight shipping container will be used to store hand tools, small parts, and other construction materials. Nonhazardous building materials such as packaging material (wood, plastic, and glass), and construction scrap material (brick, wood, steel, metal scraps, and pipe cuttings) will be stored in a separate covered storage facility adjacent to the shipping container. All hazardous -waste materials such as oil filters, petroleum products, paint, and equipment maintenance fluids will be stored in structurally sound and sealed containers, under cover within the hazardous materials storage area. Very large Items, such as framing materials and stockpiled lumber, will be stored in the open in the materials storage area. Such materials will be elevated on wood blocks to minimize contact with runoff. Installation Schedule: The materials storage area will be installed after grading and prior to any infrastructure constructed at the site. Maintenance and Inspection: The storage area will be inspected weekly and after storm events. The storage area will be kept clean, well - organized, and equipped with ample cleanup supplies as appropriate for the materials being stored. Perimeter controls, containment structures, covers, and liners will be repaired or replaced as needed to maintain proper function. Designate Washout Areas: Describe location(s) and controls to minimize the potential for stormwater pollution from washout areas for concrete mixers, paint, stucco, etc. BMP Description: Concrete Washout The temporary concrete washout area will be constructed with a recommended minimum length and minimum width of 10 feet, but with sufficient quantity and volume to contain all liquid and concrete waste generated by washout operations. The washout area will be lined with plastic sheeting at least 10 mils thick and free of any holes or tears. Signs will be posted marking the location of the washout area to ensure that concrete equipment operators use the proper facility. Concrete pours will not be conducted during or before an anticipated storm event. Concrete mixer trucks and chutes will be washed in the designated area or concrete wastes will be properly disposed of off -site. When the temporary washout area is no longer needed for the construction project, the hardened concrete and materials used to construct the area will be removed and disposed of according to the maintenance section below and the area will be stabilized. Installation Schedule: The washout area will be constructed before concrete ours occur at the site. Maintenance and Inspection: The washout areas will be inspected daily to ensure that all concrete washing is being discharged into the washout area, no leaks or tears are present, and to identify when concrete wastes need to be removed. The washout areas will be cleaned out once the area is filled to 75 percent of holding capacity. Once the area's holding capacity has been reached, the concrete wastes will be allowed to harden; the concrete will be broken up, removed, and taken to the landfill for disposal. The plastic sheeting will be replaced if tears occur during removal of concrete wastes from the washout area. Establish proper equipment/vehicle fueling and maintenance practices: Describe equipment/vehicle fueling and maintenance practices that will be implemented to control pollutants to stormwater (e.g., secondary containment, drip pans, spill kits, etc.) BMP Description: Vehicle/Equipment Fueling and Maintenance Several types of vehicles and equipment may be used on -site throughout the project, including graders, scrapers, excavators, loaders, paving equipment, rollers, trucks and trailers, backhoes, and forklifts. All major equipment/vehicle fueling, and maintenance will be performed off -site. A small, 20-gallon pickup bed fuel tank will be kept on -site in the combined staging area. When vehicle fueling must occur on -site, the fueling activity will occur in the staging area. Only minor equipment maintenance will occur on -site. All equipment fluids generated from maintenance activities will be disposed of into designated drums stored on spill pallets in accordance with Part 3.1. Absorbent, spill -cleanup materials and spill kits will be available at the combined staging and materials storage area. Drip pans will be placed under all equipment receiving maintenance and vehicles and equipment arked overnight. Installation Schedule: BMPs implemented for equipment and vehicle maintenance and fueling activities will begin at the start of the project. Maintenance and Inspection: Inspect equipment/vehicle storage areas and fuel tank weekly and after storm events. Vehicles and equipment will be inspected on each day of use. Leaks will be repaired immediately, or the problem vehicle(s) or equipment will be removed from the project site. A fee of $176.80 will be charged for the first reinspection and $280.80 for each subsequent reinspection that is necessary. 3. A temporary stone construction entrance must be provided wherever traffic will be leaving the construction site and where there exists the possibility of transporting mud directly onto a road or other paved area. 4. All denuded areas on the site must be stabilized within seven (7) days of final grading with permanent vegetation or a protective ground cover suitable for the time of year. Temporary soil stabilization must be applied within seven (7) days to denuded areas that may not be at final grade but will remain dormant (undisturbed) for longer than thirty (30) days. 5. A permanent stabilization/vegetative cover must be established on all denuded areas within nine (9) months of the start of clearing or grading. Unless documented in writing to the Program Authority at the start of land disturbance, the nine (9) months will be measured from the permit issuance date. If an extension is necessary and construction extends for more than a year, a permit maintenance fee of $170 will be required. 6. All storm sewer inlets that are operable during construction must be protected so that sediment -laden water cannot enter the storm water conveyance system without first being filtered or otherwise treated to remove sediment. 8. All temporary erosion and sediment control measures must not be removed until disturbed areas are stabilized. After stabilization is complete, all measures must be removed within 30 days, unless otherwise authorized by the Program Authority. Trapped sediment and the disturbed soil areas resulting from the disposition of temporary measures must be permanently stabilized to prevent further erosion and sedimentation. The Contractor will conduct inspections and the Contractor will bear primary responsibility for inspections of all Erosion and Sediment Control devices and pollution prevention measures to ensure continuing effectiveness. The inspector must be considered Qualified Personnel. Inspections must include all areas of the site disturbed by construction activity, off -site areas (covered by the permit), and areas used for storage of materials that are exposed to precipitation. Inspectors must look for evidence of, or the potential for, pollutants entering a stormwater conveyance system. Control measures identified in the SWPPP shall be inspected for proper installation, maintenance, and operation. Discharge locations, where accessible, shall be inspected to ascertain whether control measures are effective in minimizing impacts to receiving waters. Where discharge locations are inaccessible, nearby downstream locations shall be inspected to the extent that such inspections are practicable. Locations where vehicles enter or exit the site shall be inspected for evidence of off -site sediment tracking. iii. Major observations should include: the location(s) of discharges of sediment or other pollutants from the site; amount of rainfall at the construction site since the previous inspection; weather information and a description of any discharges occurring at the time of inspection; location(s) of control measures that need to be maintained; location(s) of control measures that failed to operate as designed or proved inadequate for a particular location; and location(s) where additional control measures are needed that did not exist at the time of inspection. iv.
